HAZARDS DEGREE OF RISK CONTROL MEASURES ACTION PLAN
Hazard
Regardless of control
measures
Persons
at risk
*1
Worst
outcome
*2
Probability
*3
Risk Rating
A-E
Existing control measures taken
against the risk
What further control measures are
required when and by whom
Burns could be suffered
on the lights which will
inevitably heat up during
use; especially high risk
when needing to change
coloured filters.
St
V
(model)
Min/Maj Pr B Ensuring filters needed are planned
out in advance so we will be able to
factor enough time into our allocated
slot to let the lights cool before
swapping the filter.
Ensuring anyone dealing with the filters
is extremely careful as to not burn
themselves.
Could use an overhead projector to place
the filters on as opposed to having them
put directly into the box lights if they
have already been on for a while to
prevent the risk of getting burned.
Tripping over wires. St
V
(model)
Sf
Cl
Min Fr C Ensure all wires are tucked away
safely at the end of use.
Make people aware of wires around
the floor as soon as they enter the
room.
Do not strain wires so that they
cannot lay flat on the ground; this
could cause a worse injury if tripped
over.
Keep wires away from the door so
people won’t trip when they
immediately walk in.
Could ask cleaners for “wet floor” signs
to make the loose wires incredibly clear
to anyone walking into the room.

Sf = staff
St = students
Cr = contractors
Cl = cleaners
V = visitors
D = people with disabilities
X = young/inexperienced
L = lone workers
W = women of child-bearing
age
*2 This is without control measures
F = fatal
Maj = major injury or permanent disability
Min = minor injury
NR = non-reportable
*3 This is with control measures in place
Fr = frequent/likely/could occur repeatedly/expected
Pr = probable/not surprised/might occur often
Ps = possible/could occur sometime
R = remote/unlikely but conceivable
U = unlikely/improbable such that likelihood is almost zero
