Case Assessment Format
The case assessment takes place after the intake and assessment interviews have been conducted. The helping professional must evaluate the application for services to determine eligibility for services. This is just one process for conducting a case assessment.

I. Examine your agency’s guidelines for eligibility as well as federal or state guidelines, if applicable. What are your agency’s guidelines for eligibility?

The scenario
As a human service professional, the agency I represent only provides career assistance, none of which is monetary; all other assistance is a referral to other resources. My sincerely motivated client cannot afford clothes for interviews. Other options have been exhausted.
Kitchener's ethical decision-making model
Kitchener's model is based on five assumptions that need to be at the core of any ethical evaluation and assumes that all ethical behaviors are presumed to be based on 1) autonomy, acting on free-will 2) beneficence, the intention is of benefit to the client, 3) nonmaleficence, actions are not harmful to the client, 4) justice, respect to rights and dignity of client and agency, and 5) fidelity, to commit to client need as appropriate to agency mission (Martin, 2018).
Applying the NOHS ethical principle to my rationale
The NOHS preamble states its purpose as providing human service professionals and educators with guidelines to manage ethical dilemmas effectively broken down into two sections. The first section focusing on human service professionals standards, and the second section focusing on human service educator standards. The human service professional standards categorize responsibilities of and to the client, community, and agency. The standards hone in on respect for dignity, honoring integrity, doing no harm, and avoiding exploitation by maintaining self-awareness and awareness of historical and current global influences that impact oppression and marginalization and demonstrate ethical personal and professional aspects. A human service professional has a duty to clients to recognize client strengths, a right to confidentiality, protect client records, avoid intimate relationships with clients, and not impose personal biases and values. To the public and society, these standards extend a professional obligation to provide services without discrimination or bias, the awareness of laws and advocacy if these violate client rights, how social and political issues impact diversity, and social justice advocacy eliminate oppression (Martin, 2018).

Assessment means appraisal or evaluation of a situation, the person(s) involved, or both. As the initial stage in helping, assessment generally focuses on identifying the problem and the resources needed to resolve it. Focusing on the people who are involved includes attention to client strengths that can be a valuable resource to encourage client participation and facilitate problem solving. The benefits of the strengths-based approach to assessment were discussed in Chapter One. As the opening example shows, data are gathered and assessed at this phase to show the applicant’s problem in relation to the agency’s priorities. Identifying possible actions and services and determining who will handle the case are also part of the assessment phase. In this example, a preliminary screening follows the referral. The Malcom Baldrige National Quality Award was created by the 100th United States Congress on August 20, 1987 by Public law 100-107 (Library of Congress, n.d.). The award is named for Malcolm Baldrige, the 26th Secretary of Commerce, who served under President Reagan from January 22, 1981 until his accidental death on July 25, 1987 from a rodeo accident in California. Secretary Baldrige was well-known for his award-winning managerial accomplishments and contributions to the United States Government. During his tenure, Secretary Baldrige reduced the Department of Commerce’s administrative staff by 25% and its annual budget by 30% through process improvements and efficiencies (National Institute of Standards and Technology, 2016).
The Malcom Baldrige National Quality Award is the United States’ topmost national level award given to an organization in recognition for their performance excellence. This annual award can be presented to as many as 18 organizations across six categories: healthcare, small business, service, manufacturing, education, and nonprofit (National Institute of Standards and Technology, 2017). The Baldrige award has four functions. Assessment means appraisal or evaluation of a situation, the person(s) involved, or both. As the initial stage in helping, assessment generally focuses on identifying the problem and the resources needed to resolve it. Focusing on the people who are involved includes attention to client strengths that can be a valuable resource to encourage client participation and facilitate problem solving. The benefits of the strengths-based approach to assessment were discussed in Chapter One. As the opening example shows, data are gathered and assessed at this phase to show the applicant’s problem in relation to the agency’s priorities. Identifying possible actions and services and determining who will handle the case are also part of the assessment phase. In this example, a preliminary screening follows the referral. This chapter explores the assessment stage of the helping process: the initial contact with an applicant for assistance, the interview as a critical component in data gathering, and the case record documentation that is required during this phase. You can refer to Figure 2.1 to see the place assessment has in the helping process. The assessment phase concludes with the evaluation of the application for services. For each section of the chapter, you should be able to accomplish the following objectives.

The paper assesses my previous employer against the Malcom Baldrige National Quality Award performance excellence criteria using the seven performance excellence criteria categories as outlined in the National Institute of Standards and Technology’s Baldrige Performance Excellence Program assessment tool entitled “Are We Making Progress.”
The Malcom Baldrige National Quality Award
The Malcom Baldrige National Quality Award was created by the 100th United States Congress on August 20, 1987 by Public law 100-107 (Library of Congress, n.d.). The award is named for Malcolm Baldrige, the 26th Secretary of Commerce, who served under President Reagan from January 22, 1981 until his accidental death on July 25, 1987 from a rodeo accident in California. Secretary Baldrige was well-known for his award-winning managerial accomplishments and contributions to the United States Government. During his tenure, Secretary Baldrige reduced the Department of Commerce’s administrative staff by 25% and its annual budget by 30% through process improvements and efficiencies (National Institute of Standards and Technology, 2016).
The Malcom Baldrige National Quality Award is the United States’ topmost national level award given to an organization in recognition for their performance excellence. This annual award can be presented to as many as 18 organizations across six categories: healthcare, small business, service, manufacturing, education, and nonprofit (National Institute of Standards and Technology, 2017). The Baldrige award has four functions. about tomorrow’s weather, about whether we will wake up with a headache
tomorrow morning, and about whether someone’s estimate of the labor re-
quired to repair our car is correct. Considerable effort is directed toward
coping with uncertainty. Some farmers have costly irrigation systems in or-
der to make output less dependent on variations in rainfall. And many of
us buy insurance of various sorts to limit our exposure to some kinds of un-
certainty. Moreover, there are government programs like disaster aid and
unemployment insurance that are intended to offset some of the effects of

Operating in the northern coastal city of La Coruña (or A Coruña in the local Galician language), Spanish entrepreneur Amancio Ortega was brainstorming names for his new shop and settled on “Zorba” after the classic movie Zorba the Greek. He simply thought it was “a nice name.” Unfortunately, there was a bar with the same name a few blocks away and the bar’s owner was worried patrons would be confused. The molds for the letters for Ortega’s shop had already been cast, so they played around with what they had and came up with “Zara.”[] As it turns out, for Zara it’s technology, not the name, that has made all the difference in its rise to dominate the decidedly ungeeky fashion industry.
Today, Zara is the game-changing crown jewel in the multibrand empire of Inditex Corporation (Industrias de Diseño Textil), the world’s largest pure-play fashion retailer and a firm that’s bigger than Gap, H&M, Topshop, and anyone else in the space. The firm’s supremacy is plotted and executed from “The Cube,” the gleaming, futuristic headquarters located in La Coruña’s Arteixo industrial area. The blend of technology-enabled strategy that Zara has unleashed seems to break all of the rules in the fashion industry. The firm shuns advertising and rarely runs sales. Also, in an industry where nearly every major player outsources manufacturing to low-cost countries, Zara is highly vertically integrated, keeping huge swaths of its production process in-house. These counterintuitive moves are part of a recipe for success that’s beating the pants off the competition and has catapulted Ortega to become the world’s third richest man, ahead of Warren Buffet.

Players(playerID, playerName, team, position, birthYear)

baseball have a unique ID
● playerName is player’s name
● team is the name of the MLB team the player is currently playing on (or the last team the
player played for if they are not currently playing)

EXECUTIVE SUMMARY
Hurricane Katrina was an extraordinary act of nature that spawned a human
tragedy. It was the most destructive natural disaster in American history, laying waste to
90,000 square miles of land, an area the size of the United Kingdom. In Mississippi, the
storm surge obliterated coastal communities and left thousands destitute. New Orleans
was overwhelmed by flooding. All told, more than 1500 people died. Along the Gulf
Coast, tens of thousands suffered without basic essentials for almost a week.
But the suffering that continued in the days and weeks after the storm passed did
not happen in a vacuum; instead, it continued longer than it should have because of – and
was in some cases exacerbated by – the failure of government at all levels to plan,
prepare for and respond aggressively to the storm. These failures were not just
conspicuous; they were pervasive. Among the many factors that contributed to these
failures, the Committee found that there were four overarching ones: 1) long-term
warnings went unheeded and government officials neglected their duties to prepare for a
forewarned catastrophe; 2) government officials took insufficient actions or made poor
decisions in the days immediately before and after landfall; 3) systems on which officials
relied on to support their response efforts failed, and 4) government officials at all levels
failed to provide effective leadership. These individual failures, moreover, occurred
against a backdrop of failure, over time, to develop the capacity for a coordinated,
national response to a truly catastrophic event, whether caused by nature or man-made.
The results were tragic loss of life and human suffering on a massive scale, and an
undermining of confidence in our governments’ ability to plan, prepare for, and respond
to national catastrophes.
Effective response to mass emergencies is a critical role of every level of
government. It is a role that requires an unusual level of planning, coordination and
dispatch among governments’ diverse units. Following the terrorist attacks of 9/11, this
country went through one of the most sweeping reorganizations of federal government in
history. While driven primarily by concerns of terrorism, the reorganization was designed
to strengthen our nation’s ability to address the consequences of both natural and man-
made disasters. In its first major test, this reorganized system failed. Katrina revealed
that much remains to be done.
The Committee began this investigation of the preparations for and response to
Hurricane Katrina within two weeks of the hurricane’s landfall on the Gulf Coast. The

Demographics include the client’s name, age, gender,
relationship status, ethnicity, occupation, length of employment,
age and gender of any children.
Example: Mary T. is a 25 year old married bi-racial female who
has worked for City Savings and Loan as a teller for the last 2
years. She has been married to her husband for 4 years. They
have no children and no previous marriages.
2. Presenting problem
The presenting problem is the reason why the person is
receiving your services. The presenting problem can also be
called the client’s chief complaint.
Example: Mary presents with multiple concerns related to an
unfulfilling marriage, potential loss of employment, and
concerns over her husband’s gambling.
3. Goal
Clients come to therapists to accomplish something. I think it is
a good idea to find out what the client’s goal is. What do they
want to get from your agency?
Example: Mary would like to first focus on concerns related to
her husband’s gambling. Mary states that her husband told her
last week that he is approximately $45,000 in debt. Mary is also
worried about her job security.
4. Legal/ethical
This is a one of the most critical categories. Not addressing
your legal and ethical responsibilities can be legal problems for
you.

· You must provide the name of your supervisor.
· You must provide a written Informed Consent that usually
includes information about the limits of confidentiality, your
responsibilities in reporting abuse, your fee, your cancellation
policies, and what the client should do if an after-hours clinical

of my status as an intern and the name of my supervisor. She
was verbally informed of the limits of confidentiality,
cancellation policy, and office fees. Mary signed the
Acknowledgment of HIPAA Guidelines form.
5. Crisis/safety
Is the client in crisis? If so, describe the type and severity of
the crisis and your interventions to address this concern. You
must be exact.
Example: While her situation is difficult, Mary does not appear
to be in crisis at this time. She denied any thoughts or feelings
related to self harm. She denied any past history of self harm.
There are no reporting responsibilities present. She seems to
have an active and extended support system.
6. Diversity
What are the differences between you and the client? These
differences can be about age, gender, education, socioeconomic
status, sexual orientation, marital status, religion, stage of life,
and should be noted.
The concern is whether or not these differences pose a difficulty
for the helper to remain objective and focused on the client’s
concerns. If the differences between the client and therapist are
too great for the helper, it should be noted how the helper will
handle this problem.
Example: There are some differences between Mary and myself.
While we are of the same gender, she is married and I am not.
9. Mary is of a different ethnicity than I am and she is a few years
younger than I. There may also be some differences in our
educational background, as well. I do not feel these differences
will impede our working together. If I felt otherwise I would
discuss my concerns with Mary to see if she had any similar
concerns. Secondarily, I would seek consultation from my
supervisor or a clinician skilled in working with someone from
her culture. If the differences between us were too great and I
was not of optimal service to my client, I would provide her
with an appropriate referral.
It is a good idea to state that despite the differences between
you and the client you feel confident you can be of service. If
the differences were too great you should discuss your
limitations with your supervisor and get consultation from a
clinician of more similar backgrounds. Or you can refer the
client to a therapist with more similar demographics to the
client than you have.
7. Assessment
Your assessment is your impression of the client based on the
assessment questions you asked.
Example: Mary is devoted to her husband and family, but is
clearly struggling with their financial difficulties. She
complains about increased anxiety, decreased sleep and some
disruption in concentration. She notes a change in mood as she
is “worried all the time” and finds herself crying when she feels
hopeless. Mary does not describe herself as a person who
worries a lot. Her acute symptoms seem particular to her
husband disclosing the extent of his debts. She states her
appetite is stable and has not experienced any recent gain or
loss of weight. She is oriented to person, place and time.
Thinking is linear and her memory appears good. Affect
matches content. She states her performance at work is affected
by her personal worries, but there is no current disciplinary
action against her. She denies any suicidal or homicidal
ideations or actions in the past or present. She states a social

which includes a large extended family and strong ties to her
church community. She states her husband is generally
supportive, they are effective parents, but has been less
available due to his own worries. He has been avoiding her.
