FORMATION OF AQUEOUS HUMOUR
Active secretion:
• 80% aqueous humor is secreted by the non-pigmented ciliary epithelium.
• It is an active metabolic processes which needs a number of enzymatic system
It depends on blood pressure in the ciliary capillaries, the plasma oncotic pressure and
the level of IOP
.

BLOOD AQUEOUS BARRIER
• It is formed by tight junction between the cells of non pigmented epithelium and
capillary endothelium of ciliary body .
• It allows only selected pharmacological molecules to pass through the barrier, which is
useful for intraocular infiltration.
NORMAL IOP
• 10 mm of Hg to 21 mm of HG (mean 16 mm of HG).
• (21 mm of Hg is considered the upper limit of normal)
Factors determine the level of IOP:
• Rate of aqueous secretion
• Resistance encountered in the outflow channels
• Level of episcleral venous pressure
Diurnal variation of IOP
Physiological variation of IOP by 3-4 mmHg in morning and evening (high in the morning).
Diurnal fluctuation in normal eye
4 mm of Hg
Diurnal variation in glaucomatous eye
8 mm of Hg

I-CARE
GLAUCOMA
• It is a chronic, progressive optic neuropathy caused by a group of ocular condition
which leads to damage of optic nerve with loss of visual function
PATHOGENESIS OF GLAUCOMATOUS
DAMAGE
• The Indirect Ischaemic theory – high IOP causes death of nerve fibre by interfering
with the microcirculation of the optic nerve head.
• Direct mechanical theory – high IOP directly damages the residual nerve fibres as they
pass through the optic nerve head (stasis of axoplasmic flow)
CAUSES OF GLAUCOMA
• Obstruction of flow of aqueous humor from posterior chamber to anterior chamber,
e.g. pupillary block glaucoma
• Hypersecretory Glaucoma - Excess secretion of aqueous humour, e.g. epidemic dropsy.
• Blockage of angle due to peripheral anterior synechiae - PACG.
• Blockage of angle of AC due to sclerosis of the trabecular meshwork – POAG.
• High episcleral venous pressure e.g. - Sturge Weber syndrome.
CLASSIFICATION OF GLAUCOMA (IN BRIEF)
Open angle glaucoma (OAG)
• Primary (POAG)
• Secondary:
-Paratrabecular (memberane)
-Trabecular (pigment dispersion, pseudoexfoliation syndrome, neovascular, steroid response, hyphema, angle
recession, uveitic)
-Post-trabecular (raised episcleral venous pressure)
Angle closure glaucoma (ACG)
• Primary (PACG)
• Secondary:
-Posterior pushing forces (posterior synechiae with seclusion pupillae, phacomorphic glaucoma)
-Anterior pushing forces (peripheral anterior synechiae, neovascular glaucoma)
CLASSIFICATION OF GLAUCOMA IN MORE DETAIL
ANGLE CLOSURE GLAUCOMAS
PRIMARY OPEN ANGLE GLAUCOMA (POAG)
Symptoms:
• Generally asymptomatic:
• Painless progressive dimness of vision
• Non-specific complaints e.g headache
• Frequent change of presbyopic glass
• Difficulty in dark adaptation
• Diminished field of vision
Classical triad
• Raised IOP
• Cupping of the optic disc
• Characteristic visual field defects (at
least 2 of the 3 signs must be present
to make a diagnosis of POAG)
OPTIC DISC FINDINGS IN POAG
• Cup: disc ratio > 0.3
• Asymmetry of cupping between the two optic nerve head > 0.2
• Narrowing, notching or pallor of the neuroretinal rim (NRR)
• Disc haemorrhage
• Vascular changes
• Laminar dot sign
• Peripapillary atrophy

SEVERITY OF GLAUCOMA
PREDISPOSING FACTORS FOR POAG
• Genetic predisposition
• Race – Black
• Age – Elderly (4–6 decade)
• Myopia
• Diabetes mellitus
Pathogenesis of POAG:
Increase resistance to the outflow of the aqueous at the TBM, especially at the
juxtacanalicular region, this is partially due to age related changes in the tissue

• Applanation/Schiotz tonometry
• Gonioscopy
• Automated perimetry

ALGORITHM FOR TREATING POAG
PRIMARY ANGLE CLOSURE
GLAUCOMA
• PACG includes a spectrum of condition in which peripheral iris moves forwards to
block the TBM at the angle causing a rise of IOP
PREDISPOSING FACTORS FOR PACG
Anatomical factors
• Short eye (hypermetropia)
• Smaller corneal diameter
• Shallow AC
• Relative anterior positioning of lens -
iris diaphragm
• Bigger size of lens
General factors
• Age – 5th decade
• Sex – F:M – 1:4
• Personality – nervous patient
• Season – rainy season
• Family history – may be inherited
• Race – more common in South east
Asians
PRECIPITATING FACTORS OF PACG
In a predisposed eye, any of the following factors may precipitate an attack:
• Dim illumination
• Emotional stress
• Use of mydriatic drugs, e.g. atropine, cyclopentolate, tropicamide
STAGES OF ACG
• Prodromal stage
• Phase of constant instability
• Acute congestive attack
• Chronic angle closure
• Absolute glaucoma
CLINICAL FEATURES OF ACG
Symptoms
• Acute intense pain radiating along the
distribution of 5th cranial nerve
• Headache, nausea, vomiting
• Decreased vision
• Redness, lacrimation, photophobia
Signs
• Lid oedema
• Ciliary congestion
• Hazy, cloudy, insensitive cornea
• Shallow AC
• Mid-dilated vertically oval, non-reacting
pupil
• Raised IOP (40-60mm Hg)
• VA- HM/ PL/PR
• Fundus- Not visualized due to corneal
oedema
• Instillation of glycerine drop clears
cornea
• Fundus- hyperemic disc, disc
TREATMENT OF AN ACUTE ATTACK
• Usually the IOP is >50 mm of Hg
ALGORITHM FOR TREATING ACUTE ACG
• Mannitol (20%) I.V 1.5-2 gm/kg body weight ( rapid drip) or Acetazolamide I.V 500 mg
• If the pupil is still blocked- press over central part of cornea with a sterile cotton bud to
open the peripheral block
• Oral glycerine (50%) with lemon juice TDS can be given
• Pilocarpine 2% drop is started after the IOP is reduced by Mannitol ( in a very high IOP
Pilocarpine can not act on sphincter muscle as it becomes ischaemic)
• Beta blockers- BD
• Timolol maleate 0.5%
• Betoxolol 0.5
• Analgesic systemically
• Steroid drops to reduce congestions
• LASER treatment – once the IOP is reduced and eye becomes quite – YAG LASER peripheral
iridotomy is done
• Surgical treatment – if the medical and LASER treatment fails to control the IOP, then
trabeculectomy is done
.
AFTER EFFECTS OF AN ACUTE ATTACK
• Reduced vision
• Corneal edema
• Shallow AC
• Iris – sector atrophy
• Pupil – irregular
• Anterior capsular opacity, glaucomflecken
• Optic disc – hyperemic
COMMON ANTIGLAUCOMA DRUGS IN
BRIEF
Topical medications:
• Beta blockers: Timolol, Betaxolol, Carteolol, Metipranolol.
-mech: decrease aqueous production
• Prostaglandin analogues: Latanoprost, Travoprost, Bimatoprost
-mech: increases uveoscleral outflow
• Alpha-2 agonists: Brimonidine, Apraclonidine
-mech: decreases aqueous production, increase uveoscleral outflow, neuroprotective
• Topical Carbonic Anhydrase inhibitors: Dorzolamide, Brinzolamide
-mech: decreases aqueous production
SYSTEMIC DRUGS
• Carbonic anhydrase inhibitors: acetazolamide
-mech: decreases aqueous production
• Hyperosmotic agents: glycerol, mannitol, isosorbide
-mech: dehydrates the vitreous
LASER THERAPY OF GLAUCOMA
• Nd:YAG laser Peripheral Iridotomy
Indications:
• POAG (acute ACG, intermittent ACG, chronic ACG)
• POAG with narrow angles
• Secondary ACG (irido-IOL block, irido-vitreal block, subluxated lens with pupil block)
Prophylactic: Fellow eye of patient with PACG, Narrow occludable angles.
SURGICAL TREATMENT OF GLAUCOMA
• Indications for trabeculectomy:
• When IOP is raised to a level where there is evidence of progressive VF or ON
which will threaten quality of visual function, despite adequate medical treatment
• Uncontrolled POAG with maximal medical treatment
-Failure of medial treatment (IOP not controlled with progressive VF or ON damage)
-Side effects of medical treatment
-Non-compliance with medical treatment
.
-Additional considerations:
-Young patient with good quality of vision
-One-eyed patient (other eye blind from glaucoma)
-Family history of blindness from glaucoma
-Glaucoma risk factors (HTN, DM)
• Uncontrolled PACG after laser PI and medical treatment
• Secondary OAG or ACG

  • 15. BLOOD AQUEOUS BARRIER • It is formed by tight junction between the cells of non pigmented epithelium and capillary endothelium of ciliary body . • It allows only selected pharmacological molecules to pass through the barrier, which is useful for intraocular infiltration.
  • 16. NORMAL IOP • 10 mm of Hg to 21 mm of HG (mean 16 mm of HG). • (21 mm of Hg is considered the upper limit of normal) Factors determine the level of IOP: • Rate of aqueous secretion • Resistance encountered in the outflow channels • Level of episcleral venous pressure Diurnal variation of IOP Physiological variation of IOP by 3-4 mmHg in morning and evening (high in the morning).   • 21. GLAUCOMA • It is a chronic, progressive optic neuropathy caused by a group of ocular condition which leads to damage of optic nerve with loss of visual function
  • 22. PATHOGENESIS OF GLAUCOMATOUS DAMAGE • The Indirect Ischaemic theory – high IOP causes death of nerve fibre by interfering with the microcirculation of the optic nerve head. • Direct mechanical theory – high IOP directly damages the residual nerve fibres as they pass through the optic nerve head (stasis of axoplasmic flow)

CAUSES OF GLAUCOMA • Obstruction of flow of aqueous humor from posterior chamber to anterior chamber, e.g. pupillary block glaucoma • Hypersecretory Glaucoma - Excess secretion of aqueous humour, e.g. epidemic dropsy. • Blockage of angle due to peripheral anterior synechiae - PACG. • Blockage of angle of AC due to sclerosis of the trabecular meshwork – POAG. • High episcleral venous pressure e.g. - Sturge Weber syndrome.

CLASSIFICATION OF GLAUCOMA (IN BRIEF) Open angle glaucoma (OAG) • Primary (POAG) • Secondary: -Paratrabecular (memberane) -Trabecular (pigment dispersion, pseudoexfoliation syndrome, neovascular, steroid response, hyphema, angle recession, uveitic) -Post-trabecular (raised episcleral venous pressure) Angle closure glaucoma (ACG) • Primary (PACG) • Secondary: -Posterior pushing forces (posterior synechiae with seclusion pupillae, phacomorphic glaucoma) -Anterior pushing forces (peripheral anterior synechiae, neovascular glaucoma)
  • 25. CLASSIFICATION OF GLAUCOMA IN MORE DETAIL

PRIMARY OPEN ANGLE GLAUCOMA (POAG) Symptoms: • Generally asymptomatic: • Painless progressive dimness of vision • Non-specific complaints e.g headache • Frequent change of presbyopic glass • Difficulty in dark adaptation • Diminished field of vision Classical triad • Raised IOP • Cupping of the optic disc • Characteristic visual field defects (at least 2 of the 3 signs must be present to make a diagnosis of POAG)
  • 28. OPTIC DISC FINDINGS IN POAG • Cup: disc ratio > 0.3 • Asymmetry of cupping between the two optic nerve head > 0.2 • Narrowing, notching or pallor of the neuroretinal rim (NRR) • Disc haemorrhage • Vascular changes • Laminar dot sign • Peripapillary atrophy

PREDISPOSING FACTORS FOR POAG • Genetic predisposition • Race – Black • Age – Elderly (4–6 decade) • Myopia • Diabetes mellitus Pathogenesis of POAG: Increase resistance to the outflow of the aqueous at the TBM, especially at the juxtacanalicular region, this is partially due to age related changes in the tissue

PREDISPOSING FACTORS FOR PACG Anatomical factors • Short eye (hypermetropia) • Smaller corneal diameter • Shallow AC • Relative anterior positioning of lens - iris diaphragm • Bigger size of lens General factors • Age – 5th decade • Sex – F:M – 1:4 • Personality – nervous patient • Season – rainy season • Family history – may be inherited • Race – more common in South east Asians
  • 37. PRECIPITATING FACTORS OF PACG In a predisposed eye, any of the following factors may precipitate an attack: • Dim illumination • Emotional stress • Use of mydriatic drugs, e.g. atropine, cyclopentolate, tropicamide
  • 38. STAGES OF ACG • Prodromal stage • Phase of constant instability • Acute congestive attack • Chronic angle closure • Absolute glaucoma

CLINICAL FEATURES OF ACG Symptoms • Acute intense pain radiating along the distribution of 5th cranial nerve • Headache, nausea, vomiting • Decreased vision • Redness, lacrimation, photophobia Signs • Lid oedema • Ciliary congestion • Hazy, cloudy, insensitive cornea • Shallow AC • Mid-dilated vertically oval, non-reacting pupil • Raised IOP (40-60mm Hg) • VA- HM/ PL/PR • Fundus- Not visualized due to corneal oedema • Instillation of glycerine drop clears cornea • Fundus- hyperemic disc, disc
  • 40. TREATMENT OF AN ACUTE ATTACK • Usually the IOP is >50 mm of Hg

ALGORITHM FOR TREATING ACUTE ACG • Mannitol (20%) I.V 1.5-2 gm/kg body weight ( rapid drip) or Acetazolamide I.V 500 mg • If the pupil is still blocked- press over central part of cornea with a sterile cotton bud to open the peripheral block • Oral glycerine (50%) with lemon juice TDS can be given • Pilocarpine 2% drop is started after the IOP is reduced by Mannitol ( in a very high IOP Pilocarpine can not act on sphincter muscle as it becomes ischaemic) • Beta blockers- BD • Timolol maleate 0.5% • Betoxolol 0.5 • Analgesic systemically • Steroid drops to reduce congestions • LASER treatment – once the IOP is reduced and eye becomes quite – YAG LASER peripheral iridotomy is done • Surgical treatment – if the medical and LASER treatment fails to control the IOP, then trabeculectomy is done

COMMON ANTIGLAUCOMA DRUGS IN BRIEF Topical medications: • Beta blockers: Timolol, Betaxolol, Carteolol, Metipranolol. -mech: decrease aqueous production • Prostaglandin analogues: Latanoprost, Travoprost, Bimatoprost -mech: increases uveoscleral outflow • Alpha-2 agonists: Brimonidine, Apraclonidine -mech: decreases aqueous production, increase uveoscleral outflow, neuroprotective • Topical Carbonic Anhydrase inhibitors: Dorzolamide, Brinzolamide -mech: decreases aqueous production
  • 45. SYSTEMIC DRUGS • Carbonic anhydrase inhibitors: acetazolamide -mech: decreases aqueous production • Hyperosmotic agents: glycerol, mannitol, isosorbide -mech: dehydrates the vitreous

LASER THERAPY OF GLAUCOMA • Nd:YAG laser Peripheral Iridotomy Indications: • POAG (acute ACG, intermittent ACG, chronic ACG) • POAG with narrow angles • Secondary ACG (irido-IOL block, irido-vitreal block, subluxated lens with pupil block) Prophylactic: Fellow eye of patient with PACG, Narrow occludable angles.

SURGICAL TREATMENT OF GLAUCOMA • Indications for trabeculectomy: • When IOP is raised to a level where there is evidence of progressive VF or ON which will threaten quality of visual function, despite adequate medical treatment • Uncontrolled POAG with maximal medical treatment -Failure of medial treatment (IOP not controlled with progressive VF or ON damage) -Side effects of medical treatment -Non-compliance with medical treatment
  • 48. . -Additional considerations: -Young patient with good quality of vision -One-eyed patient (other eye blind from glaucoma) -Family history of blindness from glaucoma -Glaucoma risk factors (HTN, DM) • Uncontrolled PACG after laser PI and medical treatment • Secondary OAG or ACG
