Pain became the 5th vital sign in the 1990s to emphasize its importance in medical care, but this led to overtreatment of pain with opioids. Pain should be regularly assessed for all patients, especially those unable to communicate, using standardized scales. Nurses and doctors are responsible for performing pain assessments to guide treatment plans.
