Introduction and Definitions
The term “Electronic Health Records” (EHR) means
something different to each of the stakeholders in
health care, but it always seems to carry a degree of
emotional baggage. Increasingly, EHRs are advert-
ized as a nearly unmitigated good that will transform
medical care, improve safety and efficiency, allow
better patient engagement, and open the door to an
era of cheap, effective, timely, and patient-centered
care.1 Indeed, for some EHR proponents the ben-
efits of adopting them are so obvious that adoption
has become an end in itself.2 But for others — and
especially for a number of skeptical practitioners and
patients — EHR is a code word that portends the cor-
porate transformation of health care delivery, the loss
of patient privacy, the demand that patients bear more
responsibility in health care, and the unreflective take-
over of the health care system by people who do not
understand medical care or how health care relation-
ships unfold.3
For our purposes, we will consider EHRs impar-
tially, as a set of tools that can be used for a variety of
purposes. We define EHRs broadly as any electronic
means of storing and transferring health-related
information. We exclude from this definition the use
of the telephone and fax, arguably precursors to the
electronic means of data exchange now available. Like
face-to-face and paper-based interactions, the tele-
phone and fax are generally limited to two people.
Breaches of phone line security, while possible and
perhaps even frequent, are unlikely to affect thou-
sands of people at once.
In this paper, we examine the development of a new
set of EHR tools, Personal Health Records (PHRs).
PHRs may be variously defined (Table I) and have sev-
eral potential functional and payment models (Table
II), but the general aim of all PHRs is to increase
patients’ access to and sense of ownership over their
health care information. According to the Markle
Foundation, the advent of PHRs “represents a transi-
tion from a patient record that is physician-centered
to one that is patient-centered, prospective, interac-

Developing a Health Care Perspective
Access to healthcare services is very essential for sustainable level of living and good health. Several scholars have described access as “the timely use of personal health services to achieve the best possible health outcomes”. One of the issues facing many countries across the world including those with systems for universal healthcare is providing appropriate and timely healthcare access for deprived patients. Currently there is limited information on how those patients living in a context of social and material deprivation perceive obstructions in the system of healthcare. In this paper we shall discuss several resources addressing the issue of access to services in the healthcare system.
According to Andersen, Davidson, & Baumeister (2014), in their article titled “improving the access to care”, access refers to the actual utility of individual services for heath as well as everything else that can facilitate or impede their use. In this article they present research and policy issues as well as basic trends which are related to evaluating and monitoring the access to healthcare services. They show how evaluating and monitoring offers the platform for the prediction of health services, promotion of social justice and the improvement of efficiency and effectiveness for the delivery of health services. They analyzed access and healthcare outcomes using a behavioral model which provides a systematic framework of individual and contextual framework
They expanded the behavioral model by emphasizing on two new aspects. They include the life quality as an input and healthcare outcome and genetics as a factor for predisposing. They also examined some examples of access indicators which include efficiency and effectiveness measures, utilization, potential access and healthcare needs. Changes that occurred in these indicators over time were tracked using trend data. Finally they did observations on access and the present status as well as new areas of improving access via ACA which has played a big role in improving access to health care.
Next we are going to analyze the article written by Acharya et al., (2017) titled “Partnerships in mental healthcare service delivery in low-resource settings: developing an innovative network in rural Nepal”. This article provides an understanding on the contribution of mental illness on the worldwide burden diseases which are non communicable. However, the authors note that there has been an extremely limited access to ethnically sensitive, appropriately contextual and high quality service for mental healthcare. Despite the availability for interventions to improve outcomes for the patients, this situation still persists. The authors suggest that there is need for the development of partnerships network for adaptation.

A hundred years ago, the most common means of transportation was the horse and carriage. As the first automobiles began to appear, people referred to them as “horseless carriages.” The people of that era conceptualized this new invention in terms of the existing technology. Even the inventors of the technology were not immune to this viewpoint. Isn’t the engine in the front of a car today because that’s where the horse was yesteryear?
As the automobile began to appear across the country, many people did not rush to adopt it or understand the full potential of the change that society was about to undergo. The new vehicles seemed to some to be fancy toys, inferior to the horse and carriage in many ways.
♦ The supply chain of the period was built to feed and water the horse, not fuel the car.
♦ The condition of the roads, which were passable by horse, often caused cars to become stuck.
Viewed within the existing infrastructure of their time, the critics were right; driving an automobile instead of a horse seemed like a lot of work for very little gain.
People of our era use the term electronic medical records because they are thinking in terms of paper medical records. However, the opinion in a report by the IOM is that “Merely automating the form, content, and procedures of current patient records will perpetuate their deficiencies and will be insufficient to meet emerging user needs.”2
Using the horseless carriage analogy, you can see that healthcare workflow has been designed around the infrastructure of a paper chart. Adapting the electronic chart to fit the old technology provides a level of comfort during the transition to the new system, but it also prevents us from seeing the full potential of the EHR. Similar to driving early automobiles on inadequate roads, implementing an EHR without considering the landscape can make it seem like a lot of work for very little gain.
In chapter 1, doctors Bachman and Wenner stated that adopting an EHR changes the way clinician’s work. Implementation of an EHR enables the patient records to be used in ways that paper medical records cannot. To achieve these benefits clinicians must make it part of their workflow that is as natural as driving their car to the office.
Chapter 1 also listed the Internet as one of the social forces driving EHR adoption. If you are a college student today, the Internet as we know it did not exist when you were born. Or, have you watched a detective show in which the witnesses all had differing accounts? The same can hold true for conducting an evaluation of a health information technology project. difficult to reach until the session ended late in the afternoon, because the workshop facilitators
regard cell phone calls and text messages as an unnecessary distraction.
Going back into his office, Steve emailed a message to Lucy asking her to call him as
soon as possible. Then, he resumed his search for the memo about the meeting with his boss
and the other production managers. He finally found it in his large collection of unprocessed

A hundred years ago, the most common means of transportation was the horse and carriage. As the first automobiles began to appear, people referred to them as “horseless carriages.” The people of that era conceptualized this new invention in terms of the existing technology. Even the inventors of the technology were not immune to this viewpoint. Isn’t the engine in the front of a car today because that’s where the horse was yesteryear?
As the automobile began to appear across the country, many people did not rush to adopt it or understand the full potential of the change that society was about to undergo. The new vehicles seemed to some to be fancy toys, inferior to the horse and carriage in many ways.
♦ The supply chain of the period was built to feed and water the horse, not fuel the car.
♦ The condition of the roads, which were passable by horse, often caused cars to become stuck.
Viewed within the existing infrastructure of their time, the critics were right; driving an automobile instead of a horse seemed like a lot of work for very little gain.
People of our era use the term electronic medical records because they are thinking in terms of paper medical records. However, the opinion in a report by the IOM is that “Merely automating the form, content, and procedures of current patient records will perpetuate their deficiencies and will be insufficient to meet emerging user needs.”2
Using the horseless carriage analogy, you can see that healthcare workflow has been designed around the infrastructure of a paper chart. Adapting the electronic chart to fit the old technology provides a level of comfort during the transition to the new system, but it also prevents us from seeing the full potential of the EHR. Similar to driving early automobiles on inadequate roads, implementing an EHR without considering the landscape can make it seem like a lot of work for very little gain.
In chapter 1, doctors Bachman and Wenner stated that adopting an EHR changes the way clinician’s work. Implementation of an EHR enables the patient records to be used in ways that paper medical records cannot. To achieve these benefits clinicians must make it part of their workflow that is as natural as driving their car to the office.
Chapter 1 also listed the Internet as one of the social forces driving EHR adoption. If you are a college student today, the Internet as we know it did not exist when you were born. It is our newest technology. While we continue to evolve what we can do with it, the Internet changes the way we work.

Topic: Explain the data interchange standards required to enable the flow of the
information.
As part of the Stage 2 assignment, you will identify Data Interchange Standards the
Midtown Family Clinic EHR system will use to exchange information with external
organizations. For this discussion, we will explore several different Data
Interchange Standards, or "Interoperability Standards" as the ONC defines them.

The Interoperability Standards Advisory (ISA) is meant to serve at least the following
purposes:
1. To provide the industry with a single, public list of the standards and
implementation specifications that can best be used to address specific
clinical health information interoperability needs. Currently, the ISA is focused
on interoperability for sharing information between entities and not on intra-
organizational uses.
2. To reflect the results of ongoing dialogue, debate, and consensus among
industry stakeholders when more than one standard or implementation
specification could be used to address a specific interoperability need,
discussion will take place through the ISA public comments process. The web-
version of the ISA will improve upon existing processes, making comments
more transparent, and allowing for threaded discussions to promote further
dialogue.

Introduction
SAFETY AND HEALTH POLICY FOR XYZ COMPANY
XYZ COMPANY
places a high value on the safety of its employees. XYZ COMPANY is committed to providing a safe workplace for all employees and has developed this program for injury prevention to involve management, supervisors, and employees in identifying and eliminating hazards that may develop during our work process.
It is the basic safety policy of this company that no task is so important that an employee must violate a safety rule or take a risk of injury or illness in order to get the job done. The time during which employees are participating in training and education activities shall be considered as hours worked for purposes of wages, benefits, and other terms and conditions of employment.
The training and education shall be provided at no cost to the employees.
Members of the Safety/Health Committee will be allowed reasonable time to exercise the rights of the committee without any loss of pay or benefits.
Employees are required to comply with all company safety rules and are encouraged to actively participate in identifying ways to make our company a safer place to work.
Supervisors are responsible for the safety of their employees and as a part of their daily duties must check the workplace for unsafe conditions, watch employees for unsafe actions and take prompt action to eliminate any hazards.

This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.

Though many patients are already utilizing personal health records (PHRs) to manage and track their own health, some believe that an integrated system would provide a better, more comprehensive picture of a patient’s health history. As a result, many EHR platforms are now equipped with a PHR tool. This PHR tool allows patients to enter health information as they would in a stand-alone PHR system. In addition, web- based portals within the EHR allow patients to access information entered by their physicians and health care providers. Like many emerging trends and technologies, there is much discussion about the potential benefits and challenges of this type of integrated system. While many health care professionals are excited about the empowerment provided to patients, others express significant concerns about access, security, ethics, and other implications. In this Discussion, you explore how integrating PHRs into EHR platforms could impact you and your patients.
