Work Experience:
• October, 2010 - Present: Social Worker, Santuari Hospice: As part of the
leadership team I was very involved with the opening of the hospice. I played a
key role in orientation, policies and accreditation. I provide ongoing education to
the staff and volunteers, as well as support to the team. My communitcation
skills are very effective in communication with all members of the
interdisciplinary team. I lead rounds once a week with all team members
present. The other part of my position is to support those admitted to hospice
and their families/support systems. My assessment skills are excellent and I am
able to support the family and resident as needed. This includes financial
resources with in the community, the writing of wills, Power of Attornies and/or
Personal Directives, as well as emotional support. I also provide bereavement
follow up with families. I also find it very important and rewarding to supervise
students and have had 4 students to date.
• October, 2007 – December 2010: Social Worker, Palliative Consult Team:
The Palliative Consult team follows patients in an acute care setting that have
either just been given a life changing diagnosis or are somewhere along the
journey after their diagnosis. As the social worker for this team, I worked very
closely with our physician and CNS to meet the needs of the patients and their
families. I assisted with advanced care planning and goals of care. I arranged
family meetings to discuss the best course of action for the patient and the
family. Listening and counseling was a key responsibility, as well as connecting
the families with resources in the community. It was also extrememly important
to document and share with the staff as to what was going on with the patient.
My communication skills, both written and verbal, are excellent.
• May, 2007 – October 2007: Casual Social Worker: I covered various acute
care units at various hospitals as needed.

• Duties included assessment of each resident and reporting the needs to the
team. This included a depression screen and a Mini Mental score as well as
emotional support for the resident and their families or assisting with financial
resources.
• I worked closely with the offices of the Public Trustee and Public Guardian to
ensure the rights of the residents were looked after, ensuring they were not
taken advantage of by family or friends, or for those who had no family or
friends.

Work Experience:
• I coordinated and provided acute patient care as a member of the
interdisciplinary care meetings with doctors, nurses and therapists to provide the
patient optimum care and rehabilitation opportunities. I performed social
assessments to identity potential needs for the patients discharge. I provided
intervention and support for patients and families in crisis and high risk cases;
alcohol/drug abuse, child or elder abuse/neglect, etc. I also provided as needed
coverage for home care and in home hospice and participated in training new
employees. I revised standing orders for orthopedic patients to ensure each
patients needs were met for a safe and successful discharge after surgery. I
also supervised a social work student who completed 500 hours under my care.
