Dokumen ini menjelaskan pedoman mengenai dokumen informasi produk (DIP) yang harus dimiliki sebelum melakukan notifikasi kosmetika di Indonesia. DIP terdiri dari empat bagian utama: dokumen administrasi, data mutu dan keamanan bahan kosmetika, data mutu kosmetika, serta data keamanan dan kemanfaatan kosmetika. Dokumen ini juga mencakup persyaratan dokumen yang diperlukan untuk kosmetika dalam negeri, impor, kontrak, dan lisensi.

PEDOMAN DOKUMEN INFORMASI PRODUK
I. PENDAHULUAN
Pemohon notifikasi harus memiliki Dokumen Informasi Produk (DIP)
sebelum melakukan notifikasi Kosmetika. DIP harus disimpan dan siap
ditunjukkan apabila sewaktu-waktu diaudit oleh petugas Badan Pengawas
Obat dan Makanan.
Pedoman DIP merupakan panduan bagi industri Kosmetika, importir
Kosmetika atau usaha perorangan/badan usaha yang melakukan kontrak
produksi dalam menyiapkan dan menyusun DIP bagi Kosmetika yang
akan dinotifikasi. Selain itu Pedoman DIP juga merupakan panduan bagi
petugas dalam melakukan audit.
II. FORMAT DIP
Dokumen DIP terdiri atas 4 bagian, yaitu:
Bagian I : Dokumen Administrasi
Bagian II : Data Mutu dan Keamanan Bahan Kosmetika
Bagian III : Data Mutu Kosmetika
Bagian IV : Data Keamanan dan Kemanfaatan Kosmetika
Isi dari setiap bagian adalah sebagai berikut:
A. Bagian I : Dokumen Administrasi
Dokumen administrasi untuk:
a. Kosmetika dalam negeri, paling sedikit berisi:
1. fotokopi surat izin produksi Kosmetika yang masih berlaku
sesuai dengan bentuk dan jenis sediaan yang dinotifikasi;
2. fotokopi surat pernyataan terkait merek; dan
3. fotokopi sertifikat CPKB yang masih berlaku sesuai dengan
bentuk dan jenis sediaan yang dinotifikasi atau surat
pernyataan penerapan CPKB.
b. Kosmetika impor, paling sedikit berisi:
1. fotokopi Angka Pengenal Importir (API) sesuai dengan
peraturan perundang-undangan;

2. fotokopi surat penunjukan keagenan yang masih berlaku,
dibuat dalam Bahasa Indonesia dan/atau Bahasa Inggris
dan paling sedikit mencantumkan:
a) nama dan alamat produsen/prinsipal negara asal;
b) nama importir;
c) nama produk/merek Kosmetika;
d) tanggal diterbitkan;
e) masa berlaku penunjukan keagenan;
f) hak untuk melakukan notifikasi, impor, dan distribusi
dari produsen/prinsipal negara asal; dan
g) nama dan tanda tangan direktur/pimpinan
produsen/prinsipal negara asal.
3. fotokopi surat perjanjian kerjasama kontrak antara
pemohon notifikasi dengan penerima kontrak produksi yang
disahkan oleh notaris dan mencantumkan merek dan/atau
Nama Kosmetika serta tanggal masa berlaku perjanjian,
untuk usaha perorangan/badan usaha yang melakukan
kontrak produksi dengan industri Kosmetika di luar wilayah
Indonesia.
4. fotokopi Certificate of Free Sale (CFS) untuk Kosmetika
Impor yang berasal dari negara di luar ASEAN, dikeluarkan
oleh pejabat yang berwenang atau lembaga yang diakui di
negara asal yang dilegalisir oleh Kedutaan Besar/Konsulat
Jenderal Republik Indonesia setempat, dikecualikan untuk
Kosmetika Kontrak yang diproduksi di luar wilayah
Indonesia;
5. fotokopi surat pernyataan terkait merek;
6. fotokopi sertifikat CPKB atau surat pernyataan penerapan
CPKB untuk industri yang berlokasi di negara ASEAN;
7. fotokopi sertifikat CPKB untuk industri Kosmetika yang
berlokasi di luar negara ASEAN dan industri Kosmetika di
luar wilayah Indonesia yang menerima kontrak produksi
dengan ketentuan sebagai berikut:
a) diterbitkan oleh pejabat pemerintah yang berwenang
atau lembaga yang diakui di negara asal ;
b) dilegalisir oleh Kedutaan Besar/Konsulat Jenderal
Republik Indonesia setempat;

c) mencantumkan masa berlaku;
d) jika masa berlaku sebagaimana dimaksud pada huruf
c) lebih dari 5 (lima) tahun atau tidak mencantumkan
masa berlaku maka sertifikat dinyatakan berlaku
selama 5 (lima) tahun.
Dalam hal ketentuan sebagaimana dimaksud pada huruf a)
tidak terpenuhi, maka importir harus melampirkan:
1) fotokopi sertifikat CPKB yang dikeluarkan oleh
lembaga sertifikasi terakreditasi yang diakui setara
dengan Good Manufacturing Practice (GMP) ASEAN; dan
2) fotokopi surat izin industri/produksi Kosmetika.
c. Kosmetika kontrak, paling sedikit berisi:
1. fotokopi surat izin produksi Kosmetika dan sertifikat CPKB
yang masih berlaku sesuai dengan bentuk dan jenis
sediaan yang dinotifikasi dari industri penerima kontrak;
2. fotokopi surat perjanjian kerjasama kontrak antara pemberi
kontrak dengan penerima kontrak produksi yang disahkan
oleh notaris dan mencantumkan merek dan/atau nama
Kosmetika serta tanggal masa berlaku perjanjian;
3. fotokopi surat izin usaha di bidang Kosmetika dari pemberi
kontrak; dan
4. fotokopi surat pernyataan terkait merek.
d. Kosmetika lisensi, paling sedikit berisi:
1. fotokopi surat izin produksi Kosmetika yang masih berlaku
sesuai dengan bentuk dan jenis sediaan yang dinotifikasi;
2. fotokopi surat penunjukan atau persetujuan dari
perusahaan pemberi lisensi yang mencantumkan merek
dan/atau nama Kosmetika; dan
3. fotokopi sertifikat CPKB yang masih berlaku sesuai dengan
bentuk dan jenis sediaan yang dinotifikasi atau surat
pernyataan penerapan CPKB.
B. Bagian II: Data Mutu dan Keamanan Bahan Kosmetika
Data tentang mutu dan keamanan bahan kosmetika meliputi:
(1) Data mutu bahan Kosmetika berdasarkan:
a. Spesifikasi masing-masing bahan termasuk spesifikasi air,
bila ada dalam formula;

I. PREFACE
The notification applicant must have Product Information Document (PID) before
conducting Cosmetics notification. The PID must be kept and ready to be shown
anytime if it is audited by the officer of National Agency of Food and Drugs Control.
PID Guidance is guidance for the Cosmetics Company, Cosmetics importer or
proprietorship/company which makes production contract to prepare and arrange PID
for Cosmetics which will be notified. In addition, PID Guidance is also guidance for
officer to audit.
II. PID FORMAT
PID Document consists of 4 parts, they are:
Part I : Administrative Document
Part II : Data of Quality and Safety of Cosmetics Material
Part III : Data of Cosmetics Quality
Part IV : Data of Safety and Benefit of Cosmetics
The contents of each part are as follows:
A. Part I : Administrative Document
Administrative document for:
a. Domestic cosmetics, at least contains:
1. Photocopy of production license of Cosmetics which is still valid in
accordance with the form and kind of cosmetics product which is notified;

5. 2. Photocopy of statement letter concerning the brand; and
3. Photocopy of CPKB certificate which is still valid in accordance with form
and kind of cosmetic product which is notified or statement letter of CPKB
implementation.
b. Imported cosmetics, at least contains:
1. Photocopy of Importer ID Number in accordance with legislation;
2. Photocopy of agent appointment letter which is still valid, is made in
Indonesian Language and/or English and at least contains:
a) Name and address of the producer/principal of the origin country;
b) Name of the importer;
c) Product name/brand of the Cosmetics;
d) Date of issue;
e) Validity period of agent appointment;
f) Rights to notify, import, and distribute from the producer/principal of
origin country; and
g) Name and signature of the director/producer chairman/principal of the
origin country.
3. Photocopy of agreement letter of contract cooperation between the
notification applicant and production contract recipient which is legalized
by notary and contains brand and/or name of Cosmetics and validity period
of the agreement, for proprietorship/business entity which makes
production contract with Cosmetics company outside Indonesian territory.
4. Photocopy of Certificate of Free Sale (CFS) for Imported Cosmetics which
is from countries outside ASEAN, issued by the authorized officer or
recognized institution in the origin country and legalized by local
Embassy/Consulate General of Republic of Indonesia, excepted for
Contract Cosmetics which is made outside Indonesian territory;
5. Photocopy of statement letter concerning the brand;
6. Photocopy of CPKB certificate or statement letter of CPKB implementation
for company which is located in ASEAN countries;

6. 7. Photocopy of CPKB certificate for Cosmetics company which is located in
ASEAN countries and Cosmetics company outside Indonesia which
receives production contract under provisions as follows:
a) Issued by the authorized government officer or recognized institution in
the origin country;
b) Legalized by local Embassy/Consulate General of Republic of
Indonesia
c) Contains validity period;
d) If the validity period as stated in letter c) is more than 5 (years) or there
is no validity period, then the certificate is certified to be valid for 5
(five) years.
If the provisions as stated in letter a) are not satisfied, the importer must
submit:
1) Photocopy of CPKB certificate which is issued by accredited
certification institution which is recognized to be equivalent to ASEAN
Good Manufacturing Practice (GMP); and
2) Photocopy of license of industry/production of Cosmetics
c. Contract Cosmetics, at least consists of:
1. Photocopies of Cosmetics production license and CPKB certificate which
are still valid in accordance with the form and kind of Cosmetics product
which is notified from the company which receives the contract;
2. Photocopy of agreement letter of contract cooperation between the contract
grantor and contract recipient which is legalized by notary and contains
brand and/or name of the Cosmetics and validity period of the agreement;
3. Photocopy of license of Cosmetics business from the contract grantor; and
4. Photocopy of statement letter concerning the brand.
d. License Cosmetics, at least contains:
1. Photocopy of Cosmetics production license which is still valid in
accordance with the form and kind of Cosmetics product which is notified;

7. 2. Photocopy of appointment or approval letter from license grantor company
which contains brand/or Cosmetics name; and
3. Photocopy of CPKB certificate which is still valid in accordance with form
and kind of Cosmetics product which is notified or statement letter of CPKB
implementation.
B. Part II: Data of Quality and Safety of Cosmetics Material
Data of quality and safety of cosmetics material contains:
(1) Data of cosmetics material quality based on:
a. Specification of each material including water specification, if the formula
contains water;
