This document outlines various policies and guidelines for nursing students regarding their clinical duties. It discusses uniforms, attendance, conduct, and consequences for violations. 3. a. Reporting time
For Nursing Circle:
6:15 am for 7 – 3 shift
2:15 pm for 3 – 11 shift
10:15 pm for 11 – 7 shift
b. A student with incomplete/crumbled/dirty
uniform should not be allowed to go on duty
and is considered absent.

4. Male:
Prescribed white polo; white sando (no
printed shirt is allowed; white pants; white
socks (regular size); badge and nameplate.
Female:
Prescribed striped blue uniform; white
apron; white chemise; stockings and shoes;
nameplate and a white cap.

5. c. Requirements:
c.1. Hair should be kept tidy and not touching the
collar. Hair color is not allowed. Only black or
dark brown accessories are allowed (for
female only).
c.2. Wearing of jewelry is not allowed; heavy
perfume is discouraged; light make-up is
encouraged; fingernails should be cut short;
colored nail polish is not allowed.

6. c.5 Personal hygiene is a must.
c.7 Using cellphone is not allowed during
duty.
c.8 Students are not allowed to receive
visitors during duty time.
c.9 Complete of paraphernalia, borrowing
from group mates is not allowed.

7. d. Attendance
Student is expected to come on duty on time
everyday according to its area of assignment
and on the specified dates of exposure.
d.1 Tardiness – 3 accumulated late is considered
1 day absent. 1:3.
d.2 Excused absences due to:
-- illness: present medical certificate
-- death of immediate relative, natural
disasters and calamities
-- replacement equivalent to number of
days absent. 1:1

8. d.3 Unexcused absences such as:
-- violation of uniform regulation
-- failure to submit requirements
-- attendance to fiesta, wedding, birthday, etc.
d.4 A student concern should submit a letter of
explanation, noted by the Chairman and
approved by the Dean before she or he is
allowed to go on duty again.

9. e. Blunders and Misdemeanors
e.1. Error in administration of medications
-- administration of IV push medication
without the CI’s presence
-- flushing of clogged IV’s
-- failure to observe the rights of medication
administration.
Penalty of 3 days extension and/or
may also mean expulsion depending
on the gravity of the case.
-- giving medication prepared by other
students.

discomfort, harm to their patient.

e.2 Leaving the area without proper permission
or beyond the time allotted for snacks (15 min)
and meals (30 min). 1 day extension
e.3 Going out without proper endorsement to
other member of the health team. 1 day
extension for every offense.

11. e.4 Entertertaining visitors during duty hours
without the CI’s permission; loitering in the
clinical area without valid reasons. Reading of
non-professional magazine and pocketbooks.
Sleeping/napping during duty hours.
e.5 Copying of other student’s written requirements.
A grade of 5.0 for that requirement and a
warning. Second offense would mean sanction
for cheating will apply.

12. e.6 Other misdemeanors and offenses are also
subject to disciplinary measures in accordance to
university are as follows:
-- caught drinking alcoholic beverages, taking
prohibited drugs, engaging fighting, gambling
extortion and vandalism.
-- smoking inside the classroom, hallways and
hospitals.
-- possession/circulating pornographic materials.
-- verbal assault to a teacher, student, etc.

13. e.7 A student who fails in the clinical area
that is a component of the whole exposure
even if with an over-all passing grade, he/she
will be sent for completion of duty on the
same area after deliberation within Marawi
City and Iligan City health facilities.
