Diagnosis is the second phase of the nursing process. In this
phase, nurse use critical thinking skill to interpret assessment
data to identify client problems. Nursing diagnosis are
developed based on data obtained during the nursing
assessment. A nursing diagnosis increases patient safety and
means more effective care.

1. Using the phrase ‘ Complex factor’- When there are several factors or they are too
complex to state in a brief phrase. e.g. decisional conflict and chronic low self-esteem
related to complex factor.
2. Using ‘possible to’ describe either the problem or the etiology. When the nurse believes
more data is required about the client’s response (problem) or the etiology. The
