This document discusses rehabilitation in heart transplant patients. It provides statistics on heart transplants performed from 1988-2006, including the most common causes of heart disease. It also discusses treatment at the time of transplant, trends in recipient characteristics, and survival rates post-transplant. Complications after transplant like hypertension, renal dysfunction, hyperlipidemia, diabetes and coronary altery disease are common. Exercise intolerance is also discussed. Rehabilitation including exercise training can help increase VO2 max and reduce complications like osteoporosis and obesity. Safety precautions during rehabilitation are also recommended.
This document summarizes the results of a survey about Brunei's health industry.
The survey found that over half of respondents were between 20-25 years old. Most respondents were female. The majority felt that government hospital services were slow. The most suggested area for improvement was the pharmacy and service of nurses and doctors. Over two-thirds had issues finding parking at government hospitals and most thought more parking spaces were needed. Most did not think facilities at government hospitals were up to world standards yet and areas like equipment and certain sections were still lacking. Finally, more people preferred private hospitals over government ones due to better service, faster times, and more effective medicine.
- A survey of 600 likely Michigan voters found that a large majority believe global warming constitutes a serious threat, though Republicans and conservatives are divided on its severity.
- Across party and demographic groups, a solid majority favor action by the US to reduce carbon emissions that cause global warming.
- A plurality believe that efforts to reduce global warming will create new American jobs rather than cost jobs or not affect jobs.
- Three-quarters support a proposed energy plan that would require reducing carbon emissions from factories and power companies while also requiring increased use of clean energy sources.

The document provides background information on erectile dysfunction (ED) treatment options and market analysis. It discusses key events and competitors from 1990-2002. An analysis of ED patients' ages across different countries found the average age was highest in the US and UK. Barriers to treatment varied by age, with embarrassment and believing the condition wasn't serious enough being top barriers. A SWOT analysis identified Cialis' strengths as its effects lasting up to 36 hours and fewer side effects, while weaknesses included being a new drug and banned direct-to-consumer advertising in Europe. The document proposes targeting men aged 50-65 with Cialis and positioning it as allowing for more intimacy due to its duration and flexibility. It provides pricing
Continuous Monitoring and Timely Intervention are Needed to Improve Adherence...HMO Research Network
Kaiser Permanente Southern California studied adherence to adjuvant hormonal therapy (AHT) among 10,827 breast cancer patients between 2000-2007. They found only 49% of patients took AHT as prescribed, with discontinuation rates reaching 25% by the fifth year. Continuous monitoring of pharmacy records is needed to identify underutilization early and develop timely interventions like reminder letters or calls to improve AHT adherence and outcomes.
This document summarizes data on the economics of obesity and obesity interventions in the United States. It presents 8 exhibits that show: 1) The rise of obesity rates in the US from 1961-2008; 2) Projected obesity rates under optimistic and pessimistic scenarios; 3) International comparisons of overweight rates over time; 4) Increased healthcare spending associated with obesity by payer and service; 5) Estimated cost per quality-adjusted life year saved of different obesity interventions; and 6) The interaction between medicine and economics as it relates to cost and clinical effectiveness. This document outlines a screening project conducted with primary care providers to identify at-risk women and incorporate screening tools for alcohol, smoking, and abuse into practice. It provided a screening and resource package, conducted academic detailing, and administered pre- and post-test questionnaires. The results showed increased screening rates for tobacco, alcohol, and abuse from pre- to post-test. While the response rate for the post-test was lower, providers reported increased use of screening tools and community referrals. The academic detailing approach was found useful by most providers.
This document discusses the history and adoption of intensity-modulated radiation therapy (IMRT). It notes that IMRT was proposed over 40 years ago but was not widely implemented until the 1990s with the availability of commercial treatment planning systems. Two surveys from 2002 and 2004 found that adoption of IMRT in the United States grew rapidly, from 32% to 74% of radiation oncologists using it. While commonly available now, most centers only use IMRT to treat a subset of patients. The document reviews the types of cancer most commonly treated with IMRT and radiation oncologists' impressions of its effects.
The document contains the results of multiple choice surveys or tests with options of "Bien" or "Mal" and percentages for each. Across all the surveys depicted, the percentage of "Bien" responses ranged from 14% to 44% while the percentage of "Mal" responses ranged from 56% to 86%. Two sections at the bottom summarize general results for limits and derivatives, with 44% selecting "Bien" and 56% selecting "Mal" for limits, and 31% selecting "Bien" and 69% selecting "Mal" for derivatives.
The survey analyzed the relationship between mental health outcomes and relationship status in over 8,000 gay and bisexual men. It found that single men reported higher rates of suicidal thoughts, loneliness, sadness, depression, and suicide attempts compared to partnered men. Partnered men had slightly higher rates of these mental health issues than men partnered with women. The results suggest that being in a relationship is associated with better mental health outcomes for gay and bisexual men.
Factors Associated with ART Non-adherence in Rural Achham, Nepal Bibhusan Basnet
This study evaluated factors associated with non-adherence to antiretroviral therapy (ART) among 105 HIV patients in rural Nepal. The results showed an 85.7% adherence rate based on patient reporting. Key factors associated with non-adherence included family obligations from field work (p<0.0001), long travel times to clinics (p=0.0219), depressed mood (p=0.0112), and socio-cultural stigma (p=0.006). Overall, patients had low rates of medication side effects and opportunistic infections, and high family support for HIV status. The study identifies challenges with ART access and adherence in remote rural settings.

ABDOMINAL TRAUMA in pediatrics part one.drhasanrajab
Abdominal trauma in pediatrics refers to injuries or damage to the abdominal organs in children. It can occur due to various causes such as falls, motor vehicle accidents, sports-related injuries, and physical abuse. Children are more vulnerable to abdominal trauma due to their unique anatomical and physiological characteristics. Signs and symptoms include abdominal pain, tenderness, distension, vomiting, and signs of shock. Diagnosis involves physical examination, imaging studies, and laboratory tests. Management depends on the severity and may involve conservative treatment or surgical intervention. Prevention is crucial in reducing the incidence of abdominal trauma in children.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

problem is ignored and not solved.
• Her PhD was completed at the age 69.
• As the wife of a prostate cancer survivor and an
Epidemiologist Jo-an felt that partners, especially
women were left out of the picture.
• Jo-an is staying involved, hopeful that other women will
join her.

17. • I passed out when I self-injected.
Then we went to Cialis with no
positive result. Now back to
injections, with my wife doing it.
We have not had frequent sex
beause it upsets her to do the
injections and ruins the mood.
Will keep trying
18. • We tend to keep too busy and
have not been concentrating
on sex lately. Levitra almost
works.
19. • The range of time mattered a lot.
It's been over two years. The first
six months there was almost
nothing; now after two years we
can often have enjoyable sex
without Viagra, although we
usually do it with Viagra to be
sure we have the fun we want.
20. • Hard to answer since most of the 12 years were
satisfactory and only recently started hormone
therapy which changed everything. Other issue is
that I am 12 years older so always wonder what
would be the natural course of my potency if I was
not treated or had Prostate Cancer. Once it
metastasizes and survival becomes an issue one's
oersoective on the importance of sex changes. I
wish there were more resources available now to
both my wife and i about achieving intimacy
without intercourse. She does not like to use the
internet for a source.
21. • Some of the implications of these questions appear
to center on whether of not we can have successful
intercourse or not. Many men I believe must reach a
point where they come to peace with the situation.
Is sex more important than life. We also must learn
that our partner, especially nearing age 60 is more
interested in feeling loved, than having rousing
sexual encounters. The intimacy that results when
the man concentrates on the female's needs, and
bringing her to an aroused state and orgasm is a
satisfying sexual encounter for both.
22. • Thanks for being willing to
research this area. For
individuals like me, it's
important to see that there are
others out there that have
similar issues.
23. • Again, I think it is important that
the doctor visit with both husband
and wife. I think my wife may
have been more interested if she
realized that my sexual desires
were normal for a man of my age
(65 ) at the time of surgery.
24. Broaden discussion/presentation topics beyond just PC
treatment and fundamental incontinence and ED
treatments. Emphasize that an active life, including sex, is
quite achievable despite damaged plumbing. Emphasize
that early PC detection is very important--every male
should know this!
My greatest hope is that more doctors will be told that they
must address the subject of sex. If they aren't comfortable
doing this, or don't have the time to do it properly, they
should direct the patient & his partner to a good source of
information. It's sad when the problems are not
understood until they've become established, and the
patient doesn't even know he could have done things to
help with his rehabilitation.
Lack of success achieving penetrative sex has left us with
a failing desire for sex at all. We agreed prior to his
operation 8 years ago that life was more important... it
seems it was so easy to believe that then. We failed to
appreciate how devastating it would be to a relationship.
It's like losing a loved-one; the memory is dear but the loss
tangible.

Disappointing number of Partners participating in the survey 31%. The partners have had such a “small” voice and it is sad that they missed making a larger impact on this survey.
When broken down per Capita Canada actually had more people who did the survey.
Fewer countries included as compared to the main survey sample of 651 respondents.
A broad spectrum of ages ranging from 42 to 84 with the bulk in the middle age 62. A perfect bell curve.
Some extremes and not as even a bell curve as for all 651respondents but the number are significantly smaller, 24% of the total
The majority , 50%. of the men had some form of surgery
After treatment penetrative sex certainly took a “nose dive” compared to pre-treatment estimates. Survivors and Partners concurred in their reports of frequency with the exception where the Partners reported more sexual activity 3 or more times per week. These were not matched partners, however.
Examining the results of the various forms of prostatectomies the initial results were lower when compared to the other forms of treatment. The improvement over 24 months was significant with the radical nerve sparing heading the group. The radiation group on the other hand showed a significant decrease in erectile function over a 24 month period. The number of men receiving radiation was a much smaller group and these findings need to be followed up with a larger group. It must be emphasized that no matter what the treatment is there is significant erectile dysfunction. In light of the fact that there is recovery over time it is extremely important to exercise the penile tissue; oral medication started early even if it doesn’t result in an erection followed up with penile shots at 3 weeks after treatment. Some surgeons advocate a pumpIt is of interest to note that the group that had the better recovery of erectile function were in the age group 60 to 70. Why?
The “Never” category gets less over time and the “Always” category improves over time. Partners and survivors are close to agreement except in the “Always” category when the survivors are? Truth full or optomistic?
Partner guess for Survivor felt that he had a bigger problem than he said he had. The Partner felt the Survivor had a smaller problem than he stated. The Survivor felt the Partner had less of a “Big” and “Moderate” problem than they stated . Lisa Slide #14 or #15 ?
In the first 3 categories the Survivor over estimated the problem his Partner had with his ED. The Survivor underestimates the “ Big Problem “ his Partner has with his ED and the Partner overestimates the “Big Problem” category that their Survivor has.
There was contamination of the “date” data which only allowed us to capture 79 matched couples. As in the small survey sample I did for my dissertation we found that the Partner felt that their Survivor partner felt penetrative sex was less “Important” than he found it. Again the Survivor guessed that his Partner felt penetrative sex was more “Important” than they were. In the “Somewhat Important” category the Partner felt penetrative sex was more disruptive than their Survivor guessed.Although ED problems and Penetrative Sex are not identical questions the inability of Survivors and Partners to accurately guess what each other feels identifies a lot of questions needing resolution. Open, honest communication is essential if sexuality issues are to be solved.
It appears that having an orgasm is extremely important for both parties. Achieving an orgasm without penetration or ejaculation is possible. This could be a vital factor in compensating for and removing the stress from, the loss of penetrative sex .
There are many methods of achieving an orgasm with or without penetrative sex. It is up to each individual person or couple to select methods they are comfortable with. Again honest open discussion is essential.STILL WORKING ON THIS TO TIDY IT UP
