This document is a worksheet for ANG and RegAF members requesting formal training. It requires the member to provide contact information and certify that training obligations will be met. Supervisor and commander authorization is also required to ensure the training does not conflict with mission needs. The final section is for the Base Education and Training Manager to track the request status.

Members are responsible for completing the entire worksheet and getting all the required signatures prior to submitting to the NGB Training Office. Failure to
do so will result in the worksheet being returned to the member without action.

I understand that this is a REQUEST for training only. Completion of this form does not guarantee my attendance. I further understand that if selected for
training, I am obligated and required to attend as scheduled. I have coordinated this request for training with my supervisor and they have consented to my
attendance. Personal obligations will not conflict with this request for training.
By Signing below I CERTIFY that:
(1) I am within Air Force standards and will be required to meet those standards upon departure for training, should I not meet Air Force standards at that
time, I understand that I will not be allowed to depart.
(2) I understand that in accordance with ANGI 36-2101, Chapter 5, a service commitment will be incurred as a result of my attendance at the requested
course, and I must extend or reenlist to cover the required period (Enlisted), or sign a written agreement to serve the prescribed period (Officer). I further
understand that I will not be allowed to attend training unless the service agreement is completed, and that all commitments incurred as a result of training
will begin on the day after my training has been completed.
(3) I CAN NOT cancel this quota if the school start date is within 45 days UNLESS APPROVED BY THE ANGRC COMMANDER.
