Prenatal exposure to recreational drugs affects global motion
perception in preschool children.
A very interesting paper that adds another piece of the puzzle to the puzzle of FASD and the neurodevelopment disabilities of prenatal alcohol exposure.
I found in my practice, consistently, that those diagnosed with the neurodevelopment disabilities of fasd found relief from marijuana and tobacco. The relief had to be a neurological process that resulted in a calming of uncontrolled thoughts and emotions.
It is a tragedy that mainstream science does not see prenatal alcohol exposure as the significant tool for the investigation of brain function that it is.

It is my observation that those with FASD exist in two states,
1 -a mind of chaotic, uncontrolled and uncomfortable thoughts, usually described as being bored.
2- a mind perseverating [ super focused ], with or without physical activity.
They seek the second to escape the first.
What they perseverate on is determined by their particular set of cognitive, emotional, information processing, memory, expressive and sensory disabilities; as well as their early childhood experience and their immediate environment, including how others relate to them.
What they may perseverate on to soothe themselves extends from cutting, provoking others, to more acceptable behaviors, such as playing video games, reading and sports.
Alcohol and hard drugs are used to obliterate the 1st state of mind. Those with FASD can often stop using them providing they have an alternative focus of perseveration.
This is not true of Marijuana and Tobacco, which generally appear to have a specific action that reduces their multiple chaotic thoughts and allows them to focus on one process.

Schizophrenia
The topic selected was schizophrenia. This is a severe and chronic mental disorder affecting the way humans think, feel and behave. Although this mental disorder is not very common, it has disabling symptoms.
Schizophrenia has continuously been viewed as a disorder that occurs in neurodevelopment. It is associated with a microdeletion syndrome of a chromosome. Neurons migrate to the pial surface from the brain's ventricular zone. "Migration of the neuron cell body is mediated via microtubule-based transport organized by the centrosome. First, the centrosome moves up the microtubule, followed by the nucleus and the cell body” (Pearlson, 2015). Neuronal migrations are reportedly caused by Reelin.
The etiology of schizophrenia is unknown, however, several risk factors have been associated with its development. These include environmental and genetic factors. Genetic factors are, however, insufficient to account for the development of the disease. They must be accompanied by other factors for the disease to develop. "Because the concordance rate for monozygotic twins only approaches 50%, genetic makeup alone is not sufficient for the development of schizophrenia, and non-genetic or sporadic forms of the disorder must exist” (Miyamoto et al, 2013).
Environmental factors that play an important role in the etiology of schizophrenia include obstetric complications such as hypoxia, preeclampsia, and premature birth. Other environmental factors include maternal viral infections and stress occurring in neurodevelopment. Stress during neurodevelopment may be caused by other factors such as microbial infections.
The pathology of schizophrenia includes the transcriptional dysregulation at the cerebral cortex and chromosomal conformations. “Anatomic, neurotransmitter and immune system abnormalities have been implicated in the pathophysiology of schizophrenia” (Miyamoto et al, 2013). Differences have been observed in the brains of people with schizophrenia compared to the brains of people without the disease in neuroimaging studies. The medial temporal areas of the brain have a decreased volume while the ventricles are larger. Structural abnormalities such as volume reductions and ventricular enlargements have been associated with the disease.
The dopaminergic system abnormalities are also associated with schizophrenia. The immune system is also disturbed in people with the disease. “Overactivation of the immune system (eg, from prenatal infection or postnatal stress) may result in overexpression of inflammatory cytokines and subsequent alteration of brain structure and function” (Nuckols et al, 2013). Anatomic abnormalities may also be observed in the hippocampus.

Abstract
A 15-year-old girl diagnosed with FASD underwent 100 courses of hyperbasic oxygen therapy (HBOT). Prior to HBOT, single motion emission compute tomographic begin imaging (SPECT)
revealed areas of hypo-perfusion bilaterally in the orbitofrontal region, temporal lobes and right dorsolateral—frontal, as well the medial aspect of the left cerebellum. Following two sets of HBOT treatments (60 plus 40), over 6 months, there was improvement in perfusion to the left cerebellum as well as the right frontal lobe. This was paralleled by improvement in immediate cognitive tests and an increase in functional brain volume. A follow-up 18 months after HBOT showed sustained
improvement in attention with no need for methylphenidate, as well as in math skills and writing.
This year as a priority of Proof Alliance’s legislative platform, major legislation that requires all children entering foster care be screened for prenatal exposure to alcohol in Minnesota was passed and signed into law. It is believed Minnesota is the first state in the nation to pass this legislation.

The possible equivalence to the brain activity of those with FASD in relation to “being bored” is explored, with reference to brain anatomy and function.
Actual FASD clinical cases are presented to illustrate what individuals with FASD mean by “boredom”: describing the role of perseveration as a relief process.
Finally, the manner in which these processes are misinterpreted is explored, with implications for Psychiatry and the Justice System.

Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.

Abstract
Objective:
The purpose of this study was to determine whether microglial activity,
measured using translocator-protein positron emission tomography
(PET) imaging, is increased in unmedicated persons presenting with
subclinical symptoms indicating that they are at ultra high risk of
psychosis and to determine whether microglial activity is elevated in
schizophrenia after controlling for a translocator-specific genetic
polymorphism.
2. The authors used the second-generation radioligand [11
C]PBR28 and
PET to image microglial activity in the brains of participants at ultra
high risk for psychosis. Participants were recruited from early
intervention centers. The authors also imaged a cohort of patients with
schizophrenia and matched healthy subjects for comparison. In total,
56 individuals completed the study. At screening, participants were
genotyped to account for the rs6971 polymorphism in the gene
encoding the 18Kd translocator protein. The main outcome measure
was total gray matter [11
C]PBR28 binding ratio, representing microglial
activity.
Results:
[11
C]PBR28 binding ratio in gray matter was elevated in ultra-high-risk
participants compared with matched comparison subjects (Cohen’s d
>1.2) and was positively correlated with symptom severity (r=0.730).
Patients with schizophrenia also demonstrated elevated microglial
activity relative to matched comparison subjects (Cohen’s d >1.7).
Conclusions:
Microglial activity is elevated in patients with schizophrenia and in
persons with subclinical symptoms who are at ultra high risk of
psychosis and is related to at-risk symptom severity. These findings
suggest that neuroinflammation is linked to the risk of psychosis and
related disorders, as well as the expression of subclinical symptoms.

CONCLUSION
FASD have a devastating effect on society and on the individual. Human imaging
studies have revealed the presence of structural changes that occur throughout the
CNS as well as neuropsychological and behavioral changes that contribute to the
clinical diagnosis of FASD. Future studies will undoubtedly elaborate on the
correlation between specific neuropathological changes in the brain and behavioral
alterations. Animal models have been instrumental in defining the relationship
between temporal exposure to alcohol during early stages of development and the
sequelae of events that follow as the animal ages. The neuroimmune system is
emerging as central player in alcohol pathology although the extent of our
knowledge is still in its infancy. Microglia appear to be one of the most important
players in this regard. These cells normally function in maintaining homeostasis
within the CNS, however, if chronically activated they can cause damage to
resident cells and tissues. The microglial response to alcohol is complex and
involves the TLR signaling pathway. A more complete understanding of the
signaling pathways involved in alcohol modulation of neuroimmune mechanisms
in the CNS as well as the role of these molecules in modulating addiction to
alcohol must be a focus of future research studies.
