This document summarizes a presentation given by @hupertan on big data analytics in urology. The presentation took place on December 7, 2019 at the Rencontres d'Urologie de Fontevraud conference. The presentation discussed the history of health data, definitions of big data, artificial intelligence, machine learning, and deep learning. It provided examples of current medical applications of these technologies and speculated on the future of urology with big data. The presentation was given remotely over Zoom.

Automated image analysis has had a long history but continues to grow with massive improvements in algorithms, speed, performance, and with emerging opportunities for high throughput tissue biomarker analysis and automated decision support for primary diagnostics. Of particular importance is the development of computer vision and image analysis for H&E stained samples. This talk will outline recent advances in automated tissue analysis for biomarker discovery and diagnostics and how adoption of digital pathology will drive the demand for quantitative imaging and decision support.
As an example, PathXL have developed TissueMark for the automated identification and analysis of tumour in lung, colon, breast and prostate cancer digital H&E slides. The conventional pathological estimation of % tumour nuclei in H&E samples shows gross variation between pathologists, undermining the quality of next generation sequencing, molecular testing and patient therapy and potential of false negative diagnoses. TissueMark uses a combination of pattern recognition, glandular analysis and nuclear segmentation to identify premaligant and invasive cancer patterns in H&E stained tissues and use this to assess tumour cell numbers and annotate samples for nucleic acid extraction and molecular profiling. Benchmark data was generated to validate TissueMark technology and showed concordance of automated data with manual counts, accelerating tumour markup and improving sample quality assessment. This represents an example of how automated imaging of tissue samples can be of immense value in quantitative tumour analysis for molecular diagnostics, thereby improving reliability in discovery and diagnostics.
This together with other examples in pathology research and practice will demonstrate that next generation tissue imaging technology in digital pathology could radically change how pathology is practiced.

In order to classify Covid 19 disease datasets such mild, middle and severe diseases, the proposed model utilizes the notion of controlled machine education and GWO optimization to regulate if the patient is affecting or not. An efficiency analysis is calculated and compared of disease data for both algorithms. The results of the simulations illustrate the effective nature and complexity of the data set for the grading techniques. Compared to SVM, the suggested model provides 7.8 percent improved prediction accuracy. The prediction accuracy is 8 better than the SVM. This results in an F1 score of 2 percent better than an SVM forecast. The semi-LASER sequence optimized for 2-HG detection with a TE of 110 ms successfullydemonstrated distinct cystathionine peaks in glioma patients with molecularly definedoligodendroglioma (IDH-mutant and 1p/19q codeleted) at 7T. While a prospective, betterpowered study is needed to confirm our observations, we propose that our method has thepotential to allow presurgical stratification of patients with IDH-mutant glioma into those witholigodendrogliomas and astrocytomas; which is of important prognostic significance.

Background: The digital twin paradigm holds great promise for healthcare, most importantly efficiently integrating many disparate healthcare data sources and servicing complex tasks like personalizing care, predicting health outcomes, and planning patient care, even though many technical and scientific challenges remain to be overcome. Objective: As part of the QUALITOP project, we conducted a comprehensive analysis of diverse healthcare data, encompassing both prospective and retrospective datasets, along with an in-depth examination of the advanced analytical needs of medical institutions across five European Union countries. Through these endeavors, we have systematically developed and refined a formal Personal Medical Digital Twin (PMDT) model subjected to iterative validation by medical institutions to ensure its applicability, efficacy, and utility. Findings: The PMDT is based on an interconnected set of expressive knowledge structures that are calibrated to capture an individual patient’s psychosomatic, cognitive, biometrical and genetic information in one personal digital footprint in a manner that allows medical professionals to run various models to predict an individual’s health issues over time and intervene early with personalized preventive care.Conclusion: At the forefront of digital transformation, the PMDT emerges as a pivotal entity, positioned at the convergence of Big Data and Artificial Intelligence. This paper introduces a PMDT environment that lays the foundation for the application of comprehensive big data analytics, continuous monitoring, cognitive simulations, and AI techniques. By integrating stakeholders across the care continuum, including patients, this system enables the derivation of insights and facilitates informed decision-making for personalized preventive care.

Diagnostic Efficacy of Ultra-High-Field MRS in Glioma PatientsUzay Emir
Synopsis: We have recently initiated a 7T MRS glioma consortium, intending to bring together experts in the field to discuss pitfalls, promises, and potential research avenues of MRS in gliomas. During the "GlioMaRS-NET Workshop" in 2020, we tested the efficacy of UHF MRS for predicting the molecular characteristics by visual inspection by conducting a survey with a set of previously acquired UHF spectra from glioma patients. In the post-genomics era, the World Health Organization (WHO) classification of gliomas has become even more tightly integrated with molecular parameters in addition to histology. Integrated diagnoses offer prospects for precision medicine strategies tailoring therapies for each individual. The International Society of Neuropathology-Haarlem Consensus has proposed the personalized layered diagnosis for gliomas that merge all of the distinct but related information. For the final diagnosis, the concept of integrated diagnosis (Layer 1) relies on all available data, which include the histological classification (Layer 2, e.g., astrocytoma vs. oligodendroglioma), the grading (Layer 3, e.g., WHO grade II vs. III), and molecular characteristics (Layer 4, e.g., isocitrate dehydrogenase (IDH)-mutant, 1p/19q-codeletion) (1). Thus, the personalized layered diagnosis approach has emphasized to tailor the diagnostic imaging to contribute to the integrated diagnosis by discovering reliable imaging biomarkers and tools capable of differential diagnosis according to genetic subtypes. It has recently been shown that definitional features of glioma, such as IDH mutation and 1p/19q codeletion (Layer 4), can be identified with non-invasive MRS at 3T, opening up exciting new opportunities for diagnostics, clinical trials, and assessment of treatments (2,3). Ultra-high-field (UHF, >=7T) MRI scanners offer enhanced detection relative to routine 3T MRI of 2-HG peaks in the MRS spectra of IDH mutated patients (4,5). This is due to increased SNR at the higher field strength and increased spectral dispersion, which can improve the delineation of 2HG from neighboring metabolites such as glutamate and glutamine. Furthermore, at 7T, the 2-HG peak is not only to contribute to layer 4 of the integrated diagnosis by identifying molecular features but also to detect subtle changes due to the reprogramming of cellular metabolism during the disease progression or treatment. With the recent successes of 2-HG and increased use of UHF MRI in clinical settings as a basis, we have recently initiated a 7T MRS glioma consortium, intending to bring together experts in the field to discuss pitfalls, promises, and potential research avenues of MRS in gliomas. Predictive model for acute myocardial infarction in working-age population: a...IJECEIAES
Cardiovascular diseases are the leading cause of mortality in Latin America, particularly acute myocardial infarction (AMI), which is the primary cause of atherosclerotic cardiovascular morbidity. This study aims to develop a predictive model for the probability of AMI occurrence in the working-age population, based on atherogenic indices, paraclinical variables, and anthropometric measures. The research conducted a cross-sectional study involving 427 workers aged 40 years or older in Popayán, Colombia. Out of this population, 202 individuals were screened with a 95% confidence interval and a 5% error margin. Epidemiological, anthropometric, and paraclinical data were collected. A binary logistic regression model was employed to identify variables directly associated with the probability of AMI. Predictive classification models were generated using statistical software JASP and the programming language Python. During the training stage, JASP produced a model with an accuracy of 87.5%, while Python generated a model with an accuracy of 90.2%. In the validation stage, JASP achieved an accuracy of 93%, and Python reached 95%. These results establish an effective model for predicting the probability of AMI in the working population.

Background: The waist-to-height ratio (WHtR) may be a better
adiposity measure than the body mass index (BMI). We
evaluated the prognostic performance of WHtR in patients
with acute ischemic stroke (AIS). Methods: First, we compared
WHtR and BMI as adiposity measures in 712 healthy
adults by tetrapolar bioimpedance analysis. Thereafter,
baseline WHtR was analyzed as predictor of 12-month allcause
mortality in 821 Mexican mestizo adults with first-ever
AIS by a Cox proportional hazards model adjusted for baseline
predictors. Results: In healthy individuals, WHtR correlated
higher than BMI with total fat mass and showed a higher
accuracy in identifying a high percentage of body fat (p <
0.01). In AIS patients a U-shaped relationship was observed

Lors du 113 Congrès de l'Association Française d'Urologie, le Dr Hupertan, membre du Comité d'Andrologie et de Médecine Sexuelle, a fait une présentation autour de la vasectomie. A partir d'une présentation destinée aux urologues, une version adapté «Grand public» a été réalisée. La présentation contient des points juridiques, des aspects technique et administratifs. Des hypothèses pour expliquer le retard français, ont été également évoqué. N'hésitez pas à commenter et diffuser.
La vasectomie contraceptive, est aujourd'hui la techique de contraception masculine la plus répandue. Plus de 60 millions d'hommes ont bénéficié déjà de cette technique. On connait les effets secondaires (il y en a pas), les risques, les modalités techniques. Néanmoins, cette technique est peu répandue en France. Pour casser les mythes et les idées reçues, j'organise des FB live pour répondre aux question.
La vasectomie contraceptive est une méthode de contraception masculine définitive. Il s'agit d'une opération chirurgicale bénigne qui consiste à ligaturer les deux canaux déférents pour empêcher les spermatozoïdes de se mélanger au liquide spermatique.

Traitement de l’adénome de la prostate au laser Greenlight XPS : proposition d’un outil de prédiction (nomogramme) préopératoire des résultats fonctionnels attendus

Objectifs
La photovaporisation prostatique (PVP) au laser Greenlight est utilisé de façon exponentielle depuis quelques années. Les résultats fonctionnels sont hétérogènes d’une série à l’autre. De fait, certains collègues sont réticents à l’utilisation de cette technique. Le but de ce travail était de proposer un modèle capable de prédire les résultats fonctionnels de la PVP lors de la consultation pré opératoire.
Méthodes
Le nomogramme a été construit à partir des données inclues dans une base prospective monocentrique sur 300 PVP consécutives. Les résultats postopératoires étaient évalués à 3 mois (scores IPSS et qualité de vie (QdV)). Un résultat fonctionnel optimal était défini ainsi : IPSS ≤ 7 et QdV ≤ 3 à 3 mois postopératoire. Plusieurs modèles ont été sélectionnés. La capacité de discrimination était évaluée avec l’aire sous la courbe. Le nomogramme le plus performant a été choisi à l’aide de l’indice de Harell et de l’analyse des courbes de décision.
Résultats
Les critères préopératoires suivantes ont été inclus dans le nomogramme final : âge (médiane 69 ans), volume de la prostate (médiane 50 ml), score IPSS (médiane 18), Débit maximal (Qmax) < 5 ml/s, antécédent de rétention urinaire. La meilleure capacité de discrimination a été obtenue avec la régression logistique binaire. L’indice de Harell (précision du nomogramme) a été mesurée à 0,78 (IC 95 % = [0,71–0,84] soit une précision de presque 80 %.
Conclusion
Le laser Greenlight est une technique chirurgicale en plein essor dans le champ de l’HBP. Il existe des indications de la PVP déjà définies par la littérature et le CTMH. Par ailleurs, le nomogramme que nous proposons permettrait aux urologues de sélectionner les meilleurs candidats à cette technique en définissant à l’avance les résultats fonctionnels attendus (Fig. 1).
Les troubles urinaires du bas appareil, LUTS, TUBA, bien qu'il s'agit des troubles dites fonctionnels, ont un impact majeur sur la qualité de vie, volet sociale ou le volet intime.
Des nouveaux traitements existent, avec un rapport bénéficie risque très séduisant.

Recomendações da OMS sobre cuidados maternos e neonatais para uma experiência pós-natal positiva.
Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.

plus de 70 000 visiteurs, il m’a permis
5 ans après de publier ce livre chez
Editions Leduc.s, écrit dans la même
logique que le livre.
Je suis toujours à la recherche des
auteurs qui souhaiteraient publier des
articles destinés aux patients.
