Present a preliminary care coordination plan developed previously to a patient, communicating professionally and sensitively. 2. Collaborate with the patient to evaluate the session outcomes, address revisions to the plan, and ensure three hours are spent and logged. 3. Build on the preliminary plan with a 5-7 page final plan citing peer-reviewed sources and addressing patient health issues, interventions, resources, ethics, policy, goals, and patient satisfaction. The plan must also be submitted to CORE ELMS.
