Lipogranuloma has been reported to develop after introduction of lipoid material into the body accidentally, self injection or as a part of medical treatment. From the outset these lesions confuse the physicians with the neoplastic growths unless the history is elicited carefully. Such lesion as a result of exposure to high pressure diesel injury to hand is hardly reported. One of our patients sustained such injury and developed Lipogranuloma of hand. Surgical treatment was offered to this patient in multiple stages and the specimen was found to have Lipogranuloma microscopically. This case was unique because the lesion involved the digit circumferentially which alarm meticulous surgi-cal treatment in order to maintain the neurovascular integrity and the function as well.

The use of adipose tissue transfer in plastic and reconstructive surgery is not new and has been studied for more than a century but problems such as unpredictability in results and a low rate of graft survival due to partial necrosis were always among major concerns. However, emerging information regarding the potential of adipose derived stem cells, new methods of cell extraction, graft preparation and injection techniques have increased the popularity of fat transfer and the efforts toward development of cell based therapies for various diseases from Adipose Derived Stem Cells (ADSC’s) and Stromal Vascular Fraction (SVF) of the adipose tissue. Although the mechanism of action of those stem cells is not fully known, their paracrine activities and transformation to various cell types can be responsible for reported clinical outcomes [1,2]. Breast cancer is the most common malignancy distressing females in the world. The bone metastases are one of the most common site of disease dissemination. Up to 75% of stage IV Breast cancer patients developing skeletal metastases [1] metastatic bone disease can result in skeletal related events (SREs), including humoral hypercalcaemia of malignancy (HHM), pathological fractures, spinal cord compression, and pain [2]. The median time from bone metastases diagnosis to first SRE can be as short as 1.8 months, with the incidence of SREs growing considerably during the first 12 months following diagnosis [3]. This Bone metastases affect patient survival, movement, and quality of life by negative impact. The molecular mechanisms involved in the metastasis, colonization, and proliferation of breast cancer cells in bone are multifaceted and include crosstalk between breast cancer cells and the bone microenvironment. The capability of metastatic breast cancer cells to capture normal biological processes involved in bone remodeling is a key driver of osteolytic and osteoblastic bone lesions. As such, our understanding of how breast cancer cells manipulate normal bone remodeling pathways is essential for the development of new therapeutic agents to improve patient outcomes.

ABSTRACT- Introduction: Surgical Site Infections (SSI) still remains a significant problem following an operation
and the third most frequently reported nosocomial infections. SSI contributes significantly to increased health care costs in
terms of prolonged hospital stay and lost work days.
Objective: The current study was undertaken to identify incidence of SSI and the risk factors associated with it, and the
common organism isolated and its antibiotic sensitivity and resistance.
Material and Methods: A total number of 3211 patients admitted in general surgical wards for elective surgery in the
study period, out of which 1225 were clean and clean contaminated cases, fulfilling our study criteria. Totally 56 cases
had surgical site infections which had been taken up for this study. Wound discharges were sent for culture and sensitivity.
Results and Conclusions: The overall infection rate was 4.57%. The SSI rate was almost equal in clean surgeries and
clean contaminated ones. Superficial surgical site infections in the most commonest type and accounted for about 66.07%
of all the SSI’s and deep surgical site infection accounted for about 25% with 8.92% was organ space. The most
commonly isolated organism from surgical site infections was staphylococcus aureus followed by pseudomonas and then
E. coli. Drains, prosthesis usage and other risk factors of SSI have been identified. Most of the organisms which were
isolated were multidrug resistant. The high rate of resistance to many antibiotics underscored the need for a policy that

Our case describes an unusual presentation of dermatomyositis
in a patient with ovarian carcinoma. Th e eruption appeared as a venous stasis like dermatitis. Th e temporal sequence of onset after chemotherapy administration suggested a possible drug-induced process. However, in the context of underlying ovarian carcinoma, a paraneoplastic process offered an alternative explanation for
dermatomyositis.

Abstract— Joint replacement operations which are applied to reduce the pain and increase the movement capacity are among the surgical procedures that are used mostly nowadays. Even though a dramatic recovery is seen in the life of the patient after total knee prosthesis, possible prosthesis infection increases cost and causes high morbidity. This study was conducted with the aim to determine rates of surgical site infection after performing primer total knee prothesis operation in our clinic. Furthermore, it has been intended to understand risk factors which may cause infection and then take precautions. This study was conducted from January 2008 to January 2013, 252 knees underwent primary total knee arthroplasty operations. Among these patients infection rates, relationship to risk factors and infection treatments were analysed. It was observed that iIn 252 knees, 10 (4%) superficial infections were found, debridement and antibiotics were applied to 3 knees out of 10 and only antibiotic treatment was applied to the rest 7 knees. Deep infection was detected in 4 knees (1,6%) out of 252. Acute deep infection in 1 knee and recovery was provided with debridement and intravenous antibiotics treatment. Late deep infection was not detected in any of patients. Delayed deep infection was detected in 3 (1,1%) of these knees though. Among all risk factors only increased body mass index showed increased superficial wound infection rate. It can be concluded that among the factors like rheumatoid arthritis, diabetes, age, gender, body mass index, just body mass index has an impact on superficial infection rate. Our infection rates were comparable to rates mentioned in universal literature for primary total knee replacement operations.

Two cases are presented here, one underwent Saucerization and then primary split thickness skin grafting. Another un-derwent I&D for her carbuncle. They were followed up for 8 weeks to assess their outcome. Saucerization produced the shortest length of hospital stay while I&D resulted in shortest wound healing. As a new modality of treatment now-a-days two new modalities gaining popularity for better cosmetic purpose: primary split thickness skin grafting & transposition of local skin/musculocutaneous flap.

Abstract—In Italy the hydatid disease is more prevalent and new cases are highlighted more frequently in Sicily, Sardinia, (Italy). Aim of this study is to put the indication in search of iaditea nature in both spleen swelling and muscle tendon.
Material and Method Patients observed during the period 2007-2009 at the Surgical Clinic III and Digestive Surgery, Policlinico G Rodolico were explored for Hydatid cyste at various sites. Diagnosis of cysts ecchinococcus occurred primarily for various four reasons either for compression of bodies involved or for eosinophilia or for instrumental investigation or for anaphylactic reaction to rupture of cysts. Biological diagnosis is based on serology rather than isolation of the parasite (indirect diagnosis);
Results Patients attended during the period 2007-2009 Hydatid cyst was found in 0.5% of all cases in liver along with 4 in the lung, 3 in splenic, 2 in the mammary and 2 in the chest wall No 2. The Surgical treatment with the complete removal of the cyst with a satisfactory postoperative course in the absence of cases of relapse of the disease and by following the therapeutic act, the assumption of mebendazole 50mg / kg / day for 3 weeks at a dose of 400mg for 4 months
Conclusions There is a need to define diagnostic methods with high specificity and sensitivity, which can provide a valid diagnostic aid for the cases clinically difficult to diagnose. And the final diagnosis must then also be based on the development of immunological methods that allow the determination of specific antibodies in the serum and their titration and / or the circulating antigen determination.

Problem wounds represent a significant and growing challenge to our healthcare system. The incidence and prevalence of these wounds are increasing in the population resulting in growing utilization of healthcare resources. These problem wounds may arise from excessive pressure, trauma, venous insufficiency, diabetes mellitus, vascular disease, or prolonged immobilization leading to its difficult management with significant increases in cost, disability, and liability. Healing of such wounds is a dynamic pathway requiring the presence of oxygen for optimal restoration of tissue integrity and function needs good building blocks for repair and a good transport system to get the building blocks to the site of action. Hyperbaric oxygen therapy (HBOT) is used as a therapeutic modality which leads to an increase in tissue oxygen pressures at the wound site and hence allowing the reversal of a hypoxic state by increasing the oxygen diffusion within the plasma, consequently promoting angiogenesis, encouraging fibroblastic activity and supporting the tissues to resist against bacteria. It is used as an adjunctive treatment to enhance best-practice wound care and employing HBOT in a directed and appropriate way can significantly enhance wound healing efforts. We review a case of 17 year old girl with Grade I pressure ulcer over her right heel since 6 years and role of hyperbaric oxygen therapy as an adjunctive for its management.

Necrotizing fasciitis of the perineum and external genitalia is a
life-threatening infective gangrene, primarily seen in adults but
relatively rare in children. We present a nine-year-old male child
with spinal bifida and double incontence who was admitted at our
hospital due to gangrenous right hemi-scrotal ulcer extending to
the right thigh. It was proceeded with painful swollen hemi-scrotum 2wks prior to admission. We treated him aggressively with
broad spectrum antibiotics and early surgical debridement. Being
paraplegic with double incontinence hence spending most of the
time dressed with diapers we therefore think of poor hygiene and
the diaper rash as the etiological factors. Early surgical debridement with appropriate antibiotics and aggressive supportive care
usually gave good results.

Two cases are presented here, one underwent Saucerization and then primary split thickness skin grafting. Another un-derwent I&D for her carbuncle. They were followed up for 8 weeks to assess their outcome. Saucerization produced the shortest length of hospital stay while I&D resulted in shortest wound healing. As a new modality of treatment now-a-days two new modalities gaining popularity for better cosmetic purpose: primary split thickness skin grafting & transposition of local skin/musculocutaneous flap.

Abstract—In Italy the hydatid disease is more prevalent and new cases are highlighted more frequently in Sicily, Sardinia, (Italy). Aim of this study is to put the indication in search of iaditea nature in both spleen swelling and muscle tendon.
Material and Method Patients observed during the period 2007-2009 at the Surgical Clinic III and Digestive Surgery, Policlinico G Rodolico were explored for Hydatid cyste at various sites. Diagnosis of cysts ecchinococcus occurred primarily for various four reasons either for compression of bodies involved or for eosinophilia or for instrumental investigation or for anaphylactic reaction to rupture of cysts. Biological diagnosis is based on serology rather than isolation of the parasite (indirect diagnosis);
Results Patients attended during the period 2007-2009 Hydatid cyst was found in 0.5% of all cases in liver along with 4 in the lung, 3 in splenic, 2 in the mammary and 2 in the chest wall No 2. The Surgical treatment with the complete removal of the cyst with a satisfactory postoperative course in the absence of cases of relapse of the disease and by following the therapeutic act, the assumption of mebendazole 50mg / kg / day for 3 weeks at a dose of 400mg for 4 months
Conclusions There is a need to define diagnostic methods with high specificity and sensitivity, which can provide a valid diagnostic aid for the cases clinically difficult to diagnose. And the final diagnosis must then also be based on the development of immunological methods that allow the determination of specific antibodies in the serum and their titration and / or the circulating antigen determination.

These problem wounds may arise from excessive pressure, trauma, venous insufficiency, diabetes mellitus, vascular disease, or prolonged immobilization leading to its difficult management with significant increases in cost, disability, and liability. Healing of such wounds is a dynamic pathway requiring the presence of oxygen for optimal restoration of tissue integrity and function needs good building blocks for repair and a good transport system to get the building blocks to the site of action. Hyperbaric oxygen therapy (HBOT) is used as a therapeutic modality which leads to an increase in tissue oxygen pressures at the wound site and hence allowing the reversal of a hypoxic state by increasing the oxygen diffusion within the plasma, consequently promoting angiogenesis, encouraging fibroblastic activity and supporting the tissues to resist against bacteria. It is used as an adjunctive treatment to enhance best-practice wound care and employing HBOT in a directed and appropriate way can significantly enhance wound healing efforts. We review a case of 17 year old girl with Grade I pressure ulcer over her right heel since 6 years and role of hyperbaric oxygen therapy as an adjunctive for its management.

Necrotizing fasciitis of the perineum and external genitalia is a
life-threatening infective gangrene, primarily seen in adults but
relatively rare in children. We present a nine-year-old male child
with spinal bifida and double incontence who was admitted at our
hospital due to gangrenous right hemi-scrotal ulcer extending to
the right thigh. It was proceeded with painful swollen hemi-scrotum 2wks prior to admission. We treated him aggressively with
broad spectrum antibiotics and early surgical debridement. Being
paraplegic with double incontinence hence spending most of the
time dressed with diapers we therefore think of poor hygiene and
the diaper rash as the etiological factors. Early surgical debridement with appropriate antibiotics and aggressive supportive care
usually gave good results.

1. Abstract
Lipogranuloma has been reported to develop after introduction of lipoid material into the body ac-
cidentally, self injection or as a part of medical treatment. From the outset these lesions confuse the
physicians with the neoplastic growths unless the history is elicited carefully. Such lesion as a result
of exposure to high pressure diesel injury to hand is hardly reported. One of our patients sustained
such injury and developed Lipogranuloma of hand. Surgical treatment was offered to this patient in
multiple stages and the specimen was found to have Lipogranuloma microscopically. This case was
unique because the lesion involved the digit circumferentially which alarm meticulous surgi-cal
treatment in order to maintain the neurovascular integrity and the function as well.
3. Introduction
Lipogranuloma, also known as paraffinoma or oil granuloma
has been observed to develop in various parts of the body in re-
sponse to the local infiltration of lipoid material either acciden-
tally or iatrogenically in an attempt to augment the soft tissue.
Some individuals reported this pathology after self injection of
petrolatum jelly into the genitalia in order to increase its girth.
But Lipogranuloma involving hand has rarely been observed.
We received a patient with lipogranuloma which involved the
thumb and first web space of right hand.
4. Case Report
A 29 years old male presented with a diffuse swelling and in du-
ration of his right thumb extending into the first web space and
anatomical snuff box. He had a history of high pressure diesel
injury to the pulp of his right thumb while fixing a motor vehicle
engine. Initially he had inflammatory reaction over the site and
loss of distal part of his pulp for which he received conservative
treatment. Gradually the swelling subsided partially and the pulp
healed. After 6 months of injury he noticed the entire thumb

with first web space and anatomical snuff box area changed from
soft to firm and the swelling increased slowly. On examination we
found the involved part swollen uniformly (Figure1A &
Figure1B) with a firm consistency. Based on history and clini-cal
findings we diagnosed it to be lipogranuloma of hand. Since the
area affected bears enormous significance in terms of func-tion we
planned to remove the granuloma in piece meal and in stages.
During surgery we found a thick layer of fibrous kind of tissue in
the subcutaneous tissue plane, the skin and the underly-ing
muscles being spared. We removed the abnormal tissue from the
first web space (Figure 2A & Figure 2B) in the first sitting and
subjected the tissue to Histopathological examination. The
Histopathological evaluation revealed fibrocollagenous tissue
studded with numerous non-caseating granulomas. The later in-
volved histiocytes, epitheoid cells and multinucleated giant cells
engulfing fat droplets. The intervening stroma showed several fat
droplets surrounded by histiocytes and giant cells. There was no
caseation necrosis and special stain demonstrated no fungi or acid
fast bacilli. There was also no evidence of malignancy in the
specimen. These microscopic findings (Figure 3) were con-sistent
with the features of lipogranuloma. After two months of
