Changes in white blood cell count after primary percutaneous coronary intervention are more prognostic of outcomes for patients with ST elevation myocardial infarction than baseline values. The study measured serial white blood cell counts before and after PCI in STEMI patients and found that persistent white blood cell elevation was an even stronger predictor of poor prognosis and increased mortality than a single high baseline count.

Some studies found associations between hypertension and perioperative complications, but others found no relationship between elevated preoperative blood pressure measurements and complications. Revascularisation strategies
1) Early revascularization through either percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) improves survival rates in patients with cardiogenic shock compared to medical therapy alone.
2) There is ongoing debate about whether culprit vessel-only PCI or multivessel PCI is better for patients with cardiogenic shock and multivessel coronary artery disease. Current guidelines recommend culprit vessel PCI initially except in certain high-risk situations.
3) An integrated approach is needed using early revascularization, inotropic support, and potentially mechanical circulatory support, with involvement of multiple specialists. Cardiogenic shock continues to pose major challenges in acute cardiac care.

1. Patients with chronic kidney disease (CKD) have a high prevalence of arterial and valvular disorders such as endothelial dysfunction, vascular calcification, arterial stiffness, annular and valvular calcification, and valvular stenosis and regurgitation.
2. The pathophysiology of these disorders in CKD involves impaired endothelial function, altered collagen turnover, and vascular smooth muscle cell dysfunction leading to calcification.
3. Arterial and valvular disorders in CKD result in increased cardiovascular disease and mortality. Progression of calcification is associated with higher mortality in dialysis patients.

This study analyzed clinical outcomes in 2002 STEMI patients undergoing primary PCI. Patients were divided into 3 age groups: <60, 60-79, and ≥80 years old. The study found that rates of adverse events like mortality, recurrent MI, stroke, and major bleeding increased with age. Specifically, patients ≥80 years old had higher rates of comorbidities and experienced the highest rates of ischaemic and bleeding complications compared to younger patients, even after adjusting for confounding factors. The study concluded that elderly STEMI patients, especially those ≥80 years old, are a high-risk group that requires individualized therapy to optimize clinical outcomes.
Management of premature ventricular complexes
Isolated monomorphic premature ventricular complexes (PVCs) without
structural heart disease are generally benign.
► Frequent PVCs can cause reversible cardiomyopathy or aggravate an existing
cardiomyopathy.
► Short coupled PVCs can trigger sustained ventricular fibrillation. These are
often from the Purkinje tissue or rarely the outflow tract.
► Beta blockers are considered first-line therapy but have low efficacy.
Catheter ablation and AADs are reasonable to suppress PVCs in appropriate
patients.
► Ablation is often curative and success depends on location and accessibility
of PVCs.
► Implantable defibrillators are reasonable in patients at higher risk of sudden
cardiac death.
Acute kidney injury in cardiac surgery
Although there has been much high-quality research conducted in this field in recent years, preventing CSA-AKI by avoiding renal insults remains the mainstay of management. Biomarkers have the potential to diagnose CSA-AKI at an earlier stage, but efficacious interventions to treat established CSA-AKI remain elusive. Off-pump coronary artery bypass may be associated with a lower risk of CSA-AKI compared to on-pump procedures, but this has not been shown to impact long-term renal or clinical outcomes.

1) Complete revascularization (CR), defined as treating all significant coronary stenoses, is associated with lower mortality compared to incomplete revascularization (IR) based on observational studies and randomized trials. IR is more common after percutaneous coronary intervention (PCI) than coronary artery bypass grafting (CABG).
2) For stable coronary artery disease (SCAD), CR is recommended when feasible, while for acute coronary syndromes (ACS) and ST-segment elevation myocardial infarction (STEMI), treating the culprit lesion only is usually recommended initially, with staged revascularization of non-culprit lesions if needed.

A retrospective chart review was conducted of 91 patients admitted with sepsis between 2013-2014 to examine outcomes of central venous catheters (CVCs) and arterial catheters (ACs). 52 CVCs were placed in 36 patients and 48 ACs in 37 patients, with 32 receiving both. Of those with CVCs, 7 complications occurred in 10 patients (19% rate). Of those with ACs, 3 complications in 4 patients (8.1% rate). Logistic regression found no significant difference in mortality between those who received lines and those who did not, including among patients with higher illness scores. The study concludes that CVC and AC placement confers no significant improvement in survival for sepsis patients and can lead to unnecessary complications

Invasive hemodynamic monitoring using a pulmonary artery catheter can help guide treatment for complex patients by providing detailed cardiac information. While use of these catheters is declining somewhat, the data they provide on issues like cardiogenic shock and pulmonary pressures can still be useful for diagnosis and management. For a complex patient, this information could be used to individualize a treatment plan involving medications, devices, or other therapies. However, there are also risks to consider from potential complications of catheter insertion or use.

3) A study of 738 patients with chronic total blockages and good collateral blood flow found that revascularizing the blockages reduced cardiac death and total cardiac events compared to medical treatment alone, showing benefit of revascularization even in patients with established collateral circulation.

This study aimed to correlate 2D speckle tracking echocardiography with coronary angiography results in 100 patients with non-ST elevation myocardial infarction (NSTEMI). Echocardiography, including speckle tracking, was performed to assess wall motion abnormalities and territorial longitudinal strain. Coronary angiography identified the culprit artery. Speckle tracking showed high sensitivity and specificity in identifying the culprit artery compared to angiography results, with sensitivity of 93.3% for left anterior descending artery, 82.7% for left circumflex, and 84% for right coronary artery. This suggests speckle tracking may help localize coronary stenosis and identify which NSTEMI patients could benefit from revascularization.

The primary endpoints were self-reported health status and NYHA functional class at 24 weeks, with secondary endpoints including 6-minute walk distance and quality of life.
Levosimendan vs dobutamine for patients with acute decompensated
The SURVIVE study was a randomized, double-blind trial that compared the efficacy and safety of intravenous levosimendan vs dobutamine in 1,327 patients hospitalized with acute decompensated heart failure who required inotropic support. The trial found no significant difference in all-cause mortality at 180 days between the levosimendan and dobutamine groups. However, the levosimendan group had greater decreases in B-type natriuretic peptide levels over the first 5 days. Both drugs showed side effects, with higher rates of cardiac failure in the dobutamine group and atrial fibrillation, hypokalemia, and headache in the levosimendan
Randomized comparison of everolimus eluting stents and sirolimus-eluting stents
This study compared everolimus-eluting stents (EES) to sirolimus-eluting stents (SES) in 500 STEMI patients undergoing primary angioplasty. No significant differences were found between the EES and SES groups in major adverse cardiac events, death, reinfarction, or target vessel revascularization. However, the EES group had a significantly lower rate of stent thrombosis compared to the SES group at 3-year follow-up. Therefore, among STEMI patients undergoing primary angioplasty, EES was found to have similar efficacy as SES but was associated with a reduced risk of stent thrombosis.
Correlation between the timi risk score and high risk angiographic findings

Recomendações da OMS sobre cuidados maternos e neonatais para uma experiência pós-natal positiva.
Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.
