SBAR PROCESS (S) SITUATION:  What is the situation you are discussing with your colleague or mentor? Identify yourself, patient, unit, room number Briefly state details of the patients issue
SBAR cont. ( B) Background:  Pertinent background information related to the situation could include the following: - The admitting diagnosis and date of admission. - List of current medications, allergies, IV fluids, and        labs. - Most recent vital signs - Lab results: provide the date, time and  results of        any previous tests for comparison. - Other clinical information - Code status (DNR)
SBAR cont. (A) Assessment:  Present your assessment of the situation. (R) Recommendation:  State your recommendation or the patient’s request. - Examples: Patient needs to be seen now. Order change.
