For this Assignment, you will submit a case presentation of a
client you encountered during your field education experience.
Review your field education experience notes and your previous
process recordings.

Create a Case Presentation that includes the following:
· An explanation of your agency and the services offered
· A description of your client, including demographics,
presenting problem, goal, legal/ethical considerations,
assessment, and proposed treatment/social services delivery
plan (include termination plans if applicable)
· An explanation of whether interacting with your client
demonstrated social work practice skills
· Identification of potential social work skills not demonstrated

This helps in supporting the needs of the clients
The clients are also kept safe
Valuable services provided to prevent isolation and health
issues.
Sound Option offer training to their daily caregivers
The field of education was at Sound Options in Tacoma
Washington. The mission of this organization is to offer home
care and the care management services with dignity and
kindness throughout Olympia. The professionals at the
organization provide continued care to help in supporting the
needs of the clients while keep them safe. The valuable services
are provided to prevent isolation and health issues. The
organization also offer training to their daily caregivers through
teleconference and virtual care visits.

The organization offers professional geriatric care management
(PCGM).
PCGM are health and human services professionals
They specialize in care management
The services offered include nursing, social work, or
psychology.
There is license counseling
Advanced home care provide safer, cost effective, and improved
healthcare outcomes.
The organization offers professional geriatric care management
(PCGM). These are the health and human services professionals
specializing in a field related to the care management for
example nursing, gerontology, social work, or psychology.
There is license counseling that helps in giving the clients a
sense of hope, freedom from suffering, and greater control. The
organization is having a team that specializes on issues related
to aging, stress in the family, and anxiety. There is also
advanced home care where there is provision of safer, cost
effective, and improved healthcare outcomes.

The characteristics of the population served
Elderly individuals.
Clients of all ages suffering from mental illness
Clients of all ages suffering from Chronic illness
The families also benefits from the services offer by the
institution
The population being served consists of the elderly individuals.
Individuals who are psychologically affected also get the
services offered by this institution. The institution has
counseling program whereby counseling and psychotherapy is
offered. This is important in giving the population a sense of
hope and freedom from suffering. The families are also
supported in dealing with anxiety and stress. The institution has
whole family in group or individual session. The institution also
supports home-based care to the aging population

Case Study
A 79-year-old American Hispanic male patient present to the
facility with the complaint of being moody all the time.
Complaints of having lost hope in life and is always feeling
worthless.
He claims to be the cause of death to his 13-year-old grandson
who drown in the river.
He became more irritable, and it was hard to help him calm
down as he kept on blaming himself for the unfortunate events
in his family.
The communication with the patient was somehow complicated
since he could not speak English clearly since most of the time,
he explained his situation in Spanish at times
A translator was called but the patient remain quite since he felt
uncomfortable when another healthcare provider was introduced
and only trusted me.

Support groups
Cognitive behavioral therapy can be used with clients who are
depressed due to crisis or trauma. So in this case we need to get
the client talking about the trauma and the negative affects it is
having and add a strength based approach trying to get the
client to look at their strengths and build on them. Adding the
family to some of the therapy is helpful because the family is
probable been affected by the drowning also and affected by the
clients moods. This may also help the client see that they do not
blame him. The client getting involved in a support group that
deals with loss and grieving can be a huge help, he will learn
skills need to deal with loss and grief and he can see that he is
not alone.

One of the social work skill that was demonstrated during the
interaction was the communication. The role of communication
was valued since the patient was communicated to when he first
presented himself and was informed that he would be helped
with his condition. An empathy was shown to the patient and
this was demonstrated by seeking further help from the
translator to help in understanding what was being
communicated by the patient. He was helped to sit down while
he became uncontrollable. The critical thinking was
demonstrated when the aspect or the role of cultural competency
was demonstrated by calling the translator to help in
understanding the language. Active listening was demonstrated
by allowing the patient to share his story and how the problem
began. Patience was also demonstrated when the patient became
disturbance. He was given time to cool down before continuing
with the assessment.

Value and ethics:
. One of the social work skills not demonstrated in the agency
was the cultural competency. In this case, it was hard to
understand the language being spoken by the client. Whoever
took the referral should have passed it on that an interpreter
would be needed for the appointment. I had to seek help from
the fellow colleague who acted as an translator. Once I realized
that yes he spoke some English but not a lot and I did not know
how much English he actually understood. The patient became
scared and was not ready to talk further once the translator was
introduced. The value and ethics was not demonstrated since the
privacy of the patient was not observed and we were not
prepared for what the clients needs are. However once they
client calmed back down and was not so frustrated with the
situation he did allow the interpreter

important
It reveals the challenges being encountered as a social worker.
It shows the necessary social skills required to be a successful
social worker
It reveals the importance cultural competency to overcome
issues associated with language barriers.
Reveals the types of services being offered in social work.
It shows the types of clients encountered.
Reveals the strategies needed to promote engagement with the
clients.
The process of preparing and engaging in a formal case
presentation is important in social work since it reveals some of
the challenges being encountered as a social worker. It also
shows the necessary social skills required to be a successful
social worker for example cultural competency to overcome
issues associated with language barriers beliefs and so on. It
also helps in knowing the types of services being offered in
social work. It reveals about the type of clients that should be
expected in seeking the services. It also reveals about the
approaches that must be taken to promote the engagement with
the clients for example through promoting trust and building
rapport

So in this case we need to get the client talking about the trauma and the negative affects it is having and add a strength based approach trying to get the client to look at their strengths and build on them. Adding the family to some of the therapy is helpful because the family is probable been affected by the drowning also and affected by the clients moods. This may also help the client see that they do not blame him. The role of communication was valued since the patient was communicated to when he first presented himself and was informed that he would be helped with his condition. An empathy was shown to the patient and this was demonstrated by seeking further help from the translator to help in understanding what was being communicated by the patient. He was helped to sit down while he became uncontrollable. The critical thinking was demonstrated when the aspect or the role of cultural competency was demonstrated by calling the translator to help in understanding the language. Active listening was demonstrated by allowing the patient to share his story and how the problem began. Patience was also demonstrated when the patient became disturbance. He was given time to cool down before continuing with the assessment. 7 social work skills not demonstrated in the agency Cultural competency. Value and ethics: . One of the social work skills not demonstrated in the agency was the cultural competency. In this case, it was hard to understand the language being spoken by the client. Whoever took the referral should have passed it on that an interpreter
  • 12. would be needed for the appointment. I had to seek help from the fellow colleague who acted as an translator. Once I realized that yes he spoke some English but not a lot and I did not know how much English he actually understood. The patient became scared and was not ready to talk further once the translator was introduced. The value and ethics was not demonstrated since the privacy of the patient was not observed and we were not prepared for what the clients needs are.
