Discussion # 1 Advanced Primary Care of Family Shared decision-making leads to better health knowledge among the elderly, children, and veterans. They leave the decision-making to caregivers and practitioners. For children and the elderly, they do not participate in their examination, diagnosis, and treatment options because of their limited literacy. The same impacts how this population responds to the management of chronic illnesses and other aspects of care (Seo et al., 2016). Shared-decision making provides the elderly with an opportunity to ask questions when needed, seek more information about their condition, and communicate their health needs. Through this process, older adults gain an understanding of their health and become experts in the decision-making process. Today, best practices of models of care such as patient-centered care enhance communication between practitioners and patients. For instance, allowing a patient to be at the forefront of the medical process is an incentive for self-management (Narva, Norton, & Boulware, 2016). The elderly are particularly vulnerable to illiteracy and will tend to research more when they are perceived as decision-makers. They will be keen on the medical process and gain knowledge as a result. Additionally, motivational interviewing ensures that children and older adults are influential in the decision-making process. It breaks the barrier of uncertainty and facilitates curiosity among the elderly, who then learn to decode instructions, and risk asking questions about various aspects of their care (LeDoux & Mann, 2019). When assisting others to achieve good and self-motivated changes, social workers draw on their relationship-based abilities and emphasize personalization and rehabilitation. A key issue is, "What components and obstacles of an assessment are there in order to reach these outcomes?" When it comes to health promotion and public involvement, social workers are well-suited for this role since primary care is all about these things. Social workers who deal with mental health have unique challenges in assessment, which necessitates them learning and using psychiatric principles. Identifying the need for mental health care requires an understanding of assessment principles.
For this research, the focus is on urban youth's increase in mental health cases which is more influenced by lack of access to mental health care as a result of poverty that affects many children and families in the US. Young people are the most impacted by poverty, accounting for 33% of the total number of individuals in poverty. Children who live in "high risk neighborhoods" are more likely to lack access to the mental health supports they need to manage their symptoms. Stabilization, individual treatment, and symptom management are all important aspects of aftercare for children who have been hospitalized for behavioral difficulties by the time they are six years old (Hodgkinson, 2017).

Explain the role of health education in health promotion. How is the nursing process used in developing health education? Describe a contemporary issue, local or global, that a family may experience today. What steps would the nurse take to address these as part of a health education plan?
The role of health education in health promotion is vital. Health promotion by definition is “educating people about healthy lifestyles, reduction of risk, developmental needs, activities of daily living (ADL’s) and preventive self-care.” (Whitney et al., 2018). In order to live a healthy lifestyle an individual must know what a “healthy lifestyle” consists of. The same goes for reducing risks, ADL’s, preventative self-care and developmental needs. It is important for the healthcare professional or in our case, the nurse, to educate an individual continuously for the duration of the patient's care with the nurse. They should be educated on what the medications are that they are being given, as well as why it is being administered, dosage, route etc. For example, patients always want to know why they are getting Lovenox or Protonix when they have not taken it before. They should also be educated on diet choices, at the hospital that I work at, sometimes the patients will be placed on certain diets like nothing by mouth (NPO), American Diabetes Association (ADA), renal, heart healthy, low potassium etc., and they state that they do not have diet restrictions at home, so they have to be educated on why they were placed on the diet and they should consider this type of diet once discharged.
The nursing process is used in developing health education because the nurse must assess the patient. This assessment is not only a head to toe assessment, but the nurse must also determine the patients needs, readiness to learn and the patient's ability and desire to actively engage in their plan of care and meeting goals. Then the nurse makes their nursing diagnoses, for example is their non-compliance evident, immobility, learning deficit etc. The nurse then develops a plan of care and action that can be made with the patient and begins to implement the plan of care. The nurse and patient evaluate how well they are meeting their care goals and continue to develop a plan that changes to the patients needs and better assists with meeting goals. Throughout the whole process the nurse is educating the patient on what the problem is preventing the patient from getting better and then the nurse educates the patient on how they can work towards solving that problem.

There has been increased concern among policy makers, scientists and communities that health is greatly affected by a number of factors that occur in a person’s lifetime and in multi levels. Prevention is sententious to curb occurrence of any disease within the population, and it has to come first even if access to quality healthcare services is provided. To adequately promote health and prevent diseases, certain policies and factors need to be addressed mostly factors that are related to health behaviors.
Social psychology is all about understanding individuals’ behavior specifically in a social setting. Basically, social psychology focuses on factors that influence people to behave in certain ways in presence of others. The two greatest contributors in the field of social psychology were Allport (1920) and Bandura (1963). To begin with, according to Allport; he argued that the interaction of individuals with others or the presence of social groups can encourage the development of certain behaviors (Kassin, 2014). This is what Allport referred to as social facilitation, in his research he identified that an audience will facilitate the performance of an actor in a well learnt and understood task; however the performance of the same actor will decrease in performance on difficult tasks which are newly learnt, and this is contributed by social inhibition. The second contributor in the field of social psychology is Bandura (1963), in his work he developed a notion that behavior in the social world could be possibly modeled, and this is what he referred to as social learning theory. He gave his explanation with three groups of children who were watching a video where in the video an adult showed aggressiveness towards a “bobo doll” and the adults who displayed such behavior were awarded by another adult or were just punished. Therefore Bandura found that children who saw the adult being rewarded were found to be more likely to imitate that adult’s behavior.
Certain theories plays important roles in health assessment, and a theory is defined as a collection of concepts in specific area of concern or interest in the world that need explanations, intervening and prediction. Theories need to be backed up with evidence that tend to explain why things will happen in relation to current situations, and followed with some actions to turn situations in certain desirable ways. Culture is defined as customary beliefs, social forms, and material traits of a racial, religious, or social group (Webster, 2019). Competence suggests having the capacity to function effectively as an individual and an organization within the context of cultural beliefs, behaviors, and needs presented by consumers and their communities (CDC, 2015). In healthcare, it is very important to be aware of different culture backgrounds. It helps with not only being able to communicate effectively but also knowing what diseases, sickness, etc. that the person is at greatest risk for. For example, in the treatment of depression, compared with white Americans, black and Latino patients are actually less likely to receive treatment (Ball et al., 2019).
The patient I was given is a 14 year old biracial male living with his grandmother in a high-density public housing complex. For the purpose of obtain a health history with this particular patient it is important to consider everything about this patient. The patients age, sex, ethnicity, living conditions, etc. will all need to be taken into account. This particular age group are reluctant to talk and have a definite need for confidentiality (Ball et al., 2019). It is important that adolescent patients be given the opportunity to speak to you privately about concerns or issues that they may have (Ball et al., 2019). It is meaningful that you let the patient know the limits of confidentiality and that if any information provided suggests that an adolescence safety or others safety may be at risk, that its grounds to “break” confidentiality (Ball et al., 2019). Prior to the office visit, there a previsit questionnaires and screeners that the patient can fill out and this sometimes helps allow the patient to write down concerns or have a choice of concerns (Ball et al., 2019). Then based off the answers, it can help you ask appropriate questions during the interviewing process.

Abstract
Purpose: The purpose of this study was to explore parents’ perceptions of
their decisional needs when considering genome-wide sequencing (GWS) for
their child. This is a partial report and focuses on how parents prefer to receive
education and information to support their decision making about GWS.
Design: This study adopted an interpretive description qualitative method-
ological approach and used the concept of shared decision making and the
Ottawa Decision Support Framework.
Methods: Participants were parents who had already consented to GWS, and
had children with undiagnosed conditions that were suspected to be genetic in
origin. Fifteen parents participated in a focus group or individual interview.
Transcriptions were analyzed concurrently with data collection, iteratively,
and constantly compared to one another. Repeat interviews were conducted
with five of the parents to confirm, challenge, or expand on the developing
concepts.
Findings: Participants felt that their decision to proceed with GWS for their
child was an easy one. However, they expressed some unresolved decisional
needs, including a lack of knowledge about certain topics that became rele-
vant and important to them later and a need for more support and resources.
Participants also had ongoing informational and psychosocial needs after the
single clinical encounter where their decision making occurred.
Conclusions: Participants expressed unmet decisional needs, which may have
influenced the quality of their decisions. The strategies that participants sug-
gested may help create parent-tailored education, counseling, decision sup-

Low health literacy is a widespread problem in the United States, affecting over 90 million adults. Research has linked low health literacy with poorer health outcomes, higher rates of hospitalization, and less use of preventative healthcare services. Improving health literacy requires interventions at both the patient and healthcare system levels, including improving patient education materials to make them easier to understand and ensuring patients comprehend medical instructions. Efforts should be sensitive to cultural and language differences and involve the communities health literacy programs aim to serve.

Abstract
Background: Although research about medically unexplained symptoms (MUS) is extensive, problems still affect a
large group of primary care patients. Most research seems to address the topic from a problem-oriented, medical
perspective, and there is a lack of research addressing the topic from a perspective viewing the patient as a capable person
with potential and resources to manage daily life. The aim of the present study is to describe and interpret the experiences of
learning to live with MUS as narrated by patients in primary health-care settings.
Methods: A phenomenological�hermeneutic method was used. Narrative interviews were performed with ten patients
suffering from MUS aged 24�61 years. Data were analysed in three steps: naive reading, structural analysis, and
comprehensive understanding.
Findings: The findings revealed a learning process that is presented in two themes. The first, feeling that the symptoms
overwhelm life, involved becoming restricted and dependent in daily life and losing the sense of self. The second, gaining
insights and moving on, was based on subthemes describing the patients’ search for explanations, learning to take care of
oneself, as well as learning to accept and becoming mindful. The findings were reflected against Antonovsky’s theory of sense
of coherence and Kelly’s personal construct theory. Possibilities and obstacles, on an individual as well as a structural level,
for promoting patients’ capacity and learning were illuminated.
Conclusions: Patients suffering from MUS constantly engage in a reflective process involving reasoning about and
interpretation of their symptoms. Their efforts to describe their symptoms to healthcare professionals are part of this
reflection and search for meaning. The role of healthcare professionals in the interpretative process should be acknowledged
as a conventional and necessary care activity.

Explain the role of health education in health promotion. How is the nursing process used in developing health education? Describe a contemporary issue, local or global, that a family may experience today. What steps would the nurse take to address these as part of a health education plan?
The role of health education in health promotion is vital. Health promotion by definition is “educating people about healthy lifestyles, reduction of risk, developmental needs, activities of daily living (ADL’s) and preventive self-care.” (Whitney et al., 2018). In order to live a healthy lifestyle an individual must know what a “healthy lifestyle” consists of. The same goes for reducing risks, ADL’s, preventative self-care and developmental needs. It is important for the healthcare professional or in our case, the nurse, to educate an individual continuously for the duration of the patient's care with the nurse. They should be educated on what the medications are that they are being given, as well as why it is being administered, dosage, route etc. For example, patients always want to know why they are getting Lovenox or Protonix when they have not taken it before. They should also be educated on diet choices, at the hospital that I work at, sometimes the patients will be placed on certain diets like nothing by mouth (NPO), American Diabetes Association (ADA), renal, heart healthy, low potassium etc., and they state that they do not have diet restrictions at home, so they have to be educated on why they were placed on the diet and they should consider this type of diet once discharged.
The nursing process is used in developing health education because the nurse must assess the patient. This assessment is not only a head to toe assessment, but the nurse must also determine the patients needs, readiness to learn and the patient's ability and desire to actively engage in their plan of care and meeting goals. Then the nurse makes their nursing diagnoses, for example is their non-compliance evident, immobility, learning deficit etc. The nurse then develops a plan of care and action that can be made with the patient and begins to implement the plan of care. The nurse and patient evaluate how well they are meeting their care goals and continue to develop a plan that changes to the patients needs and better assists with meeting goals. Throughout the whole process the nurse is educating the patient on what the problem is preventing the patient from getting better and then the nurse educates the patient on how they can work towards solving that problem.

There has been increased concern among policy makers, scientists and communities that health is greatly affected by a number of factors that occur in a person’s lifetime and in multi levels. Prevention is sententious to curb occurrence of any disease within the population, and it has to come first even if access to quality healthcare services is provided. To adequately promote health and prevent diseases, certain policies and factors need to be addressed mostly factors that are related to health behaviors.
Social psychology is all about understanding individuals’ behavior specifically in a social setting. Basically, social psychology focuses on factors that influence people to behave in certain ways in presence of others. The two greatest contributors in the field of social psychology were Allport (1920) and Bandura (1963). To begin with, according to Allport; he argued that the interaction of individuals with others or the presence of social groups can encourage the development of certain behaviors (Kassin, 2014). This is what Allport referred to as social facilitation, in his research he identified that an audience will facilitate the performance of an actor in a well learnt and understood task; however the performance of the same actor will decrease in performance on difficult tasks which are newly learnt, and this is contributed by social inhibition. The second contributor in the field of social psychology is Bandura (1963), in his work he developed a notion that behavior in the social world could be possibly modeled, and this is what he referred to as social learning theory. He gave his explanation with three groups of children who were watching a video where in the video an adult showed aggressiveness towards a “bobo doll” and the adults who displayed such behavior were awarded by another adult or were just punished. Therefore Bandura found that children who saw the adult being rewarded were found to be more likely to imitate that adult’s behavior.
Certain theories plays important roles in health assessment, and a theory is defined as a collection of concepts in specific area of concern or interest in the world that need explanations, intervening and prediction. Theories need to be backed up with evidence that tend to explain why things will happen in relation to current situations, and followed with some actions to turn situations in certain desirable ways. Culture is defined as customary beliefs, social forms, and material traits of a racial, religious, or social group (Webster, 2019). Competence suggests having the capacity to function effectively as an individual and an organization within the context of cultural beliefs, behaviors, and needs presented by consumers and their communities (CDC, 2015). In healthcare, it is very important to be aware of different culture backgrounds. It helps with not only being able to communicate effectively but also knowing what diseases, sickness, etc. that the person is at greatest risk for. For example, in the treatment of depression, compared with white Americans, black and Latino patients are actually less likely to receive treatment (Ball et al., 2019).
The patient I was given is a 14 year old biracial male living with his grandmother in a high-density public housing complex. For the purpose of obtain a health history with this particular patient it is important to consider everything about this patient. The patients age, sex, ethnicity, living conditions, etc. will all need to be taken into account. This particular age group are reluctant to talk and have a definite need for confidentiality (Ball et al., 2019). It is important that adolescent patients be given the opportunity to speak to you privately about concerns or issues that they may have (Ball et al., 2019). It is meaningful that you let the patient know the limits of confidentiality and that if any information provided suggests that an adolescence safety or others safety may be at risk, that its grounds to “break” confidentiality (Ball et al., 2019). Prior to the office visit, there a previsit questionnaires and screeners that the patient can fill out and this sometimes helps allow the patient to write down concerns or have a choice of concerns (Ball et al., 2019). Then based off the answers, it can help you ask appropriate questions during the interviewing process.

their child. This is a partial report and focuses on how parents prefer to receive
education and information to support their decision making about GWS.
Design: This study adopted an interpretive description qualitative method-
ological approach and used the concept of shared decision making and the
Ottawa Decision Support Framework.
Methods: Participants were parents who had already consented to GWS, and
had children with undiagnosed conditions that were suspected to be genetic in
origin. Fifteen parents participated in a focus group or individual interview.
Transcriptions were analyzed concurrently with data collection, iteratively,
and constantly compared to one another. Repeat interviews were conducted
with five of the parents to confirm, challenge, or expand on the developing
concepts.
Findings: Participants felt that their decision to proceed with GWS for their
child was an easy one. However, they expressed some unresolved decisional
needs, including a lack of knowledge about certain topics that became rele-
vant and important to them later and a need for more support and resources.
Participants also had ongoing informational and psychosocial needs after the
single clinical encounter where their decision making occurred.
Conclusions: Participants expressed unmet decisional needs, which may have
influenced the quality of their decisions. The strategies that participants sug-
gested may help create parent-tailored education, counseling, decision sup-
port, ...

Abstract
Background: Although research about medically unexplained symptoms (MUS) is extensive, problems still affect a
large group of primary care patients. Most research seems to address the topic from a problem-oriented, medical
perspective, and there is a lack of research addressing the topic from a perspective viewing the patient as a capable person
with potential and resources to manage daily life. The aim of the present study is to describe and interpret the experiences of
learning to live with MUS as narrated by patients in primary health-care settings.
Methods: A phenomenological�hermeneutic method was used. Narrative interviews were performed with ten patients
suffering from MUS aged 24�61 years. Data were analysed in three steps: naive reading, structural analysis, and
comprehensive understanding.
Findings: The findings revealed a learning process that is presented in two themes. The first, feeling that the symptoms
overwhelm life, involved becoming restricted and dependent in daily life and losing the sense of self. The second, gaining
insights and moving on, was based on subthemes describing the patients’ search for explanations, learning to take care of
oneself, as well as learning to accept and becoming mindful. The findings were reflected against Antonovsky’s theory of sense
of coherence and Kelly’s personal construct theory. Possibilities and obstacles, on an individual as well as a structural level,
for promoting patients’ capacity and learning were illuminated.
Conclusions: Patients suffering from MUS constantly engage in a reflective process involving reasoning about and
interpretation of their symptoms. Their efforts to describe their symptoms to healthcare professionals are part of this
reflection and search for meaning. The role of healthcare professionals in the interpretative process should be acknowledged
as a conventional and necessary care activity.

- 1. Discussion # 1 Advanced Primary Care of Family Shared decision-making leads to better health knowledge among the elderly, children, and veterans. They leave the decision-making to caregivers and practitioners. For children and the elderly, they do not participate in their examination, diagnosis, and treatment options because of their limited literacy. The same impacts how this population responds to the management of chronic illnesses and other aspects of care (Seo et al., 2016). Shared-decision making provides the elderly with an opportunity to ask questions when needed, seek more information about their condition, and communicate their health needs. Through this process, older adults gain an understanding of their health and become experts in the decision-making process. Today, best practices of models of care such as patient- centered care enhance communication between practitioners and patients. For instance, allowing a patient to be at the forefront of the medical process is an incentive for self-management (Narva, Norton, & Boulware, 2016). The elderly are particularly vulnerable to illiteracy and will tend to research more when they are perceived as decision-makers. They will be keen on the medical process and gain knowledge as a result. Additionally, motivational interviewing ensures that children and older adults are influential in the decision-making process. It breaks the barrier of uncertainty and facilitates curiosity among the elderly, who then learn to decode instructions, and risk asking questions about various aspects of their care (LeDoux & Mann, 2019). It is a strategy that promotes autonomy during the care process, which enhances learning.
