DECLARATION-CUM-REQUEST FOR MONTHLY REIMBURSEMENT OF FUEL
                  AND CAR MAINTENANCE EXPENSES

Dear Sir,

As per norms of IFCI, I am eligible to claim reimbursement of Fuel and Car Maintenance
expenses.

I am using Two Wheeler , bearing Registration No. HR29 N 3473, which is being maintained by
me at Faridabad, Haryana i.e. place of my posting.


In view of the above, I request you to reimburse me Fuel and Car Maintenance Expenses every
month, during the Financial Year April_2012 to March_2012, subject to the eligible limit.

The eligible amounts for the particular month may be credited to my Bank Account in the
succeeding month.

I agree that the above reimbursements are made on the basis of my declaration and I undertake to
preserve the Bills, Receipts etc., for a period of at least two years from the end of the Financial
Year and to produce the same to IFCI, if required by it, for the purpose of verification. I am also
aware that Income Tax Office concerned may demand the Receipts/Bills in connection with my
Income Tax Assessment.
