Electronic health records, personal health records, home monitoring, social media, and mobile apps are transforming healthcare through increased access to health data and communication. EHRs improve care coordination and quality by providing consistent patient histories while PHRs empower patients by giving them access to their own health information. Home monitoring devices and social media enhance care by facilitating remote monitoring and peer support. Mobile apps are increasingly used for health tracking and information access as most Americans now have smartphones. Overall, these technologies are shifting care delivery outside of hospitals and empowering patients through greater engagement with their health data and doctors.
