Laporan Kasus Cardiac Assessment in Non Cardiac Surgery (Sabrina Erriyanti)soroylardo1
1. Many patients undergoing major noncardiac surgery are at risk for cardiovascular events due to changes in cardiac physiology during anesthesia and surgery and the patient's underlying cardiovascular status.
2. A preoperative evaluation involves assessing the urgency of surgery, active cardiac conditions, risk of the planned procedure, and patient's functional capacity.
3. For a planned bipolar hemiarthroplasty, a 80-year-old woman with an uncontrolled type 2 diabetes and hip fracture underwent further cardiac, pulmonary, and anesthesia assessments due to her age and medical conditions which increased her risk for perioperative cardiovascular complications.
This document provides a morning report on a case of malaria. It summarizes the patient's history, including previous malaria infections, physical examination findings, laboratory test results showing Plasmodium vivax, and diagnosis of malaria tertiana. The patient's treatment plan includes IV fluids, antipyretics, anti-malarial medications, and patient education.
The document provides a morning report on a fever of unknown origin case. It summarizes:
1) A 58-year-old male presented with prolonged fever for 1 month despite previous antibiotic treatment for presumed typhoid fever. He had weight loss and decreased appetite.
2) Physical exam was normal but labs showed leukocytosis, increased CRP, and hyponatremia. Imaging found hydronephrosis and nephrolithiasis.
3) Differential diagnoses for the fever of unknown origin were discussed, including further diagnostic tests needed to establish a diagnosis. Control of diabetes and urology follow-up were also mentioned.

Kultur dan identifikasi spesies NTM penting untuk diagnosis dan penentuan pengobatan
Journal Reading Cardiac Manifestation in Dengue Infection (Nuriza)soroylardo1
Dr. Soroy Lardo discussed cardiovascular involvement in dengue virus infection. Dengue can cause cardiac manifestations like myocarditis and arrhythmias. It also impacts the vasculature by increasing permeability and causing plasma leakage. While cardiac dysfunction is often mild and self-limiting, severe cases can lead to fulminant myocarditis. The virus may directly infect cardiomyocytes in rare cases. Increased vascular permeability is a critical factor in disease severity and occurs via interactions between the virus and endothelial glycocalyx. Supportive care focuses on cautious fluid resuscitation during critical leakage periods. Further research is needed to understand disease mechanisms and identify effective treatments.
Sepsis-induced myocardial dysfunction involves complex interactions between pathogens and the host immune system. Early sepsis is characterized by circulatory abnormalities related to intravascular volume depletion and vasodilation. Despite increased cardiac output, myocardial dysfunction is significant in septic shock. Direct myocardial depression in sepsis is caused by down-regulation of beta-adrenergic receptors and the effects of cytokines like TNF-alpha and IL-1 beta. Management of myocardial dysfunction in septic shock involves prompt antibiotic therapy, early goal-directed fluid resuscitation and hemodynamic support with vasopressors and inotropes according to surviving sepsis campaign guidelines. Further research is still needed to better understand sepsis-induced myocardial dysfunction and identify more targeted treatments
Journal Reading Stem Cell dengan HIV (sharon) soroylardo1
1) Studies in murine and macaque models show that genetically modifying CD4+ T-cells to disrupt CCR5, the main HIV coreceptor, provides a selective advantage over non-modified cells and significantly reduces plasma viremia. However, a source of residual viral reservoir remains.
2) Two HIV patients who underwent allogeneic stem cell transplantation from CCR5-negative donors have remained virus-free for nearly 4 years after transplantation while kept on antiretroviral therapy (ART), suggesting genetic modification may not be necessary if ART is completely suppressive.
3) Hematopoietic stem cell transplantation remains a promising curative approach for HIV, but the timing of ART interruption

- Patient presented with a 5 day history of dizziness, muscle aching and fever. For the past 4 days, the patient experienced intermittent high fever with chills, headache and weakness.
- Based on history and laboratory findings, the patient was diagnosed with Plasmodium Vivax malaria. The patient had a history of malaria infections and exposure in endemic areas.
- The patient was also diagnosed with acute gastroenteritis. Management involved rehydration, antibiotics and monitoring electrolytes.
- Atrial fibrillation was also noted on examination but with stable hemodynamics. Management focused on risk reduction, stroke prevention and rate control therapy.
Approach acute diarrhea with comorbid diseasessoroylardo1
1. The patient, a 57-year-old female, presented with fever for one week and diarrhea for one day. She had a history of diabetes, hypertension, and coronary artery disease.
2. On examination, she had a temperature of 36°C, tenderness in the right upper quadrant, and typhoid tongue. Laboratory tests showed hypercholesterolemia.
3. She was assessed with acute gastroenteritis, likely typhoid fever given her history of travel to an endemic area. She was started on treatment and monitoring for her comorbidities. Further diagnostic tests were planned to confirm the diagnoses.
A 61-year-old female presented with abdominal pain and jaundice. Physical examination found tenderness in the right upper quadrant and jaundice. Laboratory tests showed elevated liver enzymes and bilirubin. Abdominal ultrasound found thickened gallbladder wall suggestive of cholecystitis. The working diagnosis was acute abdomen with obstructive jaundice suspected to be caused by choledocholithiasis and uncontrolled hypertension. Further examination with ERCP and US of biliary tree was recommended to evaluate for common bile duct stones. Treatment included IV fluids, antibiotics, analgesics and gastroenterology consultation.

PRESENTASI LAPORAN TUGAS AKHIR ASUHAN KEBIDANAN KOMPREHENSIFratnawulokt
