Culture is defined as customary beliefs, social forms, and material traits of a racial, religious, or social group (Webster, 2019). Competence suggests having the capacity to function effectively as an individual and an organization within the context of cultural beliefs, behaviors, and needs presented by consumers and their communities (CDC, 2015). In healthcare, it is very important to be aware of different culture backgrounds. It helps with not only being able to communicate effectively but also knowing what diseases, sickness, etc. that the person is at greatest risk for. For example, in the treatment of depression, compared with white Americans, black and Latino patients are actually less likely to receive treatment (Ball et al., 2019).
The patient I was given is a 14 year old biracial male living with his grandmother in a high-density public housing complex. For the purpose of obtain a health history with this particular patient it is important to consider everything about this patient. The patients age, sex, ethnicity, living conditions, etc. will all need to be taken into account. This particular age group are reluctant to talk and have a definite need for confidentiality (Ball et al., 2019). It is important that adolescent patients be given the opportunity to speak to you privately about concerns or issues that they may have (Ball et al., 2019). It is meaningful that you let the patient know the limits of confidentiality and that if any information provided suggests that an adolescence safety or others safety may be at risk, that its grounds to “break” confidentiality (Ball et al., 2019). Prior to the office visit, there a previsit questionnaires and screeners that the patient can fill out and this sometimes helps allow the patient to write down concerns or have a choice of concerns (Ball et al., 2019). Then based off the answers, it can help you ask appropriate questions during the interviewing process.

In beginning the interview, a consideration to remember is that eye contact is considered rude to Hmong People and that tone of voice and body language are very important; taking too loudly, placing too much emphasis on words, or talking excessively with hands and arm movements can result in noncompliance (Carteret, 2012). As this patient is young and assumed to be mainstream with Western culture, she will likely be understanding and forgiving of eye contact, tone, and body language but interactions with older family members will require care.
Gender of the nurse might play a role in some assessments, it is important to ask if a male nurse has permission to touch the abdomen or auscultate the lungs, heart, or abdomen. Questions pertaining to sex should be private and held with a nurse who is the same gender as the patient, it is of note that questions or examinations regarding sexual health can be misinterpreted as judgment of promiscuity, resulting in refusal, so sex must be addressed with much explanation and rationale without judgment (Carteret, 2012). As infection can be related to sex or sexual contact, this should be addressed with this patient.
The patient’s language preference for the interview is also important. The patient is a young adult and in college, however, her preference might be Hmong, or the language typically spoken at home. Another consideration is, does the patient want anyone else present for her interview/assessment? Hmong People have a family structure that is patriarchal, meaning, the father generally very involved in decision making and can, ultimately have the final say on a topic or treatment; the mother is caregiver and may wish to be present to help take care of the patient. Time consuming tasks may take the form of monitoring (e.g. checking blood glucose), keeping a diary, scheduling appointments, sorting and taking medications, exercising, meal planning, etc.. The Agency for Healthcare Research and Quality (2015) suggests the burden of these tasks significantly impact how patients manage their chronic conditions, and that patients often find it difficult to complete all these tasks in order to manage their condition effectively. Personally, I see examples of this every day at the hospital: Patients are not confident in their self-management ability and are therefore unable to demonstrate skill or awareness in regards to their condition. Despite receiving adequate medical attention from outstanding multidisciplinary teams, patients continue to show little interest in self-management, ultimately resulting in an overwhelming number of older adults who lack the confidence to manage one or more chronic conditions (Bodenheimer, 2005). Healthcare providers are being forced to seek new and innovative ways to connect with patients and reinforce educational material in order to give patients the confidence and skill to manage their care. I believe this objective to be most important because self-management is clinically proven to result in better outcomes. It is proven that support for patients and caregivers improve confidence in managing conditions. Recently, my hospital has added to its emphasis on education and follow up... Community health nurses must be culturally compliant to provide the most adequate and highest level of quality care. Understanding certain feelings and recognizing these is the first step for the nurse to put aside stereotypes and bias. Most of the time, they are learned behaviors prior to nursing. Stereotyping in nursing is a preconceived assumption regarding a certain group of people; this, in turn, leads to various personal feelings built upon that stereotype resulting in being bias. In health care, these feelings can lead to implicit bias feelings we unconsciously display towards patients and can impact patient care (Falkner, 2018). It is challenging for nurses not to be biased against one group or the other due to the fact that medically and scientifically there are certain groups or populations that certain condition/diseases are more prevalent than others, but "jumping the gun" per say could result in false diagnosis or inadequate treatments (Puddifoot, 2019). Community nurses must take into account the scientific and medical data related to each and every individual.
Campinha-Bacote and Munoz (2001) proposed a five-component model for developing cultural competence: Cultural awareness, Cultural knowledge, Cultural skill, Cultural encounters, and Cultural desire. One important way for nurses to achieve cultural competence and promote respect is to challenge our own beliefs and ask better questions regarding our patient populations. Before I begin I would like to say thank you for this study, although I am aware of the challenges that undocumented immigrants face, this article brings to light a whole different aspect of difficulties I did not consider. I am ashamed to admit that I have never thought about it from this perspective, much learned here.
The relationship between fear and its affect on health is very apparent as outlined in this study. Although fear can do many things, to include being a motivator, in the case of undocumented immigrants identified in this study it is paralyzing and results in higher levels of stress. Stress in turn results in significant and cumulative influences on health and health disparities (Reference study here). The three main sources of fear and how it relates to health are:
Cost:
The population identified in this study, due in large part to being undocumented, had a high rate of poverty, lower educational level, and high levels of transience. As expected this equates to an income level the precludes having health insurance in most cases. Fear experienced by these individuals was rooted not only in the cost that would be incurred from medical care, participants in the study also feared being identified by INS while being hunted down by bill collectors. Adding to the stress of healthcare cost, individuals were forced to choose between paying bills and buying food over paying for healthcare. Sadly, participants in this study looked at their diabetes and knew it would eventually kill them, just not today.
Language Barriers, Discrimination, and Immigration Status:
Participants in this study felt they were being discriminated against because many could not speak English, some recalled clinic/ER staff blatantly calling out and berating the participants for not speaking English and being in the country illegally. I find this particularly disgusting based on morality alone, medical professionals are charged with providing care to the needy, sick, and injured regardless of who they are. To further the participants stress level, communicating with physicians was difficult and often scary. Participants spoke of being made to feel guilty for needing an interpreter, and when they did have an interpreter, critical parts of the conversation were incorrect potentially causing serious medical issues with the patient.
Cultural Disconnect:
Participants described having difficulty speaking with medical staff about traditional remedies for the treatment of medical problems, some of the remedies (not all) were very effective in managing health issues with little side effects to the participants, including diabetes. Some individuals were embarrassed about what they had paid for the traditional medicine and that it ended up not working. Cultural disconnect made the participants fearful of what the provider might say about traditional remedies, negative feedback from the provider could potentially “spoil the normal identity of the patient” regarding ...

This article reports the findings of a survey that investigated attitudes toward poverty
among play therapists (N � 390) and its relation to demographic information. Multi-
variate analyses of variance (MANOVA) were used to measure the relationship
between play therapists’ demographics and their attitudes toward poverty, specifically
their structural, personal deficiency, and stigma scores. Results indicated that both
region and age resulted in differing views on poverty. Participants living in the
Northeast held stronger structural views of poverty than participants in the South.
Similarly, participants in the 50 –59 and 60 plus age groups disagreed to strongly
disagree with a personal explanation toward poverty than participants in the 30 –39 age
group. The importance of play therapists’ examining their attitudes toward poverty and
the direct impact on their work is discussed. Finally, implications of the results,
including overall findings, are explained.

The Agency for Healthcare Research and Quality (2015) suggests the burden of these tasks significantly impact how patients manage their chronic conditions, and that patients often find it difficult to complete all these tasks in order to manage their condition effectively. Personally, I see examples of this every day at the hospital: Patients are not confident in their self-management ability and are therefore unable to demonstrate skill or awareness in regards to their condition. Despite receiving adequate medical attention from outstanding multidisciplinary teams, patients continue to show little interest in self-management, ultimately resulting in an overwhelming number of older adults who lack the confidence to manage one or more chronic conditions (Bodenheimer, 2005). Healthcare providers are being forced to seek new and innovative ways to connect with patients and reinforce educational material in order to give patients the confidence and skill to manage their care. I believe this objective to be most important because self-management is clinically proven to result in better outcomes. It is proven that support for patients and caregivers improve confidence in managing conditions. Recently, my hospital has added to its emphasis on education and follow up... Community health nurses must be culturally compliant to provide the most adequate and highest level of quality care. Understanding certain feelings and recognizing these is the first step for the nurse to put aside stereotypes and bias. Most of the time, they are learned behaviors prior to nursing. Stereotyping in nursing is a preconceived assumption regarding a certain group of people; this, in turn, leads to various personal feelings built upon that stereotype resulting in being bias. In health care, these feelings can lead to implicit bias feelings we unconsciously display towards patients and can impact patient care (Falkner, 2018). It is challenging for nurses not to be biased against one group or the other due to the fact that medically and scientifically there are certain groups or populations that certain condition/diseases are more prevalent than others, but "jumping the gun" per say could result in false diagnosis or inadequate treatments (Puddifoot, 2019). Community nurses must take into account the scientific and medical data related to each and every individual.
Campinha-Bacote and Munoz (2001) proposed a five-component model for developing cultural competence: Cultural awareness, Cultural knowledge, Cultural skill, Cultural encounters, and Cultural desire. One important way for nurses to achieve cultural competence and promote respect is to challenge our own beliefs and ask better questions regarding our patient populations. For example, nurses should avoid unintentionally stereotyping a patient into a specific cultural or ethnic group based on characteristics like outward appearance, race, country of origin or religious preference. Additionally, many subcultures and variations can exist within a cultural or ethnic group. For instance, the term Asian-American includes cultures such as Chinese, Japanese, Taiwanese, Filipino, Korean and Vietnamese, and within these cultures, there are variations in a geographic region, religion, language, family structure and more.

Before I begin I would like to say thank you for this study, although I am aware of the challenges that undocumented immigrants face, this article brings to light a whole different aspect of difficulties I did not consider. I am ashamed to admit that I have never thought about it from this perspective, much learned here.
The relationship between fear and its affect on health is very apparent as outlined in this study. Although fear can do many things, to include being a motivator, in the case of undocumented immigrants identified in this study it is paralyzing and results in higher levels of stress. Stress in turn results in significant and cumulative influences on health and health disparities (Reference study here). The three main sources of fear and how it relates to health are:
Cost:
The population identified in this study, due in large part to being undocumented, had a high rate of poverty, lower educational level, and high levels of transience. As expected this equates to an income level the precludes having health insurance in most cases. Fear experienced by these individuals was rooted not only in the cost that would be incurred from medical care, participants in the study also feared being identified by INS while being hunted down by bill collectors. Adding to the stress of healthcare cost, individuals were forced to choose between paying bills and buying food over paying for healthcare. Sadly, participants in this study looked at their diabetes and knew it would eventually kill them, just not today.
Language Barriers, Discrimination, and Immigration Status:
Participants in this study felt they were being discriminated against because many could not speak English, some recalled clinic/ER staff blatantly calling out and berating the participants for not speaking English and being in the country illegally. I find this particularly disgusting based on morality alone, medical professionals are charged with providing care to the needy, sick, and injured regardless of who they are. To further the participants stress level, communicating with physicians was difficult and often scary. Participants spoke of being made to feel guilty for needing an interpreter, and when they did have an interpreter, critical parts of the conversation were incorrect potentially causing serious medical issues with the patient.
Cultural Disconnect:
Participants described having difficulty speaking with medical staff about traditional remedies for the treatment of medical problems, some of the remedies (not all) were very effective in managing health issues with little side effects to the participants, including diabetes. Some individuals were embarrassed about what they had paid for the traditional medicine and that it ended up not working. Cultural disconnect made the participants fearful of what the provider might say about traditional remedies, negative feedback from the provider could potentially “spoil the normal identity of the patient” regarding ...

This article reports the findings of a survey that investigated attitudes toward poverty
among play therapists (N � 390) and its relation to demographic information. Multi-
variate analyses of variance (MANOVA) were used to measure the relationship
between play therapists’ demographics and their attitudes toward poverty, specifically
their structural, personal deficiency, and stigma scores. Results indicated that both
region and age resulted in differing views on poverty. Participants living in the
Northeast held stronger structural views of poverty than participants in the South.
Similarly, participants in the 50 –59 and 60 plus age groups disagreed to strongly
disagree with a personal explanation toward poverty than participants in the 30 –39 age
group. The importance of play therapists’ examining their attitudes toward poverty and
the direct impact on their work is discussed. Finally, implications of the results,
including overall findings, are explained.

of credentialing requirements that ensures men-
tal health providers offer adequate and respon-
sive care to diverse populations. Although the
mental health field has embedded cultural com-
petence in their standards and guidelines, there
are discrepancies in the way the profession as-
sesses and measures competence (Sue et al.,
1996). Researchers have investigated attitudes
toward poverty in the helping professions
(Levin & Schwartz-Tayri, 2017; Noone et al.,
2012; van Heerde & Hudson, 2010; & Wit-
tenauer et al., 2015), but no study has focused
on play therapists’ attitudes toward poverty.
The purpose of this study is to fill a gap in the
literature regarding play therapist’s attitudes to-
ward poverty because awareness and knowl-
edge are key elements to implement culturally
responsive services and skills with diverse chil-
dren in a variety of settings.

Cultural competence is an important compo-
nent of professional practice, and practitioners
are expected to develop skills and understand-
ing pertaining to diverse clientele. Researchers
define cultural competence as the set of beliefs,
knowledge, and skills mental health providers
possess in order to deliver effective interven-
tions and services to members of various cul-
tures (Gilbert et al., 2007; Sue, 2006). The New
Freedom Commission on Mental Health (2003)
recognized disparities in mental health delivery

Ageist stereotypes, prejudice, and discrimination are potential barriers for health equality, in terms of the quantity and quality of care provided to older patients and their health-related outcomes (Wyman, Shiovitz-Ezra, & Bengel, 2018). Ageism provides people with a rationalization for not valuing or taking elders seriously because of perceptions associated with aging. Often times, “old age― can be used as an excuse for common health complaints among the elderly population both by both providers and patients alike. One recent qualitative study on back pain, which is one of the most common medical conditions among older adults, found that many older patients believe that pain is a “normal― part of old age and to be expected (Makris et al. 2015). This poses a great concern for providers because older patients will often not seek medical evaluation for these physical ailments. Elders are less likely to seek treatment for unmet medical needs, due to low expectancies of being helped because of their advanced age (Wyman, Shiovitz-Ezra, & Bengel, 2018). These situations can be chalked down to common
- 2. bias about the elderly population being frail and interdependent. A culturally-sensitive approach to caring for the elderly population promotes independence and autonomy, which is something that this population often struggles with as the body ages and becomes less resilient. Understanding the patient’s underlying values, beliefs, and identities can help establish provider awareness of each individual as a unique, aging patient. The older patient`s home, cultural landscape, employment history and mother tongue are important for an elderly`s subjective health history and their individual assessment of quality of life (Minde , 2015). Not only does this establish a relationship and connection with the patient, but displays a sense of respect while avoiding stereotyping. The culturally competent health worker needs to understand his/her views of this population, as well as those of the patient, while avoiding stereotyping, ageism, and misapplication of knowledge (Minde , 2015).Client ScenarioI had the pleasure of caring for a Hispanic eighty-eight-year-old female who was brought into the hospital by her granddaughter after staying at home for the past two days with right-sided upper extremity paresthesia and neck pain. This patient became a stroke alert, due to the paresthesia presentation in the emergency department. She was consequently educated on stroke-like symptoms and possible treatment options for stroke. When asked why she did not call for emergency help, she persisted to respond that she was “old and did not want to be treated because she was old―. So, although she ended up have cervical radiculopathy, she equated her symptoms to a “normal― aging process and expected health deteriorations as a natural part of life. Her granddaughter further explained that she was the matriarch of the family and was expected to “be strong― and not seek medical help for “getting older―. Working with racial/ethnic groups such as Latinos requires knowledge of additional diversity factors such as acculturation and familial structural beliefs that can have direct influence on aging issues, end-of-life concerns, and physical illness (Tazeau, 2018). This challenges providers to have multicultural knowledge on how to address health concerns, advocacy, and health promotion in a culturally competent, gerodiverse manner.ReferencesMinde , G. (2015). As part of our health assessment, based on a patient’s culture, we are more likely to expect certain illnesses that lead us to direct our health promotion and provide care. Because the elderly are classified as a frail population and the number of older Americans is increasing each year, it is important that those caring for them can meet their needs. Ideally, they would stay in their homes and care for themselves as long as possible (O’Donoghue, Botha, & Van Rensburg, 2014). However, caregivers may mention that tasks of normal daily living are becoming more and more difficult. In my clinical rotation, we had an 82-year-old patient
- 3. that was very realistic about what he thought he could and could not do following his proposed surgery. It was encouraging and heartbreaking at the same time to hear he and his wife discussing what they thought they could manage at home and then coming to the decision, that as much as it was not their first choice, they knew he would need to go to a rehabilitation facility following surgery. I am sure that this was a difficult choice to make, but both of them knew that for the best possible outcome, due to her failing health as well, he would need a brief stay away from home. As a practice, we were able to be sensitive to these issues and plan for the safest possible discharge plan to eliminate other complications. Key areas of providing cultural and socially sensitive care include open communication between the provider and patient, setting plans for what each persons expectations are for the situation at hand, and understanding that not all patients have the same beliefs, but we have to support those of each patient we are with related to culture (O’Donoghue, Botha, & Van Rensburg, 2014). Because our population is growing older each year, it is more important that ever for us to be prepared for those we will care for. “Nurse practitioners care for patients from a wide variety of cultural backgrounds, and in order to deliver high quality primary care that is meaningful, effective, and cost effective, these providers must develop a greater understanding and appreciation of the social- cultural background of clients, their families, and the environment in which they live― (Elminowski, N., 2015).Reference:Howard, B.
