Mayo clinic health system is a non-profit health organization that is based in Rochester, Minnesota and has a large number of employees ranging from 3900 physicians as well as more than 50,000 related health staff. This health care system has qualifications in scientific research and has been able to treat difficult health cases making it be one of the best organizations in the city. In terms of physical settings, Mayo clinic has presence in United States metropolitan areas of Arizona and Florida and also operates associated facilities right through Minnesota and Wisconsin. It has a very large college of medicine which also includes Mayo Medical School and Mayo School of Health Sciences (Ludwig, Viggiano, Mcgill & Oh, 1980).
Mayo clinic provides medical care for thousands of people in the U.S. and around the world; its client's bases are based on appointments as well as referrals by doctors from neighboring clinics. Many of the patients who are treated are serviced on outpatient basis which means that all the evaluations and treatments are done in this clinic. Most patients may return home after treatment while most patients who require inpatient care are always hospitalized in the clinic which is located on different campuses (Laurie, Moertel, Fleming, Wieand, Leigh, Rubin & Malliard, 1989).This medical facility has been able to provide best care to every patient and this is based on thorough clinical practice, research and education which are the secrets to success. Its model of care is defined by quality medical care delivery which includes meeting the needs of the patients. Delivery is based on care provided with compassion and trust among the physicians as well as respect.
According to the level of clinical activity, Mayo clinic is the world's largest multispecialty group medical practice which has been able to combine research, education and clinical practice to realize its activities of providing health care needs to patients and clients. It fosters team-oriented care and accountability hence has promoted absolute clinical excellence which has increased the level of clinical activities in this institution. Abstract
Blessed Healthcare Facility is a newly emerging healthcare center located in Miami. This is a privately-owned hospital, which is administered and managed according to the Scheme of the Management approved by the Order of the High Court. This facility a total of 120 beds with 100 in-patient beds and 20 daycare beds. The inpatient specialties care includes gynecology, general surgery, and general medication. These are just but a few services being offered by this healthcare facility. Other services being offered include the Day Surgery; Chaplaincy services; physiotherapy and pharmaceutical services; radiology and the laboratory services; dietetic services, which are involving the provision of the nutritional assessment, education, and assessing the patients based on their nutritional status; and the consultant out-patient clinics. There is also service related to the continence management, control of the infections, palliative care as and the management of pain; and the clinical nurse's specialists in common illness such as diabetes.

Analysis of the Services
With regard to the Consultant Out-Patient Clinics, the goal of this facility is to move closer to home. This is aimed at adhering to the hospital policy which recommends for closer movement to home by the children specialist of this facility to ensure that the general pediatrics outpatient services are effectively provided. This approach is aimed at reducing the non-attendance during appointments. It also ensures that services are provided to more accessible regions thus helping in the creation of new physical clinic capacity. The goal of offering such kind of services is to help in increasing the number of pediatrics who are brought to the facility for clinic services (Hazel & Kussel, 2019).
With regard to Diagnostic Radiological and Laboratory services, there are pathological tests as well as x-ray examinations. The radiology department is providing high-quality diagnostic service to both in and outpatients. There are also radiology services for the Daycare, and these services are aimed at helping to diagnose the treatment. This, therefore, implies that most of the tests are done within the facility and patients are not referred to other facilities for laboratory procedures. THE CLEVELAND CLINIC has long had a reputation for medical excel-
lence and for holding dov în costs. But in 2009 Delos "Toby" Cos-
grove, the CEO, examined its performance relative to that of other
hospitals and admitted to himself that inpatients did not think
much of their experience at its flagship medical center or its eight
community hospitals—and decided something had to be done.
Over the next three years the Clinic transformed itself. Its overall
ranking in the Centers for Medicare & Medicaid Services (CMS) sur-
vey of patient satisfaction jumped from about average to among the
top 8% of the roughly 4,600 hospitals included. Hospital executives
from all over the world now flock to Cleveland to study the Clinic's
practices and to leam how it changed.
The Clinic's journey also holds lessons for organizations outside
health care—ones that until now have not had to compete by cre-
ating a superior experience for customers. Such enterprises often
have workforces that were not hired with customer satisfaction in
mind. Can they improve the customer experience without jeopar-
dizing their traditional strengths? The Clinic's success suggests that
they can.
The Cleveland Clinic's transformation involved actions any
organization can take. Cosgrove made improving the patient ex-
perience a strategic priority, ultimately appointing James Merlino,
a prominent colorectal surgeon (and a coauthor of this piece), to

lead the efiFort. By spelling out the problems in a sys-
tematic, sustained fashion. Merlino got everyone in
the enterprise—including physicians who thought
that only medical outcomes mattered—to recognize
that patient dissatisfaction was a significant issue
and that all employees, even administrators and
janitors, were "caregivers" who should play a role in
fixing it. By conducting surveys and studies and so-
liciting patients' input, the Clinic developed a deep
understanding of patients' needs. It gave MerUno a
dedicated staff and an ample budget with which to
change mind-sets, develop and implement processes,
create metrics, aind monitor performance so that the
organization could continually improve. And it com-
municated intensively with prospective patients to
set realistic expectations for what their time in the
hospital would be like.
These steps were not rocket science, but they
changed the organization very quickly. What's more,
fears expressed by some physicians that the initia-
tive might conflict with efforts to maintain high qual-

THE CASE OF DAVID Z. AND AMYOTROPHIC LATERAL SCLEROSIS (ALS)
David, who has suffered with ALS for 20 years, is now hospitalized in a private religious hospital on a respirator. He spoke with his physician before he became incapacitated and asked that he be allowed to die if the suffering became too much for him. The physician agreed that, while he would not give David any drugs to assist a suicide, he would discontinue David’s respirator if asked to do so. David has now indicated through a prearranged code of blinking eye movements that he wants the respirator discontinued. David had signed his living will before he became ill, indicating that he did not want extraordinary means keeping him alive.
The nursing staff has alerted the hospital administrator about the impending discontinuation of the respirator. The administrator tells the physician that this is against the hospital’s policy. She states that once a patient is placed on a respirator, the family must seek a court order to have him or her removed from this type of life support. In addition, it is against hospital policy to have any staff members present during such a procedure. After consulting with the family, the physician orders an ambulance to transport the patient back to his home, where the physician discontinues the life support.

Efficiency in health care systems is an important aspect of service delivery. Every health facility has a variety of schedules through which health services are offered to the clients. However, these services must be exhaustive enough to display the performance of the health facility with respect to the services it offers. Therefore, each health institution has a number of criteria through which the performance in the provision of services can be established. The facility thereafter uses these results to make adjustments and improve on their services to ensure the highest levels of health service provision are achieved. Mayo Clinic is known for their top notch doctors and reliable performance. They strive to make sure they are in the top 10 percent for not only performance, but quality measures as well. Mayo Clinic makes sure all patients are comfortable during their stay and well educated on their overall health. They ensure patient satisfaction by asking all patients about their stay and what could be improved. Quality measures are measured by outcomes and process measures, and also quality rankings (Marszalek-Gaucher & Coffey, 1993).
How Quality is measured
Mayo clinic offers a wide variety of health services. The divergent characteristic of these services calls for different approaches in the measurement of quality of their services. The health practitioners at each service delivery point in the clinic have orders to constantly record quality measures that help in the classification of services. Basically, the measurement of quality aspects in the clinic can be done by the health practitioners themselves or through a quality personnel who reviews the data on the services offered at each point.

Increasingly medical practice is coming under intense scrutiny as to what is appropriate and affordable care, including serious considerations of patient safety issues and protection. Medical professionalism must be consciously adhered to as we try and find the best health care for our patients at the best value and outcomes for our patients themselves, and also for society at large. In view of escalating health care costs, physician autonomy to practice as he or she likes or deems fit has now come under siege with more and more performance monitoring, not just for appropriateness, but also for outcomes, necessity and cost-effectiveness. Physician' vested interests must be tempered with evidence-based benefits or at least be associated with no increase in harm or incur affordability issues. Fraudulent physician malfeasance are now being uncovered via whistle-blowers, or through greater more meticulous audit of various validated performance measures, and those physicians found to have flouted these due to pecuniary self-interests, overuse of tests or procedures have been found guilty and sanctioned with heavy fines, return of reimbursements as well as imprisonment, and erasure from medical registries and the removal of license to practice.

I enjoyed reading your post and I completely agree with your points. I would like to comment on one of your points regarding controlling the quality of care by using independent contractors. There are benefits to this initiatives, but there could be disadvantages too. The benefit of using teams of experts that you did not hire saves the organization costs associated with keeping full time employees, and patients are seen in a timely manner (In some cases). Many years ago, some departsments in a HMO that I work for had access problem such as patients having to wait 3-4 weeks when they need to see a specialist within the organization. No patient with ear infection or difficulty swallowing wants to wait for 3 three weeks to be seen.
We were sending patients outside for urgent MRIs, CT scans, as well as to different specialists even though the company has capabilities to perform some of these functions in house. Apart from the rising costs this created, the level of patients dissatisfaction went through the roof as some patients get to their appointments and were told that referrals that were to be autofaxed to the outside vendors were never received. Some patients were sent away (no referral, no service). Our Utilization Management department was bombarded with approving these external referrals. We have improved, regrouped, and expanded. State of the art facilities were built and still continue to be built, More physicians, nurses, and support staff were hired, and our patient satifaction rate has grown greatly. For example, we used to send our deaf patients to John's Hopkins Hospital for cochlear implants which cost way over hundred thousand dollars, but that's done in house now
