Tal Arazi-Kleinman : Screening Mammography in Israel Issues and Future

Cervical Cancer Awareness

- Cervical cancer is a major cause of cancer deaths worldwide, with over 500,000 new cases and 260,000 deaths estimated annually. Most deaths occur in developing countries where screening and prevention programs are lacking.
- Early detection through screening programs can make cervical cancer highly curable, but the majority of cases in developing nations are diagnosed at late stages when survival rates are low. Vaccination and screening can help prevent a large percentage of cervical cancer cases.
- Risk factors for cervical cancer include human papillomavirus infection and lack of access to healthcare. While HPV infection is necessary, most infections clear without causing cancer. Other factors like multiple pregnancies, smoking, and HIV infection can increase the risk.
CERVICAL CANCER
Nearly 70,000 new cancer cases were diagnosed in Peninsular Malaysia between 2003-2005 according to a 2008 report. Cervical cancer accounted for 11.9% of these 70,000 cases. Cervical cancer is caused by abnormal cell growth on the cervix which can spread if left untreated. Pap smear tests are effective at detecting pre-cancerous cervical cell changes, allowing for early treatment before cancer develops.

0.2% of all primary breast tumors. The clinical presentation and evolution can mimic a malignant
carcinoma, with the notable difference that a desmoid tumor cannot generate distant metastases. The
aim of the treatment is to achieve local control of this tumor, which can be highly aggressive by
deeply infiltrating surrounding structures, and frequently reoccurs after resection. Both the tumor and

Cervical cancer and viral etiology
Cervical cancer is the second most common cancer in women worldwide. It is caused by abnormal cell growth in the cervix and can spread to other parts of the body. Human papillomavirus (HPV) types 16 and 18 cause about 90% of cervical cancer cases. HPV is a common sexually transmitted virus that can lead to genital warts or cervical cancer depending on the viral type. Diagnosis of cervical cancer involves tests like the Pap test, colposcopy, and cervical biopsy to examine the cervix for abnormal or precancerous cells. Vaccines can protect against HPV types known to cause cervical cancer.

Screening for premalignant cervical lesions in Egypt is important given the high incidence of cervical cancer. Visual inspection with acetic acid (VIA) is recommended for screening in developing countries due to its low cost, simplicity, and ability to provide immediate results and treatment. Mansoura University experience found VIA to be a sensitive screening method, detecting cervical lesions. While a positive VIA does not always indicate cancer, it allows for low-cost screening and identification of suspicious lesions requiring further evaluation or treatment.

The method of IOERT explained
IOERT (intra-operative electron-beam radiation therapy) provides a more effective way to treat cancer by delivering a targeted radiation dose directly to the tumor site during surgery. Developed in the 1990s, it uses a mobile linear accelerator to give a one-minute radiation boost to the empty tumor bed, eliminating microscopic residual cancer cells immediately. This article discusses the first clinical uses of Elekta's new Leksell Gamma Knife Icon system at University Hospital La Timone in Marseilles, France. The Icon allows for frameless Gamma Knife radiosurgery using a mask for head fixation rather than the traditional rigid frame. The first patient was treated on August 10th for a brain metastasis using this frameless approach. Additional patients were also treated for metastases. On August 17th, the first patient began a multi-session, hypofractionated treatment for a meningioma using the frameless approach. This article describes the experience of a large cancer center in Southern Italy in continuing timely surgical treatment of genitourinary cancers during the early phase of the COVID-19 pandemic. The center established a multidisciplinary team to prioritize urgent cancer cases and implemented screening, safety protocols, and transfer agreements to identify and manage any COVID-19 positive patients while maintaining surgical volume. Through regional healthcare reorganization designating the center as "COVID-free" and other measures, the hospital was able to safely perform a similar number and mix of cancer surgeries compared to the previous year without major disruption of care.

This talk has two inter-twined aims. First, it introduces the medical challenges, and the science that is being developed to address those challenges, that underlie my (current) companies: Mirada Medical, Volpara Solutions, Perspectum Diagnostics, ScreenPoint bv, further illustrated by Guidance Navigation Holdings, IRISS Medical Technologies, and Acuitas Medical. Second, it asks why I am driven by the translation of mathematics and computing (white board) to clinical practice (white coats).
Professor Sir Michael Brady is currently Professor in Oncological Imaging in the Department of Oncology at the University of Oxford, having recently retired as Professor in Information Engineering (1985-2010). Mike is co-Director of the Oxford Cancer Imaging Centre, one of four national cancer imaging centres in the UK. He is the author of over 750 articles and 45 patents in computer vision, robotics, medical image analysis, and AI, and the author or editor of ten books. He has successfully supervised the PhD theses of 115 students. He is particularly well known for his pioneering research in quantitative methods for mammography and breast cancer more generally. Mike has a continuing strong commitment to commercialisation of his science and to entrepreneurial activity more generally. Current companies he has founded are: Mirada Medical; Matakina; Perspectum Diagnostics; Guidance; and ScreenPoint. As well, he is an NED of IRISS Medical Technologies; Acuitas Medical; and colwiz. He recently stepped down after 19 years as Deputy Chairman of Oxford Instruments plc. Finally, he is a member of the Syncona Advisory Board and Chair of the Royal Society Publications Board.

Depuis 2004, les autorités sanitaires invitent toutes les
femmes (de 50 ans à 74 ans) à effectuer une
mammographie tous les 2 ans. Objectif : diagnostiquer les
cancers du sein précocement afin de réduire la mortalité
ainsi que la lourdeur des traitements.
Après quinze ans de dépistage, le constat est sans appel :
le cancer du sein est diagnostiqué de plus en plus tôt, on
découvre des tumeurs de plus en plus petites et pourtant
le dépistage n’a réduit ni la mortalité, ni le nombre de formes avancées, ni la lourdeur des traitements. Le dépistage ne permet pas de moins mourir du cancer du sein.

- 2. www.cancer-rose.fr Verify whether, in France, the generalization of organized breast cancer screening has been accompanied by a decrease in the most mutilating interventions, total mastectomies. One of the arguments advanced by its proponents would be a reduction in heavy treatments, made possible by diagnosing smaller lesions. This assumption has not been evaluated in France. OBJECTIVE OF STUDY
- 3. www.cancer-rose.fr METHODS The annual number of total mastectomies is estimated from the PMSI informations system on hospital stays listed in GHM root 09C04 "Total Mastectomies for Malignant Tumors". This number of hospital stays has been extracted, year by year, from the ScanSanté platform delivering PMSI data (www.scansante.fr) The study covered the period 2000-2016. Using data on the incidence of invasive breast cancer, available for 2000, 2005, 2010 and 2012 on the INVS site, the ratios of total mastectomies per 1,000 new cancers are calculated.
- 5. www.cancer-rose.fr RESULTS The annual number of total mastectomies increased over the study period from 17,403 in 2000 to 19,997 in 2016. This increase is statistically significant. In order to judge whether or not a treatment has de-escalated or not, it is necessary to look at whether, for the same number of prescriptions, more or fewer procedures are performed. The ratio number of total mastectomies per 1000 new cancers has remained stable: approximately 400 total mastectomies per 1000 cancers.
- 6. www.cancer-rose.fr The PMSI data is an exhaustive record of all interventions performed in France during hospitalization. Any decrease in total mastectomies should therefore be reflected in the data. Over the period 2000 - 2016, we are instead seeing an increase in the annual number of total mastectomies and a stable ratio of total mastectomies per 1,000 new cancers. Two hypotheses : -The small tumor size at the time of diagnosis would not be an essential determinant of the indications for total mastectomy (other factors would come into play, such as small breast size, or a retro-mamelonal lesion, or surgical habits). -The reduction in total mastectomies made possible by diagnoses of smaller lesions would be offset by an increase in total mastectomies due to over-diagnosis. (lesions that are not invasive carcinomas are surgically removed) DISCUSSION

The decline in total mastectomies for breast cancer attributable to organized screening is not confirmed by the PMSI data. Communication to women about screening should contain this information. Additional studies would be useful to clarify the causes, and in particular the role of over-diagnosis. CONCLUSION
