Living it Up (LiU) is a digital self-management hub program led by NHS 24 and funded by the Ambitious vision. It aims to transform health, care, and wellbeing using familiar internet technologies. The program was driven by co-design with over 3,500 people and focuses on enabling self-management, prevention, and health/care integration centered around the individual within their community. Key services include video consultations, home health monitoring, social prescriptions, exercise prescriptions, and a personal care planning tool. The program delivered an adaptive digital platform and customizable options by November 2014 but did not achieve all goals such as the targeted community size or new sustainability vehicle due to legal constraints.
