This risk assessment form evaluates the risks of an exhibition featuring video art projected onto a radiator. It identifies audiences, cables/wires, confined spaces, and radiation as potential hazards. For each hazard, it lists the exposed persons, proposed actions to reduce risk, person responsible, and confirmation that exposed persons have been informed and risks removed. Actions include hiding wires to prevent tripping, setting up the projector to avoid eye contact, and facing the projector away from people while turning off the radiator heat to minimize radiation risk. The form is signed by the director after completing the assessment.

Complete the following table rating
each risks severity on scale from Negligible (N), Low risk (L), Moderate risk (M), Severe (S),
Very severe (V)
each risks likelihood on scale from Very unlikely (VU), Unlikely (U), Possible (P), Likely (L),

Very Unlikely Unlikely Possible Likely Very Likely
Very severe 3 4 4 5 5
Severe 2 3 3 3 5
Moderate 2 2 3 3 3
Slight 1 2 2 3 3
Negligible 1 1 2 2 3
Decide the action to take:
5 Very severe Take immediate action
4 Severe High priority
3 Moderate Risk Programme for action
2 Low Risk Action may be required
1 Negligible Probably acceptable
Then list each risk identified

Description AUDIENCES
Person(s) exposed
(detail if cast/crew (C),
outside company (O) or
public (P) )
CREW, AUDIENCE
Action to take There will be an audience at the exhibition. The main risk of this is the
potential of them tripping over wires around the exhibition space. The
wires will be hidden as best as possible, and the audience will be
encouraged not to stand in front of the projector.

3. Hazard no: 4 Risk Factor (1-5) 2
Description WIRES AND CABLES
Person(s) exposed
(detail if cast/crew (C),
outside company (O) or
public (P) )
CREW, AUDIENCE
Action to take The wires leading up to the projector and laptop create a large tripping
hazard for the audience in the area, so the wires must be hidden as
best as possible.

CAST & CREW
Action to take The exhibition space is fairly small, so the area must be made safe for
walking through. The projector must also be set up so it does not
connect with anybody’s eyes.

4. Hazard no: 19 Risk Factor (1-5) 1
Description RADIATION
Person(s) exposed
(detail if cast/crew (C),
outside company (O) or
public (P) )
CREW, AUDIENCE
Action to take The video art will be projected onto a radiator via a projector. The
projector radiates a light that is potentially harmful if stared at. The
radiator will most likely be turned off, but if it is turned on, this will also
produce heat. The projector will face away from people to reduce risk,
while the radiator presents little problem.
