1. Students and Parents/Guardians,
District Five has determined that the District issued iPad minis need to be collected for servicing over the summer. We are asking your
help in getting the District-Issued iPad Mini turned in. The following steps will help the return process go much faster. Please contact
the Eagle ER Help Desk ahead of time if there are any problems or questions completing the steps below.
Please bring this form with you when turning in the iPad.
Schedule: iPad collection will be in Room B227 Media Presentation Room behind the Media Center.
Seniors: May 21 - May 29 7:30– 8:15AM & 3:40 – 4:15PM
Underclassmen: May 30 - June 3 7:30– 8:15AM & 3:40 – 4:15PM
June 4, 5, 6 7:30– 8:15AM & 11:45 – 12:15PM

Go to Settings → General → Reset → Erase All Content and Settings
Go through the setup process to get back to the home screen:
o Set up as a new iPad, and tap "skip this step" when asked to enter an Apple ID
o Do NOT put in your iTunes account after the reset
o Do NOT put a passcode on the device

damage occurred (this is for insurance purposes, and will not be used for any discipline issues!)
