This document provides instructions for a preliminary care coordination plan assessment. Students are asked to:
1) Develop a 3-4 page preliminary care coordination plan for a individual in their community, identifying the patient's health concerns and available community resources.
2) Contact a individual in their community who is willing to participate, discuss goals for a care coordination session, and set a tentative date.
3) The plan must analyze the selected health concern and best practices, establish health goals agreed upon with the individual, and identify community resources, citing credible sources. It must be written clearly and concisely using proper formatting.
