This document discusses a proposed DNP project that would implement a communication tool based on Sieben's Domain Management Model to improve communication and symptom management for patients aged 65+ with Alzheimer's or related dementias after being discharged from hospice care. It identifies several extraneous variables that could impact the effectiveness of this tool for both caregivers and nurses. These include caregiver time/well-being, the patient's health status, nurses learning the new strategy, and their motivation to change practice. The document also reviews two literature sources that discuss additional stressors on caregivers over time and the important role of hospice nurses in supporting communication between patients, caregivers, and families during care transitions.
