This study characterized 4 ESBL-producing E. coli isolates from urology patients in Rawalpindi, Pakistan. The isolates were tested for antibiotic susceptibility and further analyzed at a reference laboratory in London. All 4 isolates were found to have CTX-M ESBL phenotypes based on their higher MIC for cefotaxime compared to ceftazidime. This suggests the emergence of CTX-M ESBLs in Pakistan is concerning and warrants further epidemiological and genetic studies to understand the spread of these resistant enzymes.

Split Pectorales Major and Teres Major Tendon Transfers for Reconstruction of...Peter Millett MD
Isolated ruptures of the subscapularis and anterosuperior rotator cuff lesions are encountered more rarely than supraspinatus or anteroposterior rotator cuff tears. In certain circumstances, reconstruction of the tendon may not be possible due to fatty degeneration and atrophy of the subscapularis muscle or poor tendon quality. Tendon transfer may represent the only surgical option for treatment. A pectoralis major tendon transfer is an acceptable salvage option for irreparable subscapularis tendon ruptures. Although limited functional goals may be expected in most cases, the majority of patients obtain a good pain relief, which improves their function below chest level. Addition of the teres major component to the transfer may be beneficial in cases where both the upper and lower portion of the subscapularis muscle is irreparable. This case report describes the resection of a large carotid artery aneurysm under cervical epidural anesthesia in a 22-year-old woman. Cervical blockade was not possible due to the location and size of the aneurysm. The patient underwent cervical epidural anesthesia, which allowed for continuous neurological monitoring during the surgery and early detection of any brain ischemia when the carotid artery was clamped. The large aneurysm was successfully resected over the course of a two hour surgery. The patient recovered well with no neurological defects. The report concludes that cervical epidural anesthesia performed by an experienced anesthetist provides safe neurological monitoring and is an acceptable approach for resecting carotid artery aneurysms when other methods are not possible.

Basic Cardiac Life Support (BCLS) provides the foundation for saving lives after cardiac arrest through maintaining adequate ventilation and circulation until the underlying cause can be reversed. The key components of BCLS are compression, airway, and breathing (CAB). There have been changes to the BCLS guidelines to emphasize the importance of high-quality chest compressions and minimize interruptions, including starting with 30 chest compressions before opening the airway. The goals of BCLS are to apply the links in the "Chain of Survival," including immediate response, early CPR with compressions, defibrillation if needed, advanced life support, and post-cardiac arrest care.
1. This study compared general anesthesia (GA) and spinal anesthesia (SA) for 100 patients undergoing lumbar disk surgery through a randomized controlled trial.
2. Intraoperatively, mean blood loss was less with GA but not significantly. Surgeon satisfaction was higher with GA. No major complications occurred with either.

This letter describes a new technique for managing difficult extubation when the tracheal tube cuff fails to deflate. A patient required laryngoscopy after extubation was initially unsuccessful due to an inflated cuff. Compared to adults, children require a larger dose of long-acting local anaesthetic like bupivacaine due to higher CSF volume, but the duration of block is shorter, around 1 hour for infants. Adjuvants can prolong the block duration but must be preservative-free. Younger children generally do not experience hypotension from spinal anaesthesia unlike adults. It has been used successfully for procedures like hernia repair and myelomeningocele repair in neonates. The needle design does not significantly impact complications like in adults.
Rapid sequence spinal anesthesia (RSS) is a technique used for urgent cesarean sections that requires effective coordination between medical staff. Segmental spinal anesthesia involves puncturing the spinal cord at higher thoracic levels using lower doses of local anesthetic, allowing selective blockade of dermatomes needed for surgery. This technique provides hemodynamic stability, less motor blockade, and faster recovery compared to conventional spinal anesthesia. Careful performance of segmental spinal anesthesia can establish it as a routine procedure for day surgery.

This study evaluated patient controlled sedation (PCS) using propofol and alfentanil for dressing changes in 11 burn patients with over 10% total burn surface area. PCS was compared to sedation provided by an anesthesiologist. Patients preferred PCS due to greater control and less discomfort during recovery. No adverse respiratory or cardiovascular events occurred with PCS. Procedural pain was higher with PCS but lower after the procedure. The study concluded that PCS is an effective and safe alternative to anesthesiologist-provided sedation for burn dressing changes, but noted the small sample size limited the strength of this conclusion and further studies are warranted.
