2. All interested applicants to
vacant positions, whether
internal or external to DepEd,
shall submit the documentary
requirements needed for
evaluation/selection.
Submission and Receipt of Applications
3. All interested applicants, whether internal or external, shall submit the following
d•ovcuments on or before the deadline indicated in the official memorandum:
a) Letter of Intent addressed to the Head of Office or highest HRMO
b) PDS (CSC Form 212,revised 2017) with Work Experience Sheet
c) Photocopy of Certificate of Eligibility/Report of Rating
d)Photocopy of PRC License/ID, if applicable
e) Photocopy of scholastic/academic record
(e.g. TOR, including graduate and post-graduate degree/units earned, if available)
f) Photocopy of Service Record or Certificate of Employment, whichever is applicable
g) Photocopy of Certificates of Training, if applicable
h) Photocopy of the latest Performance Rating covering one (1)year performance,
if applicable;
i) Checklist of Requirements and Omnibus Sworn Statement
on CAV and Data Privacy Act (Annex C)
j) Other documents as may be required.*
Documentary Requirements
4. The applicant assumes full responsibility and
accountability on the completeness, authenticity, and
veracity of documents submitted, as evidenced by the
Omnibus Sworn Statement /
CAV, signed by the
applicant.
The HRMO and/or sub-committee shall check and verify
the completeness, authenticity, and veracity of the
documents submitted. Any false and fraudulent document
submitted shall be ground/s for disqualification.

Applications shall warrant
exclusion from the pool of
official applicants

• Use application codes to protect the identity of
applicants in compliance with the Data Privacy Act and to
promote objectivity and integrity of the process
• Checklist of Requirements shall be sent through email to
applicants whose applications were not handed
personally to the HRMO/sub-committee (i.e.,applications
received via email,courier,etc.)
