Learning Need
What learning need have I identified?
I would like to practice more hands on nursing skills in order to generate confidence to help me work
independently and in a team as a nurse. These skills include but are not limited to: mixing liquid
medications with IV N/S, administering oral medications, and administering an IM injection.
Furthermore, I would like to work on building strong therapeutic rapports/relationships with my
patients this semester.
Learning Goal #1
What do I want to learn?
I would like to learn how to mix medications
from ampules in to small IV bags and hook up
the tubing and start the IV medication
adminitration. This is something that I have
so far not had an opportunity to work with is
with IV bags. Last semester we had only
learned the skill in the back half of the
semester and I did not have any patients on
the floor I was placed who had Ivs. I would
like to achieve this goal of mixing liquid
medications (at least one) with IV saline by
mid-term.
Goal #1: Activities and Timeframes
How am I going to achieve my goal?
I am going to review my notes and videos which
related to mixing IV medications so that I can write
out or recall the steps from the top of my head. As
well I will seek out opportunities to practice this
skill so that I get the hands on experience and can
get comfortable with doing it. I would like to
achieve this goal of mixing liquid medications (at
least one) with IV saline by mid-term.
Learning Goal #2
What do I want to learn?
I would like to safely administer oral medications
to at least 5 patients on A5 at the hospital. This
will require me to
be familiar of my 8 rights and three checks and to
have a strong understanding of the medications
which I am looking to deliver including what the
medications I am administering do for the patient
(ideally), what adverse effects they can cause, and
what to teach the patient about the medications.
Goal #2: Activities and Timeframes
How am I going to achieve my goal?
I will achieve my goal of safe administration of oral
medications to at least 5 different patients by
actively seeking out opportunities at clinical to
administer medications. I can use my drug guide,
drug card template, books, students, teachers,
other nurses and even patients to help me gain a
better understanding about the medications that I
am administering. Nurses, students and teachers
can provide first hand information related to some
of the medications that I will be administering as
some or many have administered them before. I
would like to accomplish this goal by mid-term.

What do I want to learn?
I would like to develop a strong therapeutic
relationship with one of my patients at clinical this
semester. I will feel that I have accomplished this
when I see more than one of the following:
-The patient remembers my name and uses it

How am I going to achieve my goal?
I am going to achieve this goal by being
professional, polite, and friendly. Whenever
possible I will make time every day to ask the
patient about how they are feeling (especially in
relation to their admitting diagnosis) and will try
to get to know them a bit by remembering their

-The patient smiles when they see me and/or
demonstrates a positive attitude when I am in

emotional support. I can also tell them (the
patients) that I work as a PSW and that I am an
expert in helping people with their ADLs. Again
what I am looking to see to be able to say that I
have achieved my goal of developing a strong
therapeutic relationship with a client is to see
three or more of the following with the same
patient:

hours I am working and the program I am taking
-The patient smiles when they see me and/or

My goal is to achieve this by the end of the
semester while working on it throughout the
semester. If I feel I have a patient who is not fond
of me or is completely indifferent towards me, this
could mean they are depressed or that I am doing
something wrong. If I observe this and it concerns
me, I will talk to my clinical instructor and tell her
what is going on and get her advice with the hope
being that I can help the patient to improve their
experience at the hospital and that I will improve
my skills and knowledge related to nursing.

What do I want to learn?
I would like to administer an intramuscular
injection to a patient as last semester I only got to
give three injections in total and all were
preloaded subcutaneous medications (two of
which were to the same client). I would like to do
this because from what I understand they are a
more painful type of injection, this means it is
more important to know what you are doing (in
order to limit the discomfort caused to the client)
and also it will be good experience to inject from a

I am going to achieve this goal by the end of
the semester. I will review my videos, and
notes and ask my peers, clinical instructor
and nurses on B4 about the MI injection in
order to prepare myself to administer it

comprehensive progress note in the patient’s
chart. This skill is important to me because it is
important to know what to include in a progress
note as I will likely be doing them a lot

is imperitive to be able to convey the important
and less important parts of a patients day for the
sake of other nurses, doctor, students and others
who will have to rely on them after to determine
how to continue the care of the patient.

throughout my clinical shifts in order to be able to
include it in the progress note later on. I will also
ask questions of clinical instructor when we are
writing these notes to get her advice.

perrry potter book “Canadian fundamentals of
nursing” for further information on progress
notes.
This goal I would like to achieve by the end of the
semester, and will look for my CI’s feed back to
indicate that it has been successful as well as my
own feelings towards my progress notes.
