Abstract
Does the United States need more midwives? This is the question this paper will explore by looking at the fact that the US has a very high newborn mortality rate and that we use more OB-GYNs for deliveries than any other country. I find that there is a correlation between OB-GYNs and the high infant mortality rate due to the fact that OB-GYNs are surgeons and they use surgery (cesarean sections) that is a lot more risky than a vaginal birth. Midwives only use surgery when necessary and their less risky tactics lead to lower newborn death rates or labor complications. Therefore, I concluded that the United States does need more midwives.
Does the United States need more Midwives?
The United States newborn death rate is the second highest in the world, this is a fact that many people are unaware of today. With the amount of money the US spends on healthcare, we should not have a rate this high. Because OB-GYNs are used more in the US than any other country, you have to wonder if there was a correlation between the two. After research, I have found a correlation between OB-GYNs and newborn death rate because of labor interventions that lead to cesarean sections. Midwives don’t have the same outcome as OB-GYNs. Midwives aren’t as quick to use interventions and their cesarean section rate is a lot lower than OB-GYNs leading to lower risk labors and therefore a lower newborn death rate. Therefore, the US needs more midwives.
Literature Review
In a documentary by Lake it is said that among 33 industrialized nations, the United States is tied with Hungary, Malta, Poland and Slovakia with a death rate of nearly 5 per 1,000 babies, according to a report from Save the Children (April 2006). This is the second worst newborn death rate in the developed world. The five countries with the lowest infant mortality rates (Japan, Singapore, Sweden, Finland and Norway) midwives were used as their main source of care for 70 percent of the birthing mothers (2008).
Cesarean sections have many more risks than vaginal births. C-sections are more likely to occur because of the interventions during labor. Pitocin is a common intervention that puts the baby in stress during labor, which leads to a cesarean section. OB-GYNs are a lot more likely to use these interventions than midwives. Lewis Mehl-Madrona, MD, PhD, coordinator of Integrative Psychiatry and Systems Medicine at the University of Arizona College of Medicine Program in Integrative Medicine, comparing 1,046 home births to 1,046 hospital births found negative outcomes consistently higher in hospital births. These included a fetal distress rate six times higher in hospitals, a respiratory distress rate 17 times higher in hospitals, babies requiring resuscitatio.
Abortion can have serious physical and psychological complications. It is linked to increased risks of cancer, uterine damage, cervical lacerations, ectopic pregnancy, and complications in future pregnancies like preterm birth. Multiple abortions further increase these risks. Abortion is also associated with negative health behaviors and poorer overall health and well-being afterwards. It is a traumatic experience for many women that can be perceived as a violation, especially if the pregnancy was wanted or the abortion was forced.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Af ter the birth of a son, the majority of the parent population are worried and some
consider undertaking this procedure on their son.. Circumcision is theprocess of removal of

newborns, particularly in the United States. Parents usually f ind it hard to make a decision
whether to get the process done or not. Earlier, it was a cultural and religious custom to get
the circumcision done. Hence, there wasn’t much issue with respect to a decision to be
made. Later as time passed by, inf ant circumcision has become the sole responsibility and
decision of the parent.

such a scenario, the members and
other stakeholders of the American
Academy of Paediatrics
(AAP)grouped up to f orm a
multidisciplinary task f orce in the
year 2007. Their main aim was to
update the recommendations made
by the Academy in the year 1999. The result of the evaluation done on the recent evidence
highlighted the f act that benef its of inf ant circumcision outweigh all the risks involved in the
process. In addition, it justif ies the decision made by the f amilies that supported the
process. The Circumcision Policy Statement of Pediatrics Volume 130, Number 3,
September 2012 clearly explains about this f act. There are numerous benef its with respect
2. to this process. Few among them are as given below.
Urinary tract infection
UTIs are not much common in men. Yet, inf ants below the age of one have a high risk of
this inf ection. Studies have reported that when compared to circumcised male inf ants,
uncircumcised inf ants are more susceptible to this inf ection. If overlooked, it may even
lead to kidney inf ection as well. Circumcised inf ants don’t need to worry about this
inf ection.
Penile Cancer
Circumcised men don’t need to worry about getting af f ected by penile cancer. Whereas f or
uncircumcised men is a matter of concern. At the same time, women with uncircumcised
male partners is more susceptible to cervical cancer as well.
Sexually transmitted infection
Circumcision reduces the risk of sexually transmitted diseases like HPV, Herpes Simplex
Virus type 2 and HIV. The chances of these inf ections are really high in uncircumcised men.
Though circumcision cannot eliminate the risk of these inf ections, it can reduce the chances
to a great extent. Physicians usually recommend inf ant circumcision rather circumcising in
later stages of lif e.
It is very common f or parents to get tensed about the risks in the inf ant circumcision
process. There are no risks involved if the process is conducted in sterile condition by
expert and trained professionals.
Complications are very minor and rare. Inf ant circumcision is recommended as the
complication rate will be considerably low when compared to later stages. Parents are
responsible to take ultimate decisions. Hence, clinicians are strictly recommended to

American Academy of Paediatrics (AAP), even the American College of Obstetricians and
Gynaecologists have certif ied these f acts.
