Hospital performance has traditionally been measured using financial metrics, but the environment of care is represented by intangible human factors that influence outcomes. In the absence of uniform quality standards, alternative methods are needed to measure effectiveness, efficiency, and equity. One approach is to examine the impact of biomedical engineering technicians on hospital quality, as their work maintaining medical equipment is central to patient care. Coordinating roles across clinical and non-clinical occupations may help improve the environment of care.

Evaluation plan involves an integral part regarding a grant suggestion providing information aimed at improving a project during the development and implementation. I will participate in the assessment of the scenario system in throughout the project. The scenario includes the hospital that is implementing the new unified as well as the Ambulatory EHR (Electronic Health Record) system that enhances the documentation of patient care. The purpose of the paper is explaining the selected scenario one, explanation of the reasons for selecting it, and summarizing of the research findings on the similar HIT implementations. More so, there is a description of the evaluation viewpoint, and goal guiding the assessment plan and same rationale.
HIT System Selected
The new system to be implemented has various modules that contain interdisciplinary assessment forms, medical notes, and clinical decision support where their documentation is guaranteed. The implementation of the unified system will enhance improved performance of the hospital in several departments. The new EHR system becomes of great importance to the hospital since there is a reduction of medical errors, reduction of the rate of the safety events of each patient, improving the quality of healthcare, deterrence of sentinel events, reduced patients readmissions as well as impact spending. Another reason for choosing the scenario is that the new system will enhance while fulfilling the requirements of meaningful use as stipulated in the HITECH (Health Information Technology for Economic and Clinical Health) Act. Therefore, the need for evaluation regarding the EHR implementation becomes paramount since it will help to identify the associated risks while adjusting the modules required when offering the medication services to the patients (Lanham, Leykum & McDaniel, 2012).

Measuring What Counts in HIS - Balanced Scorecards
This study aimed to develop a balanced scorecard (BSC) for a tertiary care private university hospital in Pakistan using a modified Delphi technique. An expert panel of clinicians and hospital managers identified and rated potential performance indicators according to importance, scientific soundness, relevance to strategy, feasibility, and modifiability. Of an initial 50 indicators, the panel selected 20 indicators across the four BSC domains of financial, customer, internal processes, and learning/growth. Evidence based practice (EBP) should be fundamental in every healthcare setting in the sense that it ensures decisions based on the best evidence integrated with clinical experience and the various expectations of patients within the healthcare setting (Gale & Schaffer, 2009). The main objective and aim in evidence-based practice protocols are to integrate the clinical expertise with the patient’s perspective and the scientific evidence in a bid to provide efficient and high quality healthcare services which are based on the needs, values, interests and culture of the patients served by the healthcare organization in question. It should be noted that evidence- based practice is essential as it does integrate the perspective of the patient, including values and culture in providing higher quality healthcare supported by research and scientific evidence (Gale & Schaffer, 2009). In essence it ensures the provision of quality and reliability of the healthcare services provided within the healthcare setting.
In regards to the healthcare organization I am currently employed by, and would opt for the implementation of EBP in, the organization is ready for the implementation of EBP in the sense that all stakeholders are in support of implementation of EBP protocols in the various units. Considering the fact that my organization is a very small critical access hospital in rural Georgia, with very limited resources, the organization is ready to fully adopt EBP. All stakeholders believe that such implementation is critical and vital for ensuring quality, and reliable healthcare service that is comprehensive and not only meets but exceeds the needs and expectations of our clients.
According to the survey, some respondents were in full support of the implantation of EBP, while others were not. It should be noted that the category scores for the survey varied due to the fact that respondents had a varied degree of preference when it comes to the implementation of EBP, and changes to practice within the facility. Most respondents responded higher in areas pertaining to changes in providing educational strategies according to EBP guidelines (Melnyk & Fineout-Overholt, 2015). Incorporating EBP within the facility basically requires all the organizational stakeholders to develop a culture of openness and inquiry since such implementation provides very clear parameters for quality and efficient care (Melnyk & Fineout-Overholt, 2015). Nurse leaders play an important role in guiding nursing professionals to achieve a health
care organization’s quality and safety standards—standards that should help patients receive
high-quality care without negative outcomes on their health. Despite efforts to maintain these
standards, health care organizations across the United States of America have witnessed an
increase in the number of poor patient outcomes (that stem from unsafe care. According to the
Quality Interagency Coordination Task Force, the consequences of quality and safety issues in
patients have amounted to an annual loss of $29 billion (Huber, 2017).
The importance of maintaining patient safety standards in health care will be explored
based on the challenges faced by a well-known nursing facility in the United States of America.
Located in a major city, the facility provides 24-hour residential care to the elderly and people
with special needs who require customized care. The facility has been facing patient safety issues
connected to how its nursing professionals administer treatment and monitor patients.
Patient Safety as a Nursing Challenge in the Nursing Facility
Nursing professionals are the primary caregivers in any health care setting; they spend
the most time with patients as they administer treatment, detect signs of deterioration in patients,
and build caring relationships with them. Therefore, safety issues that cause any negative
outcomes in their patients can affect nursing professionals emotionally and hinder their career
development (Lawton, Carruthers, Gardner, Wright, & McEachan, 2012; Huber, 2017).
The nursing facility encountered safety issues caused by nurses when administering

The role of leadership is vital to the growth and success of health care institutions. This research paper examines the meaning of leadership as it pertains the health care sector and the leadership practices employed in the same perspective. Provision of high-quality services is an increasing concern in hospitals and other health care facilities across the globe. The standards of the services are defined and influenced by some factors, leadership being one of them. This paper is meant to establish the strong connection between the leadership of a health care organization and how this impacts the kind of service the facility offers its customers. In line with this, the leadership theories that are employed are discussed to create better comprehension of the leadership in practice. A B S T R A C T
Purpose of Study: In response to the U.S. Affordable Care Act, the Centers for Medicare & Medicaid Services
proposed a change in reimbursement penalties for hospitals beginning October 1, 2012. Reducing the
occurrence of unplanned readmissions has become a more urgently focused topic. As part of the health care
system, care management aligns with physicians to signifi cantly improve service, fi nancial, and clinical care
outcomes. To address the changing health care climate in 2008, care management services were restructured
at an academic university medical center located in 1 of the 3 largest counties in California. Changing from
a unit-based to a service-based care management model partnered care managers and social workers with
physician services. We sought to assess the effect of this change on surrogates for patient experience and
clinical quality of care.
Primary Practice Setting: Tertiary academic medical center in southern California.
Methodology and Sample: Retrospective data were collected from 2 databases for all hospital patient care
services from November 2008 to January 2010 to determine whether clinical quality of care and experiential
service improvements were realized. Primary outcomes included all-cause and related readmission rates.
Secondary outcomes were Hospital Consumer Assessment of Healthcare Providers and Systems (H-CAHPS)
scores. An interrupted time series analysis compared data from the single institution for the diffusion and
postintervention periods.
Results: Comparing data from the diffusion and postintervention periods, the rate of disease-related readmissions
decreased signifi cantly (mean 5.43–4.58, p < .05), and all-cause readmissions also decreased, although the
difference failed to achieve statistical signifi cance (11.42-10.49, p = .056). H-CAHPS scores with the patient
response of “recommend this hospital” was unchanged over the 2 time points (mean 78.9%–77.8%, p = .26731).
Data also showed stable care management staffi ng rates whereas average daily census (ADC) increased over
time (ADC 274–297).
Implications for Case Management Practice: With health reform driving value-driven care transformation,
partnering care managers and social workers with physician services has the potential to impact the patient’s
experience as well as fi nancial and clinical care outcomes. Care managers serve a signifi cant role in improving
the clinical quality of care by reinforcing a consistent and clear message by the health care team to the patient
during the entire hospitalization, not just at the time of discharg.

A B S T R A C T
Purpose of Study: In response to the U.S. Affordable Care Act, the Centers for Medicare & Medicaid Services
proposed a change in reimbursement penalties for hospitals beginning October 1, 2012. Reducing the
occurrence of unplanned readmissions has become a more urgently focused topic. As part of the health care
system, care management aligns with physicians to signifi cantly improve service, fi nancial, and clinical care
outcomes. To address the changing health care climate in 2008, care management services were restructured
at an academic university medical center located in 1 of the 3 largest counties in California. Changing from
a unit-based to a service-based care management model partnered care managers and social workers with
physician services. We sought to assess the effect of this change on surrogates for patient experience and
clinical quality of care.
Primary Practice Setting: Tertiary academic medical center in southern California.
Methodology and Sample: Retrospective data were collected from 2 databases for all hospital patient care
services from November 2008 to January 2010 to determine whether clinical quality of care and experiential
service improvements were realized. Primary outcomes included all-cause and related readmission rates.
Secondary outcomes were Hospital Consumer Assessment of Healthcare Providers and Systems (H-CAHPS)
scores. An interrupted time series analysis compared data from the single institution for the diffusion and
postintervention periods.
Results: Comparing data from the diffusion and postintervention periods, the rate of disease-related readmissions
decreased signifi cantly (mean 5.43–4.58, p < .05), and all-cause readmissions also decreased, although the
difference failed to achieve statistical signifi cance (11.42-10.49, p = .056). H-CAHPS scores with the patient
response of “recommend this hospital” was unchanged over the 2 time points (mean 78.9%–77.8%, p = .26731).
Data also showed stable care management staffi ng rates whereas average daily census (ADC) increased over
time (ADC 274–297).
Implications for Case Management Practice: With health reform driving value-driven care transformation,
partnering care managers and social workers with physician services has the potential to impact the patient’s
experience as well as fi nancial and clinical care outcomes. Care managers serve a signifi cant role in improving
the clinical quality of care by reinforcing a consistent and clear message by the health care team to the patient

Improving patient discharge and reducing
hospital readmissions by using Intervention
Mapping

Abstract
Background: There is a growing impetus to reorganize the hospital discharge process to reduce avoidable
readmissions and costs. The aim of this study was to provide insight into hospital discharge problems and
underlying causes, and to give an overview of solutions that guide providers and policy-makers in improving
hospital discharge.
Methods: The Intervention Mapping framework was used. First, a problem analysis studying the scale, causes, and
consequences of ineffective hospital discharge was carried out. The analysis was based on primary data from 26
focus group interviews and 321 individual interviews with patients and relatives, and involved hospital and
community care providers. Second, improvements in terms of intervention outcomes, performance objectives and
change objectives were specified. Third, 220 experts were consulted and a systematic review of effective discharge
interventions was carried out to select theory-based methods and practical strategies required to achieve change
and better performance.
Results: Ineffective discharge is related to factors at the level of the individual care provider, the patient, the
relationship between providers, and the organisational and technical support for care providers. Providers can
reduce hospital readmission rates and adverse events by focusing on high-quality discharge information, well-
coordinated care, and direct and timely communication with their counterpart colleagues. Patients, or their carers,
should participate in the discharge process and be well aware of their health status and treatment. Assessment by
hospital care providers whether discharge information is accurate and understood by patients and their community
counterparts, are important examples of overcoming identified barriers to effective discharge. Discharge templates,
medication reconciliation, a liaison nurse or pharmacist, regular site visits and teach-back are identified as effective
and promising strategies to achieve the desired behavioural and environmental change.
Conclusions: This study provides a comprehensive guiding framework for providers and policy-makers to improve
patient handover from hospital to primary care.

Beth Fiedler Roger Bowles Reid Oetjen Biomedical Engineering Technician Dynamics on Hospital Quality Intangible Shared Performance Measures Hospital organizational performance has been largely defined by administrative data comprised of tangible financial records. Though important, the hospital environment of care is arguably represented by multiple inter-professional and intangible factors that influence patient outcomes that are not cost-driven but service-oriented. In the absence of uniform standards that address common problems across diverse health care facilities such as the association of medical equipment with adverse events, hospital administrators must view alternative methods to measure effectiveness, efficiency, and equity. One method is to examine the inter- professional impact of the Biomedical Engineering Technician (BMET) occupation in the hospital Clinical Engineering Department on the level of hospital quality in the environment of care. The proposition is premised on the knowledge that 1) the BMET duties are associated with medical equipment status, and 2) the availability of properly functioning, clean, and timely access to medical equipment that is central to healthcare. A major recommendation is the implementation of coordinated care across clinical and non-clinical health occupations.
