This document discusses depression in older adults and provides guidance for primary care providers. Late life depression is often underdiagnosed but can affect up to 36% of older medical outpatients. Risk factors include female sex, social isolation, widowhood, lower socioeconomic status, and medical comorbidities. Providers should screen for depression using the PHQ-2 and consider workups for thyroid, vitamin D, B12, and folate levels. Treatment involves both psychotherapy and medication, with options including citalopram, sertraline, mirtazapine, and trazodone over longer durations. Providers are advised to consider drug interactions and avoid potentially inappropriate medications per Beers criteria.
Arthrocentesis of the temporomandibular jointAhmed Adawy
Dr. Ahmed M. Adawy, Professor Emeritus, Dep. Oral & Maxillofacial Surgery. Former Dean, Faculty of Dental Medicine, Al-Azhar University. Arthrocentesis of the temporomandibular joint refers to lavage of the upper joint space, hydraulic pressure and manipulation to release adhesions of the “anchored disc phenomenon” and improve motion. The technique of arthrocentesis is discussed together with the indications and contraindications of the procedure. Further, the presentation includes modifications of the standard technique.
1. Arthroscopic lysis and lavage is a technique used to treat painful clicking or popping in the temporomandibular joint (TMJ) by releasing adhesions and repositioning displaced discs.
2. There are several methods for performing arthroscopic lysis and lavage of the TMJ, involving the use of one or two needles or cannulas to irrigate and lavage the joint.
3. Common solutions used in lysis and lavage include saline, local anesthetics, sodium hyaluronate, and glucocorticoids which help reduce inflammation.

This document summarizes the anatomy and physiology of the nephron, specifically the collecting tubules and juxtaglomerular apparatus. The collecting tubule is the final segment of the nephron before entering the collecting duct system. It can be divided into the cortical and medullary collecting tubules. The cortical collecting tubule reabsorbs sodium, potassium, and water. The medullary collecting tubule is responsible for acidification of urine and is permeable to water and urea. The juxtaglomerular apparatus is formed by modified cells in the afferent arteriole and macula densa. It detects changes in chloride concentration and blood pressure, triggering the release of renin and regulating the
Tricuspid atresia & transposition of great arteriesVerdah Sabih
Tricuspid atresia and transposition of the great vessels are congenital heart defects that require complex anesthetic management. For tricuspid atresia, blood cannot flow from the right atrium to the right ventricle, causing cyanosis. For transposition of the great vessels, the aorta arises from the right ventricle and pulmonary artery from the left ventricle, separating systemic and pulmonary circulation. Surgical procedures like shunts, banding, or arterial switch procedures are required. Anesthesia goals include maintaining hemodynamics like cardiac output and low pulmonary vascular resistance while avoiding hypotension, fluid overload, or arrhythmias.
Mitral regurgitation occurs when blood flows backward from the left ventricle to the left atrium due to problems with the mitral valve. It can be caused by conditions like endocarditis or myocardial infarction. Hemodynamically, it decreases cardiac output and overloads the left atrium and lungs. Clinically, patients experience dyspnea, fatigue, edema and signs of pulmonary congestion. The severity is classified by the regurgitant volume and fraction. Treatment involves diuretics, vasodilators, and sometimes surgery. The anesthetic goals are to maintain forward flow by keeping heart rate and contractility normal while decreasing afterload and preload to limit regurgitant fraction. Induction and maintenance of
This document provides information on the definition, presentation, evaluation, investigation and treatment of anemia. It defines anemia as a hemoglobin level below certain thresholds based on sex. Anemia is often identified through screening tests but can sometimes present with symptoms of advanced anemia. Evaluation involves taking a medical history and performing a physical exam and blood tests. Based on test results, anemias are classified and specific treatment is given depending on the underlying cause, such as iron supplementation for iron deficiency anemia or blood transfusions for acute blood loss.
Ropivacaine is a long-acting local anesthetic similar to bupivacaine that provides both anesthesia and analgesia. It has decreased cardiotoxicity compared to bupivacaine. Ropivacaine is metabolized in the liver and excreted renally. It works by reversibly blocking sodium ion influx in nerve fibers, inhibiting impulse conduction. It can be used for epidural, spinal, infiltration, and peripheral nerve blocks for surgical anesthesia, C-sections, and postoperative or chronic pain management. Side effects include central nervous system and cardiovascular toxicity at high doses.
Mivacurium is a short-acting non-depolarizing skeletal muscle relaxant used for muscle relaxation during surgery or mechanical ventilation. It works by competitively blocking acetylcholine receptors at the neuromuscular junction. Mivacurium has a fast onset of 2-4 minutes, short duration of action of 12-18 minutes, and is eliminated quickly by plasma cholinesterase. It is dosed at 70-250 mcg/kg intravenously for induction and 100 mcg/kg every 15 minutes for maintenance.
1) The document describes the cases of 4 female patients admitted to the hospital ward.
2) One patient presented with jaundice and was being treated for biliary obstruction. Another presented with right arm pain and was diagnosed with thoracic outlet obstruction.
3) A third patient presented with a neck swelling and was diagnosed with multinodular goiter. The fourth presented with left chest pain and was found to have a left pleural effusion.
4) Investigations were conducted and treatment plans discussed for choledochojejunostomy, modified rib resection, thyroidectomy, and pleural decortication, respectively.
