Elderly and Disabled Assistance Program
• Mission: Committed to assisting in the improvement of the human
welfare and adhering to the needs of the citizens.
• Service Population: This program will serve the elderly ages 60 and
above and disabled individuals.
Factors that affect Clients
• Political: Amount of assistance, Funds and grants provided from the
government
• Economic: Resources, Living Arrangements, Functional Status
• Social: Health, Mobility, Loneliness,
Culture will form values and influence the society in how we live while
diversity will offer different perspectives. Culture and diversity will
result in outcome in productivity as well as performance due to
organizational decisions.
Human Relations Theory
Workplace relationship and ethics will be applied.
Work will be equally divided among staff.
Case worker will interview clients and input information in the system.
Supervisor will overlook the case information and advise the case
worker of any additional information needed or edited.
Manager will ensure that clients are meeting requirements for the
program
Corporate will oversee and give approval for assistance
Information System Overview
Required documents verification and other records that are needed to
provide services should be controlled and maintained by the
organization to operate.
Email, Fax, Scan, are essentials to the information system.

Program Evaluation
In evaluating the program, you will focus on the ratings , success of
services rendered, outcome of services, financial losses/gains,
satisfaction of clients.
Employee evaluation would be focus on performance etiquette,
attendance, and accountability.
Outcome measure that will be used are the results of clients whom
services were rendered to threw the program as well as government
and community satisfaction.

individuals in efforts of providing essential resources to enable them to
live a more comfortable life.
