1. TITLE: PREDICTORS OF WEIGHT LOSS FOLLOWING ENDOLUMINAL GASTRIC POUCH REDUCTION AFTER
FAILED GASTRIC BYPASS
ABSTRACT BODY:
Background: Endoluminal gastric pouch reduction surgery (EGPRS) using StomaphyX attempts to plicate dilated gastric
pouches and gastrojejunal anastomoses (GJA) in patients who have regained weight after gastric bypass (GB).
Methods: A total of 168 patients underwent EGPRS using StomaphyX at three specialized U.S. bariatric centers over a two
year period. Patients were offered StomaphyX if they had a dilated gastric pouch and/or GJA on endoscopy and/or
fluoroscopy. A correlation analysis was conducted to identify a potential relationship between clinical outcomes and pre-
EGPRS patient characteristics.
Results: At a 6-month (4 SD) follow-up, complete data were available for 124 patients (94% female, age 49.3 +/-10.4 years
and BMI 38.9+/-7.6). Patients experienced 87%(+/-21%) excess BMI loss (EBL), corresponding to a weight loss of
125.9(+/-50.1) lb after initial GB and then regained 58.5(+/-37.1) lb, within 7.1(+/-3.7) years after GB.
The EGPR procedure resulted in a 50%(+/-24%) reduction of gastric pouch size through formation of 22 (+/-9) plications.
On average, patients lost 25 lb (range +23 (gain) to 183 lb) which corresponded to 18% EBL (8 to 23) or 43% of regained
weight. Weight loss after StomaphyX was significantly correlated with weight loss after initial GB (r=0.294, p=0.001) and
BMI at the time of StomaphyX (r=0.236; p=0.009). Weight loss did not correlate significantly with age, gender, years after
GB, the number of fasteners and percent reduction of the gastric pouch.
Conclusion: Weight loss following gastric bypass and BMI appeared to be significant predictors of weight loss after EGPRS
with StomaphyX.
