Editordiscussion about the healthcare sector in India is
Aincomplete without the mention of Diagnostic and
Pathology Services. To ensure complete healthcare,
harmony must be in place between the medical service
providers and the diagnostic and pathology service providers.
The two sectors complement each other and help form the
basis of a healthy society.
The diagnostic industry in India has grown with the development
of medical sciences. It has come a long way from simple blood
tests to the ultra-modern scans, hormone tests, and screening
tests. With growing health-consciousness, the demand of such
centers has also increased. With India’s growing image as a
medical tourism country, the role of reliable and certified
diagnostic centers becomes even more crucial.
The major market share of the diagnostic industry is currently
held by the unorganized centers which cater to the small and
medium-sized towns and cities. The Tier I and Tier II cities are
mostly catered by highly specialized and organized centers. This
skewed arrangement at times reflects the level of awareness
that the consumers have in these cities who demand and are
keen on utilizing best-in-class services.
On the other hand, it also represents the disparity in providing
services by the diagnostic centers who tend to have an
inclination towards monetary benefits instead of just human-
service. It certainly is true that to provide world-class services,
one needs to invest heavily in setting up the proper
infrastructure. But the other side of the coin is that unless and
until the services reach the last mile, the efficiency of an industry
is always under scrutiny.
On a brighter note, many diagnostic centers are realizing their
social responsibility and catering to the rural areas as well. They
often organize health-check up camps and utilize other means
to reach out to the lowest strata of society. The Government and
the NGOs also assist and promote the diagnostic industry to
ensure that health and diagnostic services reach all and sundry.
With the advancement in technology, easy access to
information, and a customer-centric approach, the face of the
diagnostic industry is changing for good. The customer is aware
of what is acceptable and what is not. The industry too, is aware
of the informed demands that the customer makes. The
Government too is aware of the changes required in the
regulatory scenario that can transform the industry for good.
This amalgamation hints at a future where more accurate
reports, right diagnosis and disease management, and in-turn
better healthcare costs will be made available for the masses.

Inquisitiveness is a primary trait of human beings. The
human brain which processes millions of thoughts in a
flash of time needs to know about everything and
anything around it. Human well being or fitness is one such
area that has grown by leaps and bound over centuries given
to the human quest to stay healthy. Specific to it is the study
of diseases, their cause and effect which comes under the
broad spectrum of Pathology. It involves the study of
diseases, what causes them and often describe or define
tests and processes that need to be followed to ascertain a
particular disease. As Louis Pasteur says, “Without
Laboratories men of science are soldiers without arms”.
The origin of this science dates back to early civilization as
discovered in their records, found in several ruins. India, the
Middle East and China are credited with the initial
development, and ancient Greece is where detailed and the
present formal form of the science took shape. But the
current situation of pathology and diagnostic services in
India is no mirror to its ancient glory. While India has now
become a highly appreciated medical tourism spot with
many advanced and globally accredited diagnostic centres,
the fact remains that the infrastructure at places is severely
lacking.
India has around 100,000 plus diagnostic centres which
include pathology and radiology. As per one market report,
approx. 62% of the diagnostic market is shared by private
sector while 38% is in Govt sector. Of the entire private
diagnostic market approx. 68% is from pathology while
32% is from radiology. Also, approx. 51% of private
diagnostic market is in hospital-based centres, 47% is in
independent centres and remaining comes from small
polyclinics. Only about 20% of them are a part of the
organized pathology providers. The remaining 80% of the
industry is highly fragmented, unregulated and
unfortunately, most do not adhere to minimum standard
requirements. Though there are few pathologists owned,
standalone labs which are very well organized and high
quality driven, but they form just a fraction of total
pathology centres.
Midst all this commotion, there are a few pathology centres
which outshine owing to their high skilled workforce,
advance technology driven platforms, strong commitment
to deliver with globally accepted quality standards and with
a continuous zeal to excel.

stabilized, Mr Burman decided to go for the bigger market

30% year-on-year. Dr. Ravi Gaur, Chief Operating
Officer of Oncquest laboratories, a post graduate in
pathology, joined Mr Burman in 2009 from Piramal
Diagnostics. Oncquest began expanding only in 2009, after
Mr Burman and team decided to grow aggressively in the
field of diagnostics.
“Rapid change is the defining feature of pathology! It will
come as no surprise to those in the clinical laboratory and
pathology field that the market is undergoing rapid change.
In recent decades health expectations have risen globally, a
lot being done to make diagnostics more affordable,
accessible and accurate. The proper scaling of pathology
services is key to this growth, as pathology is involved in
70% of all healthcare diagnosis. not just in India but

the Indian Armed Forces for about 10 years and has over 25
years of experience in the field of Pathology and Health
care management.
Aging population and the rising prevalence of chronic
conditions like obesity, diabetes, the rise of universal
healthcare & cancer and many other factors are driving
change in the pathology market. Dr. Gaur says,“Just as
demand for laboratory services is rising, there is downward
pressure to use fewer resources. Industry consolidation is as
prevalent in laboratories as in other areas of healthcare,
with increased economies of scale expected to deliver more
test results at a lower cost. The upside is the abundance of
valuable information that flows from the life cycle of
medical testing, real-time feedback to improve process and
overall management. Pathology service providers have to
adapt to these challenges in climate where the pace of
change is likely to accelerate”.
Dr. Gaur is a strong believer of setting up advanced
platform & quality driven technologies and making them
available in the Indian market at affordable cost, a vision
that he shares with Mr Aditya Burman.
There is an upsurge of new testing methods to support
initiatives such as personalised or precision medicine, also
known as genomic medicine, and point-of-care testing.
Dr. Gaur and his team is currently working in the area of
developing diagnostic assays to support personalized and
precision medicine with genomics as a base, new point of
care or near-patient testing technologies, intending to
deliver the quality and precise diagnostic facilities at all
levels and for all sectors of the healthcare industry. He has a

use of system biology and panomics aids this process by
diagnosing the illness at a molecular level. Once the cause
is determined, customized medication could be prescribed
to them. Such personalized medicine moves from the ‘one-
size-fits-all’ model that doctors have previously adhered to.
While it might be expensive to customize medicine for each
individual, gathering the genetic makeup of individuals
could be useful in classifying them into groups. Such
groups, then, will have their own customized treatment.
Collecting genomic information of the population will also
aid population health studies in the future, which can help
prevent diseases at an early stage. Precision medicine can
also reduce the cost of treatment by decreasing repetitive
administration of medicine and mitigating the side-effects
associated with them. Treating an ailment on a molecular
level will help biologists understand the underlying cause
behind their occurrence and correspondingly develop
medicine to eradicate them completely. Genetic targeting
can drive the development of drugs rather than the long and
arduous trial and error procedure.
Time to Tighten the Noose
Dr. Gaur says, “There can never be any compromise on
quality, after all pathology sample is a life. We support and
provide high of quality diagnosis. Having done so much
work for the wellbeing of the society, it hurts the team to see
the spurious growth of unaccredited diagnostic centers,
especially in Tier II and Tier III cities”. While the
Government has developed stringent laws to curb them, a
lot still needs to be done when it comes to implementation.
Dr. Gaur explains that, “diagnostic tests form the base of
treatment. One wrong diagnosis doesn t only send the’
treatment off the track but also causes a setback in the form
of lost samples, precious time and even life”. While the
onus lies on the authorities and pathology centers, the
general public too should be aware of the dangers that such
unauthorized centers pose to their health. The patients
should not hesitate in asking questions about quality norms,
sample handlers and platforms for being used for testing
their samples.
There s More to Diagnosis’

conclusive, final, finer, precise diagnosis, which can help in

molecular biology and genomics, pathologists shouldn’t just
be seen behind the microscope but come forward and
integrate with the treating clinicians, support doctors,
patients and emerge as real clinical solution provider. We

ith an aim to reduce the gap between the Indian
Wdiagnostic industry and the rest of the world,
CORE Diagnostics was founded in 2012 by
23year old Zoya Brar- a first generation entrepreneur.
CORE’s service philosophy revolves around providing
definitive diagnostics & therapy selection in shortest
possible turnaround time with second opinion on every test.
In a little over six years CORE has become synonymous
with innovation in clinical diagnostics. Over the years,
business has grown nearly 5% on a month-on-month basis.
In the following Interview with Insights Success, Zoya, the
Founder and CEO speaks about the transition that CORE
is bringing about in the diagnostic industry.
1. Kindly brief us about the company and its initial
journey.
The inception of CORE was based on a simple
observation: while the “delivery system” of healthcare
services had caught up with the western standards, the
lack of talent availability had led to a “white space” in
high-end diagnostics. Despite the presence of handful of
large players and local labs – we were able to witness the
immense potential of advanced diagnostics. There was
dire need to supplement routine testing to address the
grossly underserved high-end diagnostic market. CORE
came to the scene in 2012 to bridge this gap by making
the advanced diagnostics procedures available, affordable
and accessible in India.
2. Brief us about you and your role in the Company?
My interest in precise disease stratification, therapy
selection and the zeal to provide definitive diagnosis
within shortest turnaround time, led me to found CORE.
My bachelor’s Degree in Sociology and upbringing in the
cities of Chandigarh, Ludhiana and Delhi made me learn
about leadership through osmosis. My skills were further
honed to perform in various roles while writing for the
Youth Parliament, organizing street plays, helping people
with disabilities and several such initiatives while in
school.
At CORE, me and my team’s modus operandi as a leader
in this space is to bring the most advanced testing
diagnostic techniques and expertise to India. As a
corollary, we are the destination for all high-end
diagnostic testing. This can be made possible only by
bringing together world-class people who share the same
vision and entrepreneurial spirit. This has been the most
challenging and the most exciting part of my journey. For
startups, it is critical for employees to be committed to an
open, transparent culture. Hence, we put a lot of emphasis
right at the start - during our interview process. Today, we
proudly stand at an attrition rate of less than 5% - in an
industry that suffers from up to 50% employee turnover.
We have a team of nearly 300 people now.
3. Brief us about the services that the company offers.
CORE has established itself as the largest, and the fastest
growing company in the high end diagnostics space with
a broader coverage test menu of several thousand plus
tests in oncology, nephrology, neurology, infectious
disease, reproductive medicine, transplant medicine, etc.
CORE offers a wide range of tests in each of these
disease segments. While our initial focus was oncology
and that’s what helped us establish our brand, we have
expanded our offering to cover all important disease
segments in molecular diagnostics/ Our most recent
addition was comprehensive test menu in nephrology.

4. What are the challenges in this Sector in India?
Diagnosis sector is highly unorganized and served by a
very large number of labs focused on routine testing.
This, in and off itself is a challenge, in that it sets the
quality bar for the consumer very low.
Another big challenge is the lack of highly skilled
workforce – both on the technical side as well as the
clinical side of pathology. Lack of awareness amongst
the clinicians – regarding the importance and role of
diagnostic, prognostics, and predictive tests used to be a
challenge but I believe CORE successfully addressed
that in the first two years of its existence.
Price sensitivity is always a challenge in the Indian
market.
5. What are the unique propositions that the centre
offers giving it an edge over competitors?
We stand out in many ways. First, the panel of experts in
the US is world class and includes some of the top names
in Academic Medicine. Second, having investors based
in the Silicon Valley, Boston, London and Mumbai gives
us tremendous access and exposure. Our work culture is
transparent with a flat hierarchy.
Most importantly, we stand out on the label of
INNOVATION. We are innovative – in everything we
do. And we are perceived as such.
6. Please share the latest technology that the centre is
utilizing to offer better services.
We have brought in innovation in various dimensions.
For instance, we have the broadest menu of high-end
molecular and genomic diagnostics in Cardiology,
Oncology, and Reproductive Medicine. Identifying the
need gaps in the sector, we are focused on scientific
breakthroughs in three general areas – genomics,
informatics, and massive reductions in the cost of testing
which is the utmost need in the sector.
Not too long ago, CORE joined hands with Guardant
Health to launch the world’s leading comprehensive
liquid biopsy test in India for patients in advanced stage
of cancer. Last year, CORE added another feather in its
cap by achieving the shortest turnaround time from 7
days to 5 hours through the COREprime technology. A
similar partnership was set up with Promega biotech to
bring in technology that helps in the selection of the most
effective clinical intervention.
7. How does the firm intend to handle its Societal
Responsibility? Please mention any special charity or
program as such.
Preventive healthcare still has a long way to go before it
becomes mainstream anywhere in the world, even more
so in India. Towards this goal, CORE has established a
genetic counseling program to spread awareness and
provide counseling to patients and guidance to clinicians.
We routinely conduct CMEs for clinicians to make them
aware of the newest developments.
Finally, as a part of societal responsibility, we also offer
our services free of cost for those who most need them.

8. What are the company’s views regarding the spurious
growth of uncertified and illegal centres that crop up?
Back in 2012, spending nearly six months interviewing
clinicians and fellow diagnosticians to understand the
pain points of our industry. Even then it was clear to me
that the chasm between those that were providing reliable
results and everyone else was extremely wide. There are
nearly 1,00,000 diagnostic labs in the country, and many
of them lack the stringent quality systems that are
essential to achieve consistency, accuracy and timeliness
of test results. Less than 5 percent laboratories are
authorized by the National Accreditation Board for
Testing & Calibration Laboratories (NABL).
9. Brief us about the company’s future perspectives.
We see our broader agenda as re-shaping the diagnostic
industry – in a manner that it turns into the central pillar
of healthcare delivery. At present, we are actively
working at expanding our reach through various
partnerships with Government, Pharma, and Health tech
companies Also, we are adding new clinical areas
enabled by latest, cutting-edge technologies in whole-
genome sequencing, proteomics, and more recently,
metabolomics.
We have already become the partner-of-choice for many
Pharma company for conducting clinical trials, and we
expect our business to grow rapidly in this segment.
Finally, we believe we will soon be viewed as a Clinical
Data Science company, in addition to being a best-in-

Reading the Early Signs
With the fierce development in medical science and technology, it is now possible to conduct tests that can warn people
about the onset of a certain disease. Screening tests, as they are known, are done to detect potential health disorders in
people who currently show no symptoms of the disease. The objective here is early detection, to reduce the risk of disease
through lifestyle changes and/or surveillance. In severe cases, early detection helps to treat the disease more effectively.
Screening tests, though not 100% accurate are valued for their ability to detect potential problems, and minimize unclear,
ambiguous, or confusing results. They are generally recommended for people at high risk for disease, or very rare diseases.
Some of them being:
Cholesterol Monitoring – A waxy substance found naturally in our body and also in the food that we eat. It is required by the
body in a moderate amount and when the levels increase, the risk of cardiovascular diseases increases too. A timely detection
helps by making lifestyle changes, though it doesn’t completely rule out the risks.
Cancer Detection – Tests like pap test(smear), prostate-specific antigens, mammography, etc are done to rule out the chances
of developing cervical, prostate and breast cancer respectively. Cancer, as we know, can be controlled when detected early.
How much is too much?
Screening tests and search engines have put information in the hands of the common man. The result is a revolution that
borders on discombobulation. People are going in for screening tests, not because their medical practitioner said so, but
because the nearest diagnostic center was offering a huge discount. And yes, their neighbor or a close family-friend thinks it
is important to do so. Often people rush into treatment at the smallest pretense without bothering to understand the effects of
it. This has given rise to the phenomenon of over-diagnosis and over-treatment.
The diagnostic industry is handling out DIY kits to detect diseases. While this might be useful, they argue, it also brings with
it unnecessary worry. As soon as people see their values slightly different from what is considered normal, they jump to the
conclusion that they are sick and need immediate treatment. The fact that the treatment might take a toll on their financial,

The Harbingers of Evolved Testing
The odyssey of Genomics started when the Diagnostics and

Challenges in the Industry
Team Genomics feels that the diagnostic laboratories
industry in India is poised for strong growth because of
rising demand for healthcare but lack of regulation remains
a major concern, considering the risks it poses to patients.
Dr Upinder explains, “There is no stratification of labs and
there is lack of awareness. People don’t know the difference
between an accredited lab and any other lab. That is
because there is no legislation around it.” He further adds,
“Accreditation is a voluntary process which should be
made mandatory. Though it is not proposed to be enforced
on clinical establishments including laboratories but Under
the Clinical Establishments Act, the effort of the
government is to enforce standard treatment protocols,
ensure adherence to minimum standards and range of rates
of medical/diagnostic procedures, which is essential to
make proper healthcare available by ensuring affordability
and accessibility to the citizens of the country.”
The company feels that the real menace are the labs which
have a poor quality control system and are in no position to
give assurance on the reliability of test to their customers.
The government should formulate a ‘robust policy’ in the
interest of patients for regulating the functioning and
opening of pathological laboratories and to constitute
appropriate authority for regular checking of such
laboratories. One cannot and should not allow
mushrooming of path labs managed by unqualified
professionals and non-calibrated testing equipment.

complex disease which not only affects the patient,
but the complete family. It doesn’t only affect an
individual’s health, but other aspects like social,
professional and financial life as well.
When Dr. Manjiri Bakre lost a dear one to the disease, she
was determined to know what exactly went wrong.She
founded OncoStem Diagnostics in 2011 with a vision to
develop tests that help to identify patterns of cancer
recurrence risk. An understanding of these patterns ensures
that patients receive personalized therapies and treatments
that are best suited to their condition.
The following Interview with Dr. Manjiri highlights the
need of understanding the tumor biology in optimizing
therapy for cancer patients.
1. Kindly brief us about the company and its initial
journey.
I founded OncoStem in 2011, after being moved by the
death of a close friend due to breast cancer. She died
despite being diagnosed with early-stage disease, maybe
due to a limited understanding of the tumour biology
driving the disease. Companies in the West were
developing tests that could analyze tumour biology and
determine the risk of relapse in patients with early-stage
breast cancer patients. These tests could help personalize
a patient’s treatment plan based on that patient’s
individual risk of relapse. Since these tests are very
expensive, I approached doctors in India and pitched my
idea of developing a home-grown and affordable version
of such a test.
2. Brief us about you and your role in the Company?
I am the Founder and CEO of OncoStem Diagnostics. I
wear multiple hats in OncoStem and enjoy managing
just about every aspect of OncoStem. There is really
nothing beneath me or nothing that I would not want to
do. If I do not know about some task I will read up, talk
to the experts and learn it to do it well. I have started
right from setting up the laboratory infrastructure,
purchasing consumables to hiring and training first 2-3
employees. I do fund raising, look into finances, take
care of Scientific aspects, legal and patenting, plan out
new tests and research avenues. Speaking with
clinicians for marketing the test, attending conferences
to bring in more collaborations, marketing the test,
finding new partners to work with us within India and
abroad. I have a team of 30 very capable people now
and they help me many aspects.
3. Brief us about the services that the company offers.
OncoStem has developed a prognostic test called
“CanAssist Breast”, early-stage hormone receptor
positive breast cancer patients. It makes customized
treatment possible by analysing the patients tumor in
depth and providing a patient specific report. CanAssist
Breast categorizes patients based on the risk of cancer
recurrence clearly as either ‘low or high’ with no grey
area in between. This clear distinction of patients based
on risk of cancer recurrence allows doctors to devise
treatment plans that are in tune with the prognosis,
maintaining a balance between the benefits and side
effects. Patients who are at low risk of relapse can
potentially avoid chemotherapy and its associated side-
effects while patients who are at high risk of relapse
would benefit from the addition of chemotherapy to
their treatment regimen.
4. What are the challenges in this Sector in India?
Conducting validation studies in India can be challenging
because the clinical trial framework is not as well
established as developed countries. I must have
approached 50-60 hospitals to work on developing and
validating CanAssist Breast, which ultimately led to 10
hospitals signing up. There are many reasons for this – 1)

one is that the doctor-patient ratio in India is well below
the developed world. Each oncologist in India sees many
more patients per day leaving them with little time to
focus on research studies. 2) Lack of documented patient
records and follow-up also makes it difficult to conduct
studies that require patient history. Patients often move
back to their hometowns after treatment and are lost to
follow-up. 3) Lack of enforcement and encouragement
of importance of research studies, clinical trials, detailed
documentation from government and regulatory
authorities. If they enforce and encourage these
activities, we can develop far more world class
diagnostics, drugs and medical devices than currently
doing.
5. What are the unique propositions that the centre
offers giving it an edge over competitors?
Our product CanAssist Breast is the only test to be
validated on Indian patients. Competitors are developed
and validated in Western countries, on Caucasian
patients. Caucasian breast cancer patients tend to be older
and post-menopausal at diagnosis, whereas their
counterparts in India tend to be younger and pre-
menopausal. Our USP is that we have taken this into
consideration right from test development. Our offering
is the only product to have data on Indian patients. We
are also the most affordable offering in the market today,
giving us another edge over prohibitively expensive
Western tests. Finally, our trun around time is the shortest
so we report within 8-10 days compared to 3 weeks taken
by some of the competing tests.
6. What are the key points that you consider while
opening a new centre?
Quality is key. Given the impact on a patient’s health, all
medical testing should be done in a quality controlled
environment by qualified and well trained personnel. The
proper accreditations should be in place, as this ensures
that laboratories follow a quality management system
and face regular quality audits. At OncoStem
Diagnostics, we strive to maintain the highest quality.
Our laboratory has been audited and certified by National
Accreditation Board for Testing & Calibration
Laboratories (NABL) and College of American
Pathologists (CAP) for upholding quality systems as per
ISO 15189. Our laboratory also participates in various
external quality assurance schemes including the one run
by the National Cancer Grid based out of Tata Memorial
Hospital in Mumbai. Our test CanAssist Breast is CE
marked, making it eligible for sale in the European
Union.
7. Please share the latest technology that the centre is
utilizing to offer better services.
OncoStem flagship product ‘CanAssist Breast’ test uses a
cutting edge machine learning-based algorithm to predict
the risk of recurrence for patients. Machine learning is
known to be a more advanced tool to develop prognostic
tests where patterns of patient information need to be
understood and analysed. We are also working on full
automation of the test, including digital pathology, so that
it can be sold as a kit and performed in a decentralized
manner in the future. This will reduce the turn around
time and also allow the test to be performed in a standard
manner irrespective of which laboratory is doing the test.
8. How does the firm intend to handle its Societal
Responsibility? Please mention any special charity or
program as such.
OncoStem partners with key opinion leaders in oncology
and supports them in conducting breast cancer screening
camps and other awareness programs. We do this through
doctors to ensure that patients get the right advice from
qualified medical professionals. We are looking to reduce
the price of the test for public sector institutions so that
more and more patients are benefitted. We are also
looking partner with NGO’s who work in the breast
cancer space and can support patients in their treatment
journey.
9. What are the company’s views regarding the spurious
growth of uncertified and illegal centres that crop up?
This can be really damaging to patients as accurate
diagnosis of disease is the first and most important step in
their treatment journey. Incorrect test results can have a
life-changing and often irreversible impact on the health
of a patient. Major surgeries and treatments are planned
based on the result of medical tests. Unfortunately, only
1,038 out of 1,00,000 labs are accredited by National
Accreditation Board for Testing & Calibration
Laboratories (NABL), which is less than 5% of all labs in
India. Ideally, all labs should be accredited by the
national body as is the case in developed countries. To
ensure affordable testing in India, we should look at other
measures that can lead to lower costs rather than
compromising on quality.
10. Brief us about the company’s future perspectives.
OncoStem is working to globally validate its “Made-in-
India” test CanAssist Breast, by undertaking studies
across multiple continents. We are also working with
pharmaceutical companies to develop targeted drugs
based on the learnings from CanAssist Breast, which can
help treat those patients who don’t respond to
conventional therapy. And finally, we are also working to
build a pipeline of tests that can personalize treatment for
other cancers like ovarian cancer.

  • 5. From the Editordiscussion about the healthcare sector in India is Aincomplete without the mention of Diagnostic and Pathology Services. To ensure complete healthcare, harmony must be in place between the medical service providers and the diagnostic and pathology service providers. The two sectors complement each other and help form the basis of a healthy society. The diagnostic industry in India has grown with the development of medical sciences. It has come a long way from simple blood tests to the ultra-modern scans, hormone tests, and screening tests. With growing health-consciousness, the demand of such centers has also increased. With India’s growing image as a medical tourism country, the role of reliable and certified diagnostic centers becomes even more crucial. The major market share of the diagnostic industry is currently held by the unorganized centers which cater to the small and medium-sized towns and cities. The Tier I and Tier II cities are mostly catered by highly specialized and organized centers. This skewed arrangement at times reflects the level of awareness that the consumers have in these cities who demand and are keen on utilizing best-in-class services. On the other hand, it also represents the disparity in providing services by the diagnostic centers who tend to have an inclination towards monetary benefits instead of just human- service. It certainly is true that to provide world-class services, one needs to invest heavily in setting up the proper infrastructure. But the other side of the coin is that unless and until the services reach the last mile, the efficiency of an industry is always under scrutiny. On a brighter note, many diagnostic centers are realizing their social responsibility and catering to the rural areas as well. They often organize health-check up camps and utilize other means to reach out to the lowest strata of society. The Government and the NGOs also assist and promote the diagnostic industry to ensure that health and diagnostic services reach all and sundry. It involves the study of diseases, what causes them and often describe or define tests and processes that need to be followed to ascertain a particular disease. As Louis Pasteur says, “Without Laboratories men of science are soldiers without arms”. The origin of this science dates back to early civilization as discovered in their records, found in several ruins. India, the Middle East and China are credited with the initial development, and ancient Greece is where detailed and the present formal form of the science took shape. But the current situation of pathology and diagnostic services in India is no mirror to its ancient glory. While India has now become a highly appreciated medical tourism spot with many advanced and globally accredited diagnostic centres, the fact remains that the infrastructure at places is severely lacking. India has around 100,000 plus diagnostic centres which include pathology and radiology. As per one market report, approx. 62% of the diagnostic market is shared by private sector while 38% is in Govt sector. Of the entire private diagnostic market approx. 68% is from pathology while 32% is from radiology. Also, approx. 51% of private diagnostic market is in hospital-based centres, 47% is in independent centres and remaining comes from small polyclinics. Only about 20% of them are a part of the organized pathology providers. The remaining 80% of the industry is highly fragmented, unregulated and unfortunately, most do not adhere to minimum standard requirements. Though there are few pathologists owned, standalone labs which are very well organized and high quality driven, but they form just a fraction of total pathology centres. Such personalized medicine moves from the ‘one- size-fits-all’ model that doctors have previously adhered to. While it might be expensive to customize medicine for each individual, gathering the genetic makeup of individuals could be useful in classifying them into groups. Such groups, then, will have their own customized treatment. Collecting genomic information of the population will also aid population health studies in the future, which can help prevent diseases at an early stage. Precision medicine can also reduce the cost of treatment by decreasing repetitive administration of medicine and mitigating the side-effects associated with them. Treating an ailment on a molecular level will help biologists understand the underlying cause behind their occurrence and correspondingly develop medicine to eradicate them completely. Genetic targeting can drive the development of drugs rather than the long and arduous trial and error procedure. Time to Tighten the Noose Dr. Gaur says, “There can never be any compromise on quality, after all pathology sample is a life. We support and provide high of quality diagnosis. Having done so much work for the wellbeing of the society, it hurts the team to see the spurious growth of unaccredited diagnostic centers, especially in Tier II and Tier III cities”. While the Government has developed stringent laws to curb them, a lot still needs to be done when it comes to implementation. Dr. Gaur explains that, “diagnostic tests form the base of treatment. One wrong diagnosis doesn t only send the’ treatment off the track but also causes a setback in the form of lost samples, precious time and even life”. While the onus lies on the authorities and pathology centers, the general public too should be aware of the dangers that such unauthorized centers pose to their health. The patients should not hesitate in asking questions about quality norms, sample handlers and platforms for being used for testing their samples. CORE’s service philosophy revolves around providing definitive diagnostics & therapy selection in shortest possible turnaround time with second opinion on every test. In a little over six years CORE has become synonymous with innovation in clinical diagnostics. Over the years, business has grown nearly 5% on a month-on-month basis. In the following Interview with Insights Success, Zoya, the Founder and CEO speaks about the transition that CORE is bringing about in the diagnostic industry. 1. Kindly brief us about the company and its initial journey. The inception of CORE was based on a simple observation: while the “delivery system” of healthcare services had caught up with the western standards, the lack of talent availability had led to a “white space” in high-end diagnostics. Despite the presence of handful of large players and local labs – we were able to witness the immense potential of advanced diagnostics. There was dire need to supplement routine testing to address the grossly underserved high-end diagnostic market. CORE came to the scene in 2012 to bridge this gap by making the advanced diagnostics procedures available, affordable and accessible in India. 2. Brief us about you and your role in the Company? My interest in precise disease stratification, therapy selection and the zeal to provide definitive diagnosis within shortest turnaround time, led me to found CORE. My bachelor’s Degree in Sociology and upbringing in the cities of Chandigarh, Ludhiana and Delhi made me learn about leadership through osmosis. My skills were further honed to perform in various roles while writing for the Youth Parliament, organizing street plays, helping people with disabilities and several such initiatives while in school. At CORE, me and my team’s modus operandi as a leader in this space is to bring the most advanced testing diagnostic techniques and expertise to India. As a corollary, we are the destination for all high-end diagnostic testing. This can be made possible only by bringing together world-class people who share the same vision and entrepreneurial spirit. This has been the most challenging and the most exciting part of my journey. For startups, it is critical for employees to be committed to an open, transparent culture. Hence, we put a lot of emphasis right at the start - during our interview process. Today, we proudly stand at an attrition rate of less than 5% - in an industry that suffers from up to 50% employee turnover. We have a team of nearly 300 people now. 3. Brief us about the services that the company offers. CORE has established itself as the largest, and the fastest growing company in the high end diagnostics space with a broader coverage test menu of several thousand plus tests in oncology, nephrology, neurology, infectious disease, reproductive medicine, transplant medicine, etc. CORE offers a wide range of tests in each of these disease segments. While our initial focus was oncology and that’s what helped us establish our brand, we have expanded our offering to cover all important disease segments in molecular diagnostics/ Our most recent addition was comprehensive test menu in nephrology.

  • 20. 4. What are the challenges in this Sector in India? Diagnosis sector is highly unorganized and served by a very large number of labs focused on routine testing. This, in and off itself is a challenge, in that it sets the quality bar for the consumer very low. Another big challenge is the lack of highly skilled workforce – both on the technical side as well as the clinical side of pathology. Lack of awareness amongst the clinicians – regarding the importance and role of diagnostic, prognostics, and predictive tests used to be a challenge but I believe CORE successfully addressed that in the first two years of its existence. Price sensitivity is always a challenge in the Indian market. 5. What are the unique propositions that the centre offers giving it an edge over competitors? We stand out in many ways. First, the panel of experts in the US is world class and includes some of the top names in Academic Medicine. Second, having investors based in the Silicon Valley, Boston, London and Mumbai gives us tremendous access and exposure. Our work culture is transparent with a flat hierarchy. Most importantly, we stand out on the label of INNOVATION. We are innovative – in everything we do. And we are perceived as such. 6. Please share the latest technology that the centre is utilizing to offer better services. We have brought in innovation in various dimensions. For instance, we have the broadest menu of high-end molecular and genomic diagnostics in Cardiology, Oncology, and Reproductive Medicine. Identifying the need gaps in the sector, we are focused on scientific breakthroughs in three general areas – genomics, informatics, and massive reductions in the cost of testing which is the utmost need in the sector. Not too long ago, CORE joined hands with Guardant Health to launch the world’s leading comprehensive liquid biopsy test in India for patients in advanced stage of cancer. Last year, CORE added another feather in its cap by achieving the shortest turnaround time from 7 days to 5 hours through the COREprime technology. A similar partnership was set up with Promega biotech to bring in technology that helps in the selection of the most effective clinical intervention. 7. How does the firm intend to handle its Societal Responsibility? Please mention any special charity or program as such. Preventive healthcare still has a long way to go before it becomes mainstream anywhere in the world, even more so in India. Towards this goal, CORE has established a genetic counseling program to spread awareness and provide counseling to patients and guidance to clinicians. We routinely conduct CMEs for clinicians to make them aware of the newest developments. What are the company’s views regarding the spurious growth of uncertified and illegal centres that crop up? Back in 2012, spending nearly six months interviewing clinicians and fellow diagnosticians to understand the pain points of our industry. Even then it was clear to me that the chasm between those that were providing reliable results and everyone else was extremely wide. There are nearly 1,00,000 diagnostic labs in the country, and many of them lack the stringent quality systems that are essential to achieve consistency, accuracy and timeliness of test results. Less than 5 percent laboratories are authorized by the National Accreditation Board for Testing & Calibration Laboratories (NABL). 9. Brief us about the company’s future perspectives. We see our broader agenda as re-shaping the diagnostic industry – in a manner that it turns into the central pillar of healthcare delivery. At present, we are actively working at expanding our reach through various partnerships with Government, Pharma, and Health tech companies Also, we are adding new clinical areas enabled by latest, cutting-edge technologies in whole- genome sequencing, proteomics, and more recently, metabolomics. We have already become the partner-of-choice for many Pharma company for conducting clinical trials, and we expect our business to grow rapidly in this segment. But as it is with any other system, our body is also prone to snags and break-downs that are commonly called as diseases. The term disease is often defined as a “disorder of structure or function, especially one that produces specific symptoms or that affects a specific location and is not simply a direct result of physical injury.” So any kind of deviation or disorder from the normal working, is termed as illness, sickness or disease. When Dr. Manjiri Bakre lost a dear one to the disease, she was determined to know what exactly went wrong.She founded OncoStem Diagnostics in 2011 with a vision to develop tests that help to identify patterns of cancer recurrence risk. The following Interview with Dr. Manjiri highlights the need of understanding the tumor biology in optimizing therapy for cancer patients. 1. Kindly brief us about the company and its initial journey. I founded OncoStem in 2011, after being moved by the death of a close friend due to breast cancer. She died despite being diagnosed with early-stage disease, maybe due to a limited understanding of the tumour biology driving the disease. Companies in the West were developing tests that could analyze tumour biology and determine the risk of relapse in patients with early-stage breast cancer patients. These tests could help personalize a patient’s treatment plan based on that patient’s individual risk of relapse. Since these tests are very expensive, I approached doctors in India and pitched my idea of developing a home-grown and affordable version of such a test. 2. Brief us about you and your role in the Company? I am the Founder and CEO of OncoStem Diagnostics. I wear multiple hats in OncoStem and enjoy managing just about every aspect of OncoStem. There is really nothing beneath me or nothing that I would not want to do. If I do not know about some task I will read up, talk to the experts and learn it to do it well. I have started right from setting up the laboratory infrastructure, purchasing consumables to hiring and training first 2-3 employees. I do fund raising, look into finances, take care of Scientific aspects, legal and patenting, plan out new tests and research avenues. Speaking with clinicians for marketing the test, attending conferences to bring in more collaborations, marketing the test, finding new partners to work with us within India and abroad. I have a team of 30 very capable people now and they help me many aspects. 3. Brief us about the services that the company offers. OncoStem has developed a prognostic test called “CanAssist Breast”, early-stage hormone receptor positive breast cancer patients. It makes customized treatment possible by analysing the patients tumor in depth and providing a patient specific report. CanAssist Breast categorizes patients based on the risk of cancer recurrence clearly as either ‘low or high’ with no grey area in between. This clear distinction of patients based on risk of cancer recurrence allows doctors to devise treatment plans that are in tune with the prognosis, maintaining a balance between the benefits and side effects. Patients who are at low risk of relapse can potentially avoid chemotherapy and its associated side- effects while patients who are at high risk of relapse would benefit from the addition of chemotherapy to their treatment regimen. 4. What are the challenges in this Sector in India? Conducting validation studies in India can be challenging because the clinical trial framework is not as well established as developed countries. I must have approached 50-60 hospitals to work on developing and validating CanAssist Breast, which ultimately led to 10 hospitals signing up. What are the company’s views regarding the spurious growth of uncertified and illegal centres that crop up? This can be really damaging to patients as accurate diagnosis of disease is the first and most important step in their treatment journey. Incorrect test results can have a life-changing and often irreversible impact on the health of a patient. Major surgeries and treatments are planned based on the result of medical tests. Unfortunately, only 1,038 out of 1,00,000 labs are accredited by National Accreditation Board for Testing & Calibration Laboratories (NABL), which is less than 5% of all labs in India. Ideally, all labs should be accredited by the national body as is the case in developed countries. To ensure affordable testing in India, we should look at other measures that can lead to lower costs rather than compromising on quality. 10. Brief us about the company’s future perspectives.
