This recruitment evaluation form contains sections for interviewers to rate applicants on a scale of 1 to 5 during a job interview. Interviewers are to provide a numerical rating and brief comments for the applicant's first impression, knowledge of the company, knowledge of the job or position, ability to persuade and communicate effectively, attitude and enthusiasm, and body language. The ratings across all sections are then totaled, divided by 3, and entered as the applicant's overall score out of 10.
