A MRI scan showed a large 9 cm malignant mass in a patient's left nasal cavity that had infiltrated nearby structures. A biopsy revealed a highly anaplastic malignant tumor with melanin pigmentation. Immunohistochemistry testing was positive for S100 and HMB45, confirming the diagnosis of mucosal melanoma. Molecular testing found the tumor lacked the common BRAF V600E mutation. The large size of the tumor at diagnosis meant it had reached an advanced stage where multimodal treatment options were no longer possible.
