Selective Laser Trabeculoplasty as a Replacement Therapy in Open Angle Glauco...CrimsonpublishersMSOR
The aim of this study was to investigate the long-term effect of selective laser trabeculoplasty in open angle glaucoma patients as a replacement for medical therapy. 64 eyes of 64 primary open angle glaucoma (POAG) patients under therapy with glaucoma drugs and controlled Intra Ocular Pressure (IOP) were reviewed for 36 months. 360o selective laser therapy (SLT) sessions were performed in two sessions and the medical therapy was stopped. The patients were controlled and evaluated postoperatively 1, 3, 6, 12, 24 and 36 months after surgery for glaucoma progression. In 36 (56.3%) of the 64 patients SLT was successful and there was no need to medical treatment at 36th month. In 28 patients (43.7%) medical treatment was started due to IOP rising. In Logistic regression analysis baseline IOP was found to be significantly effective on success rate of SLT (R2: 0.718, p<0.001).
Health care reforms in England during the last decade have been influenced by the idea that encouraging competition between hospitals, with nationally fixed prices, will increase the quality of care for patients. Some research has found a positive connection between competition and outcomes. A principal criticism of such studies has been the measures of quality of care that were used. This analysis uses NHS PROMs data, collected both before and after treatment, indicating the extent to which the surgery produces improvement in patients’ self-reported health status.
Following an approach common in the literature on competition, hospital market concentration is used as an indicator of competition. Data were collected for 2011–12 for all English NHS hospitals and all elective primary hip replacements.

Healthcare operations--the set of actions that organizations like insurers take to interact with their members--sit in some sort of nebulous shadow realm between social science, medicine, and corporate bureaucracy. In this talk, Sandy will throw some additional nouns that seem more at home in the modern web era, like "machine learning" and "A/B testing," into the mix. He'll also walk attendees through an example of now Clover Health builds and tests models for predicting which of diabetic members are likely to develop complications.

Aim: To study learning effect (LE) and test retest variability (TRV) in healthy subjects and patients with primary open angle glaucoma (POAG) using Rarebit perimetry (RBP). To determine normative ranges of RBP. Methods: 61 eyes of 35 subjects underwent visual field testing with standard automated perimetry (SAP) and RBP. TRV and LE were assessed in repeated examinations conducted in 3 different days. First two examinations were conducted within 3 days and the last one within one month. LE was assessed by comparing results from the three sessions. TRV was evaluated by calculating differences between retest for each combination of single tests. To determine normative ranges of RBP were included 34 eyes of 21 healthy subjects and 62 eyes of 47 subjects with preperimetric and early POAG. Cut off value was determined between the two groups using ROC analysis. Results: No significant LE was observed in POAG group. There was a significant LE in the control group but only in the visual field zones with eccentric location. TRV was higher in POAG group and in central visual field zone. The mean MHR in control group was 94.88 (SD 2.21) and 83.56 (SD 6.95) in POAG group. Cut off value for discriminating between healthy subjects and patient with POAG was 91.50% with AUROC 0.985 (p<0.001, ROC analysis). Conclusion: RBP is fast and easy to perform test. RBP testing did not show a significant LE in glaucoma group, however, TRV was consistent. MHR can be successfully used for differentiation of healthy eye from those with early glaucoma changes.

Case Stuidy:- Vision Performance-Based Measure in Diabetic Retinopathy Medical Practice .
Purpose:-
To validate a third-generation performance-based measure of visual function titled “Assessment
of Disability Related to Vision” (ADREV) in a study of patients with diabetic retinopathy.
Design:-
Prospective, cross-sectional study.
Methods
Patients with nonproliferative or proliferative diabetic
retinopathy, free from ocular comorbidity, were recruited
from a single institute and completed the ADREV, the 25-
Item National Eye Institute Visual Functioning
Questionnaire (VFQ-25), and a clinical ophthalmic
examination. Correlation, regression, and bootstrap
analysis were conducted to determine the relationship between ADREV scoring and each of the
study's clinical and self-report measures of visual ability, while controlling for potential
confounders.

Results:-
Ninety-one patients with diabetic retinopathy
completed the study and analysis showed that the
ADREV total and subscale scores shared a stronger
relationship with the clinical measures of visual function
than did the VFQ total and subscale scores. Regression
analysis revealed that binocular visual acuity, contrast sensitivity, and better eye visual field
were the best predictors of ADREV performance.

The ADREV performance measure is a valid instrument
for the assessment of disability related to vision in patients with diabetic retinopathy.
Furthermore, the assessments provided by ADREV were more related to traditional clinical
indicators of visual impairment than were the results of the self-report measure, specifically the
VFQ-25.
