Breast Cancer Screening
• Mammogram and Clinical Breast Exams
    – Women age 40 and older
    – Women younger than 40 at higher risk for
       breast cancer (Attestation Required)

Cervical Cancer Screening
• Pap Tests and Pelvic Exams
    – Women age 40 and older
    – Women younger than 40 at higher risk for
      cervical cancer (Attestation Required)

Colorectal Cancer Screening
    – Men and Women age 50 and older – stool
      sample test (FOBT, FIT)
    – Men and women of any age who are at
      increased risk for colorectal cancer
      (Attestation Required)

Working Together:

• To increase the number of health care
  providers who are following best
  practices for tobacco intervention.

• Funded by the New York State
  Department of Health, Tobacco Control
  Program.
Opportunities for Intervention

 Smokers:

   – 70% made at least one unsuccessful quit
     attempt

   – 70% visit a physician each year

   – Almost 50% try to quit each year

   – Less than 10% are successful without assistance

ASK about        Identify and document tobacco use status for every
tobacco use.     patient at EVERY visit.
ADVISE to        In a clear, strong and personalized manner urge
quit.            every tobacco user to quit.
ASSESS           Is the tobacco user willing to make a quit attempt at
willingness.     this time?
ASSIST in quit For the patient willing to quit, use counseling and
attempt.       pharmacotherapy to help him or her quit.

ARRANGE          Schedule followup contact, preferably within the
