The COVID-19 outbreak in the United States is growing steeply
and spreading widely. As of March 26, national incidence surpassed
every other country, and as of April 28 has reported over a million
cases. The COVID-19 crisis is exposing the systemic frailties in our
healthcare system. More than 78 million people in America do not
have access to adequate health insurance [1]. Given that health insur-
ance in the US is typically provided by employers, millions more are
at risk of losing their healthcare coverage as unemployment surges.
Here we discuss how the pervasive healthcare insecurity in the US
hampers control of COVID-19. Further, we argue that universal
healthcare would alleviate the cost barriers that are impeding control
of this pandemic.
Outbreak mitigation relies on prompt diagnosis and case-isolation,
in which mild cases are quarantined at home and more severe cases
are hospitalized. These measures must be implemented rapidly in
order to be effective. However, for the millions of people who are
either uninsured or underinsured, concern about the medical expenses
that could be incurred delays diagnosis and treatment. While the Fam-
ilies First Coronavirus Response Act recently approved by Congress
stipulates that COVID-19 diagnostic testing is nominally free for every-
one, treatment is not covered. Those who are hospitalized may face
major medical expenses. For instance, the cost of 12 days in the ICU on
ventilation would likely exceed US $80,000 [2,3], even without consid-
ering the additional hospital care before and after ICU admission. In
addition to the burden on the uninsured, the under-insured are obli-
gated to pay substantial out-of-pocket sums, including thousands of
dollars in deductibles and copays. Although the Coronavirus Aid, Relief,
and Economic Security Act has invested $100 billion into the Public

Four Steps to Building the Public Health System Needed
to Cope With the Next Pandemic

COVID-19 has revealed what many publichealth practitioners have known for sometime: our nation’s disjointed and under-
funded public health system lacks the ability to
mount a coordinated response against novel epidemic
threats. Our failure to invest in our nation’s state
and local governmental public health infrastructure,
as well as key federal programs, risks a large loss of
life from future infectious disease threats, and dur-
ing crises, jeopardizes the economy and the normal
functioning of society.
Since 2010, spending for state public health agen-
cies has dropped by 16% per capita and spending for
local health agencies has fallen by 18%.1 More than
three-fourths of Americans live in states that spend
less than $100 per person annually on public health,
compared with more than 100 times that for medical
care.1 Federal support for the Public Health Emer-
gency Preparedness Program (PHEP) and the Hospital
Preparedness Program (HPP), which are meant to
provide funding for health agencies and hospitals, re-
spectively, to prepare for a response, has declined by
30% since 2006.2 The consequences of these budget
reductions include fewer staff ready to respond at a
moment’s notice, fewer dollars available for invest-
ments in technology, and in some places, a decrease
in overall coordination between health care providers
and public health agencies.
The nation’s early response to COVID-19 showed
the country’s limited capabilities in implementing ba-
sic epidemic control steps—testing, isolating, tracing,
and quarantine. In this supplement, Hamilton and
Turner3 and Ruebush et al4 provide critical and de-
tailed reviews of the nation’s epidemiologic response

A global health issue that is affecting not only the US but the health community globally is the COVID-19 pandemic. Most individuals infected with the COVID-19 virus experience mild to moderate respiratory symptoms and recover without requiring special treatment. Older adults, and those with underlying medical problems like cardiovascular disease, diabetes, cancer, and chronic respiratory disease are more likely to have serious symptoms that require hospitalization. The primary mode of transmission is through droplets of saliva and discharge from the nose when an infected person coughs or sneezes (World Health Organization, n.d.). Hospitals and healthcare systems globally are facing financial challenges due to the pandemic. Shortage of supplies include personal protective equipment, hospital equipment, and sanitization supplies. The major contributing factor for the strain felt on health systems internationally were due to a lack of preparedness to handle a large pandemic. Many hospitals asked their community for donations of PPE. Many health care professionals were forced to reuse elements of PPE (Kaye et al., 2020). Health care delivery systems are working collectively to address the current pandemic, and be more prepared for a large-scale pandemic in the future. Stakeholders include organizations such as WHO and the CDC, the countries affected by the pandemic, leaders in health care, and political leaders.

Daniel Defoe made an observation that could apply to
many public health interventions then and today, “This
shutting up of houses was at first counted a very cruel and
unchristian method… but it was a public good that justi-
fied a private mischief” (1). Then, just as today, a complex
relationship existed between the science of public health
and the practice of public health and politics. We address
the relationship between science, public health, and poli-
tics, with a particular emphasis on infectious diseases.
Science, public health, and politics are not only com-
patible, but all three are necessary to improve the public’s
health. The progress of each area of public health is relat-
ed to the strength of the other areas. The effect of politics
in public health becomes dangerous when policy is dictat-
ed by ideology. Policy is also threatened when it is solely
determined by science, devoid of considerations of social
condition, culture, economics, and public will.
When using the word “politics,” we refer not simply to
partisan politics but to the broader set of policies and sys-
tems. Although ideology is used in many different ways, in
this case, it refers to individual systems of belief that may
color a person’s attitudes and actions and that are not nec-
essarily based on scientific evidence (2).
Public Health Achievements
Science influences public health decisions and conclu-
sions, and politics delivers its programs and messages.
This pattern is obvious in many of public health’s greatest
triumphs of the 20th century, 10 of which were chronicled
in 1999 by the Centers for Disease Control and Prevention
(CDC) as great public health achievements, and several of
which are presented below as examples of policy affecting
successes (3). These achievements remind us of what can
be accomplished when innovation, persistence, and luck
converge, along with political will and public policy.
Vaccination
Childhood vaccinations have largely eliminated once-
common, terrible diseases, such as polio, diphtheria,
measles, mumps, and pertussis (4). Polio is being eradicat-
ed worldwide. The current collaboration between the
World Health Organization, the United Nations Children’s
Fund, CDC, and Rotary International is a political as well
as biological “tour de force,” and eradication of polio in
Nigeria has been threatened by local political struggles and

A global health issue that is affecting not only the US but the health community globally is the COVID-19 pandemic. Most individuals infected with the COVID-19 virus experience mild to moderate respiratory symptoms and recover without requiring special treatment. Older adults, and those with underlying medical problems like cardiovascular disease, diabetes, cancer, and chronic respiratory disease are more likely to have serious symptoms that require hospitalization. The primary mode of transmission is through droplets of saliva and discharge from the nose when an infected person coughs or sneezes (World Health Organization, n.d.). Hospitals and healthcare systems globally are facing financial challenges due to the pandemic. Shortage of supplies include personal protective equipment, hospital equipment, and sanitization supplies. The major contributing factor for the strain felt on health systems internationally were due to a lack of preparedness to handle a large pandemic. Many hospitals asked their community for donations of PPE. Many health care professionals were forced to reuse elements of PPE (Kaye et al., 2020). Health care delivery systems are working collectively to address the current pandemic, and be more prepared for a large-scale pandemic in the future. Stakeholders include organizations such as WHO and the CDC, the countries affected by the pandemic, leaders in health care, and political leaders.
