Any neonate, regardless of birth weight, size, or gestational age, who has a
greater than average chance of morbidity or mortality, especially within the first

CONCEPT OF RISK
CONCEPT OF RISK Identification of infants at risk A number of biological and
sociological factors that appear to indicate high-risk status have been identified
in the literature, 64 Concern with early identification of these factors has resulted
in the development of several screening devices. These include Parmelee's
Obstetric Complications Scale, which is used to predict 42 the effect of maternal
factors on the unborn child. The 40,63 Apgar Scoring System¹ (Chapter 9) is used
to assess fetal and perinatal factors. A number of developmental and
neurobehavioral assessments, such as the Brazelton Neonatal Behavioral
Assessment Scale¹0 (Chapter 11), the Dubowitz Neurological Assessment of the
Preterm and Full-term Newborn Infant,20 and Als' Assessment of 20 Preterm
Infant Behavior (APIB),2 are used to monitor the progress of the infant beyond
the delivery room.
