CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A MAINTENANCE EMPLOYEE WAS CHANGING V-BELTS ON AN EXHAUST FAN. HE DE-ENERGIZED THE FAN BEFORE STARTING WORK.
