Tardiness/ Absenteeism <br /><ul><li>Grace Period: 15 minutes for 6-2 shift ONLY</li></ul> No grace period for 2-10 and 10-6 shifts<br /><ul><li>Late in reporting for work at specified time:</li></ul> 1st offense: Verbal Reprimand<br /> 2nd Offense Written Reprimand<br /> 3rd Offense 1-2 days suspension<br />Note: The Senior Nurse will collect the Daily Time Record to be submitted to the Nurse Supervisor/ Charge Nurse on duty.<br />
  4. 4. C. Absence without official leave or notice:<br /> 1st Offense: Written Reprimand<br /> 2nd Offense: 3-4 days suspension<br /> 3rd Offense: 1 schedule suspension<br />Note:<br /><ul><li>A Medical Certificate should be submitted in case the reason of absence is health.

  9. 9. Emergency leave or Absent (scope of emergency leave/ absent: Life and death situation in immediate family member/s ONLY). Should submit a written letter and necessary documents for approval. All emergency leave/ absent is still subject for approval.</li></li></ul><li>2. Sleeping during working hours or neglect, or abandonment of assigned duty or post.<br /> 1st Offense: 3 days suspension<br /> 2nd Offense: 1 week suspension<br /> 3rd Offense: one schedule suspension<br />Note:<br /><ul><li>Nap for 30 mins to 1 hour is allowed only at assigned station ONLY AT NIGHT.
  10. 10. Sleep/ nap at rooms, hospital bench, and pantry will automatically subject for suspension.
  11. 11. Anybody can report the incident.</li></li></ul><li>3. Theft of hospital/ client’s property regardless of the quality/ value, conniving with others in stealing hospital/ client’s property.<br /> 1st Offense: Dismissal<br />4. Inefficiency in the performance of assigned work<br /><ul><li>Penalty depending on the grounds of mistake
  12. 12. Penalty to be given by the hospital administrator/ chief
  13. 13. Medication error= Gross negligence</li></li></ul><li>5.Automatic suspensions to be given for the following reason<br /><ul><li>Reporting to work under the influence of liquor/ drugs or participating in any drinking spree with the hospital premises and during duty
  14. 14. Habitual absences with or without prior notice
  15. 15. Those who did not notify absence 8 hours prior to specified time of duty
  16. 16. Once the individual proven for the integrity issue, he/she will have an automatic suspension
  17. 17. If an individual found doing conspiracy on the policy given to them, automatic suspension will be applied.</li></li></ul><li>6. Billing<br /><ul><li>Make sure that all remaining/ unused IVFs, medicines, materials charged to the patient with discharge order must be returned to the pharmacy for cancellation to avoid in causing delay in processing bills.
  18. 18. Direct rendering charges such as nebulisation, oxygen inhalation should be charged every shift. </li></ul>7. Medicines<br /><ul><li>All medicines purchased outside the hospital should be kept the patient’s bedside.</li></li></ul><li>8. Uniform<br /><ul><li>Schedule of wearing uniformsMonday- Friday (6-2, 2-10, and AM): white uniform
  19. 19. Saturday, Sunday, Holidays, 10-6, and PM shifts: scrub suit
  20. 20. IDs should be STRICTLY worn within the hospital premises.
  21. 21. FOR FEMALE: hairnet and clinical cap should be worn while on duty
  22. 22. FOR MALE: should be properly groomed</li>
