Over the last 20 years, the use of ultrasound in pregnancy has dramatically increased the numbers of ovarian cysts diagnosed.
The majority of these ovarian cysts in pregnancy either resolve spontaneously or are due to benign conditions.
Ovarian cancer is extremely rare in women of childbearing age and thus most of these cysts can be managed conservatively.
In terms of malignancy potential, those that are malignant are likely to be borderline.
Unless there is a suspicion of malignancy or there is a significant cyst complication, such as torsion, surgery is not indicated.
MRI is a safe and useful tool to help evaluate cysts in more detail in situations where ultrasound provides an inconclusive answer.
If surgery is planned, this should take place during the second trimester to minimise the risk of miscarriage.
Whether surgery is done laparoscopically or using a traditional open approach, it is largely dependent on operator experience and patient preference.
Aspiration of ovarian cysts is only indicated where they appear simple on ultrasound and where they are causing pain or are thought to be obstructing the birth canal.
If surgery does not take place, then ultrasound follow-up during and after pregnancy may be advised accordingly.
Ovarian cyst in postmenopausal
Ovarian cysts in postmenopausal women are initially assessed through measuring serum CA125 level and transvaginal ultrasound scan. The risk of malignancy is evaluated using the Risk of Malignancy Index (RMI), which calculates a score based on menopausal status, ultrasound features, and CA125 level. An RMI score of 200 or higher indicates a higher risk of cancer and warrants further evaluation with CT scan of the abdomen and pelvis and referral to a gynecological oncology team. While CA125 is the primary tumor marker used to calculate RMI, other markers alone are not sufficient for assessment due to low sensitivity and specificity.
Ovarian torsion
Ovarian torsion refers to the rotation of an ovary, cutting off its blood supply. It most commonly affects women ages 20-39 and can occur at any age. Risk factors include ovarian tumors, pregnancy, assisted reproduction, and abnormally large or positioned ovaries. The twisting of the ovary leads to venous congestion and ischemia over time. Patients experience sudden, severe, unilateral abdominal pain that may radiate to the back. Ultrasound and surgery are used to diagnose and treat the condition by detorsion of the ovary within 8 hours to restore blood flow before tissue necrosis occurs. Delayed diagnosis can lead to loss of ovarian function or infection.

Adnexal Masses
1) Adnexal masses are common gynecological findings that are usually benign. Differential diagnosis includes physiologic cysts, endometriomas, fibroids, and malignancies.
