- (Say Organization’s name) is excited to introduce its new patient portal, FollowMyHealth, that lets YOU take a proactive role in managing your care.
- Creating your free portal account and connecting to your health information is easy! Just follow these simple steps.
- First, check your email. Your doctor’s office will send you an email from noreply@followmyhealth.com. If you don’t see it right away in your Inbox, please check your spam folder. When you’ve located and opened this email, click on the registration link and follow the onscreen prompts to complete your registration.
- Next, click Create an Account. If you already have a portal account and wish to add an additional provider, click Add this Connection.
- Now you must choose a login method. If you’d like to create a unique username and password for your portal account, click the green FMH Secure Login icon. If you wish to use an existing username and password from Facebook, Gmail, Yahoo, Windows Live or CernerHealth, simply click on the respective icon. As a reminder -- all data is encrypted to ensure privacy and security. If you choose to use your Facebook credentials to log in to your portal account, your medical information will NEVER be posted or shared via Facebook. Using Facebook is simply a way for you to login with already established credentials.
- If you choose to log in to your portal account with FMH Secure Login, you will need to create a unique username and password, as well as provide your email address. When you’re finished, hit continue. You will next be prompted to sign in to FMH Secure Login by re-entering your newly established username and password. Please note: an email confirming your FMH Secure Login username will automatically be sent to your email address. Please print or save this email. You will need it if you ever forget your username.
- To finish your connection to your personal health record, follow the on-screen prompts on the next four screens. These screens include accepting our Terms of Service, entering your Invite Code (which can be found in your invitation email from noreply@followmyhealth.com), and accepting the Release of Information. Your health record will then begin to upload. Once populated, you’ll be able to view your account.
- Our portal is user friendly and intuitive. View items that need your attention, request appointments and more right from the Home screen.
- Click on My Health to view detailed information about your health. View test and lab results, read documents and notes from your physician, access your medication lists and much more.
- Avoid playing phone tag – communicate with your doctor by using the secure messaging feature within the portal. It’s quick and easy…just like email! To send a message to your doctor, simply access the Inbox and use it like any other email program. Your messages are encrypted to ensure privacy and security.
- While a portal provides many benefits, the most important reason is YOU and your health! It’s proven that patients who use patient portals are more engaged in their care and enjoy healthier lifestyles and improved outcomes.
