There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

4 CLEANING / DISINFECTION / STERILISATION
Users must contact their hospital's sanitary specialists. The recommended products and
procedures must be applied. Should there be any doubt concerning the compatibility of active
agents to be used, contact the local Maquet customer service department.
4.1 CLEANING AND DISINFECTING THE SURGICAL LIGHT
Check that the power is switched off and the light has cooled down before starting cleaning.

4.1.1 GENERAL INSTRUCTIONS CONCERNING CLEANING, DISINFECTION AND
SAFETY
Remove the sterilisable handles.
Wipe the equipment with a cloth moistened with a surface cleaner. Follow the manufacturer's
dilution, application time, and temperature recommendations.
Use a cloth to rinse the unit with clean water and wipe dry.
Wipe evenly with a cloth moistened with disinfectant. Follow the manufacturer's
recommendations.
Remove residues (in particular products containing aldehydes, quaternary ammonium or
surfactants) by wiping with a cloth dipped in clean water.
Wipe with a dry cloth.
Make sure no liquid residue is left on the device after cleaning.
4.1.2 EXAMPLES OF RECOMMENDED PRODUCTS
Produit GETINGE USA: TEC QUAT 256.
Produits ANIOS: SURFA’SAFE, 0.5% HEXANIOS G + R, ANIOSYME P.L.A., SALVANIOS
pH10, ANIOS DDSH.
Produits Schülke Mayr: ANTIFECT PLUS.
4.1.3 EXAMPLES OF PROHIBITED PRODUCTS
Solutions containing glutaraldehyde, phenol, iodine, bleach, alcohol or chloride ions must
not be used.
Fumigation methods are unsuitable for disinfecting the unit and must not be used.

1 A non-enzyme-based detergent is recommended. Enzymatic detergents
may damage the handles. Never soak the handles in these detergents for
prolonged periods. Rinse thoroughly.
4.2 CLEANING AND STERILISING THE HANDLES
4.2.1 BEFORE CLEANING
Use a soft cloth immediately after use to wipe away soiling from the handle surface.
Store handles in a place that keeps them moist to make further cleaning easier.
Take care to store them in such a way that the inside does not get soiled.
4.2.2 CLEANING
Immerse the handles in a detergent solution.1
Soak for 15 minutes to allow the solution to act, then clean by hand with a soft brush and a
lint-free cloth.
During cleaning, check regularly that the handles are fully clean and that no soiling remains
on the inside or outside.
If any soiling remains, repeat cleaning or use an ultrasonic cleaning process.
Rinsing: Rinse thoroughly in clean water to completely eliminate the detergent solution.
Drying: Wipe with a clean lint-free cloth.
4.2.3 DISINFECTION
Handles may be disinfected by machine (e.g. Getinge) and rinsed at a maximum temperature
of 93°.

Comply with national regulations
After cleaning, the handles must be steam sterilised as set out below:
Check that each handle is clean before continuing the process.
Wrap the handles with sterilisation wrapper material (double wrapper or equivalent). Handles
may also be placed in paper or plastic sterilisation bags2
, for easier identification and reuse.
Place the handles on steriliser trays with the opening downwards.3
Package with biological and/or chemical indicators for monitoring the sterilisation process, in
accordance with applicable regulations.
Run the sterilisation cycle according to the steriliser manufacturer's instructions.
To ensure correct sterilisation do not allow any soiling to penetrate inside the handle.
With the above sterilisation parameters:
- video sterilisable handles (PRV CFF and ZOOM) are guaranteed for up to 50 uses,
- non-video sterilisable handles are guaranteed for up to 350 uses
Dispose of sterilisable handles in the same way as other hazardous products in a hospital
environment.

5 GENERAL MAINTENANCE
5.1 PREVENTIVE MAINTENANCE
To preserve your surgical light's original performance and reliability, annual maintenance and
inspections should be performed as follows:
By a MAQUET technician or MAQUET-approved distributor during the guarantee period.
By a MAQUET technician or MAQUET-approved distributor or by the hospital's technical
maintenance department outside the guarantee period. (Contact your dealer to arrange the
required technical training.)
5.2 BASIC MAINTENANCE
Daily checks (user)
Check the lightheads for chipped paint, impact marks
and any other damage.
Check the stability/drift of the main arms and the spring
arms.
Check that the spring arm remains in position.
Three checkpoints: bottom, middle, top.
Check whether the sterilisable handle clicks and locks in
place correctly; replace it if not.

Check whether the green LED lights up when the
lighthead is turned on.
Check whether the LEDs operate correctly.
Check whether the system switches correctly between
Surgical light/Ambient light and Standard mode/AIM
mode (if option installed).
Monthly checks
(H LED power supply units with battery backup only)
Check the lightheads for chipped paint, impact marks
and any other damage.
Check whether the backup power supply turns on and
the light operates correctly if a power cut were to occur.

The operation and safety of the device may be affected by the removal of certain

9 COMMUNICATION PROTOCOL
9.1 SUBJECT
The section describes the interface between the H LED lightheads and an external remote
control.
EQUIPMENT
Communication over an RS485 network.
Master/slave type link; the lighthead is always the master.
One RS485 link per lighthead required.
Data is transmitted at 4800 baud with no parity.

9.2 RS485 DIAGRAM
The 2 ms time delay makes it possible to have a stable state 1 on the receiving end before
the start of the message. When sending data, the slave must be sure that the last stop bit of
the last byte has been sent in full before it sets its RS485 driver in receiving mode.
The lighthead is not always used with a remote control. The lighthead sends a remote
message once every 10 seconds in order to find out if the remote control is present. If a
remote control replies, it is monitored continuously (time delay of 100-500 ms between each
message).
If a reply is not received within 2 seconds, the remote control is considered absent. A new
test will be sent after 10 seconds.
9.3 WIRING
The Modbus signal is available on the RJ45 connectors on the ceiling interface or in the
electrical power supply. For each lighthead connected, all the RJ45 connectors have the
same signal.

The LED steadily lights up RED in case of a malfunction at any point .
In this case, the fault can be read on the local keypad.
When the lighthead is off, extended pressing on the minus button (for 10 to 15 seconds)
switches the llighthead to the display error mode.
One or more LEDs come on to indicate the power status.
The fault mode can be stopped by pressing the ON/OFF button.
